Attend Person Centered Planning (PCP) meetings as the Behavioral Specialist.

Initiate referrals to external resources with community agencies such as Adult Protective Services, Housing Authority, or public utility companies. Advocates with these entities for purposes of maintaining community stability.

The SocialWork Designee in Training program provides an individual the opportunity to enter the socialwork field in a career that offers both fulfilling work by helping others and upward mobility in the field through continuing education and real-word training with an experienced socialworker.
Position: SocialWork Designee in Training
The SocialWork Designee in Training will learn how to execute resident assessments, care management, care planning, addressing psychosocial concerns, and may serve on the Behavior, Pre-Care Plan, and Care Plan committees.
Principle Duties and Responsibilities:
The SocialWorker Designee in Training will perform the following duties listed and others as required to improve the quality of life for residents. Duties and responsibilities may be added or modified at any time.
Responsible for exercising discretion and independent judgment to assist the management of the Social Services Department
Will encourage residents through counseling and other interventions to achieve their highest level of psychosocial interaction
Will implement person-centered care plans for residents and will monitor and follow-up on directives as outlined in the resident's plan of care
Counsel residents/families regarding psychosocial, adjustment, and behavioral issues.
Participate in Behavior Management Team meetings to monitor resident behaviors and psychotropic medication use
Participate in interdisciplinary care conferences with residents and families
Assist with resident discharge planning
Performs other duties as required.
Working Conditions:
Nursing home setting. Lifting up to 50 lbs. when needed. Prolonged standing or walking, bends, lifts, carries, pushes and pulls various equipment and residents throughout the workday. Extended, unusual, or mandated working hours when required.
Requirements:
This professional agrees to complete an online Social Service Designee Program sponsored by the facility to obtain a certificate that is required by regulation within the required time frame. In addition, the position requires that the professional will continue their education in socialwork with a long-term goal of obtaining a Bachelor's in SocialWork. Ability to give instructions, organize time and complete tasks with minimal supervision. Ability to manage multiple requests and projects simultaneously. Excellent written, verbal and interpersonal skill necessary.
Experience:
A minimum of 1 (one) year as a Certified Nursing Assistant. The ability to apply necessary skills and techniques in a medical setting.
Education:
An Associate's Degree, Bachelor's Degree, and or college courses in psychology and sociology are preferred.
