Staff Training: Provide training to staff on behavior strategies, positive interaction techniques, structured routines, coping skills training, and redirection strategies to ensure high-quality care.

Attend Meetings: Actively participate in meetings as needed to discuss client progress, program updates, and team goals.
Clinical Consultation: Provide consultation on clinical care policies and procedures with providers and the treatment team, ensuring compliance and effective implementation.
Medication Coordination: Work closely with psychiatrists and other medical professionals to ensure medications are coordinated with the client's treatment plans and overall care.
Program Implementation: Model and monitor the consistent implementation of treatment programs, ensuring adherence to best practices.
Progress Reviews: Conduct and provide detailed quarterly progress reviews for clients, ensuring updates on their development and treatment goals.
Relationship Building: Maintain a positive, professional, and collaborative relationship with all management, staff, and treatment team members.
Additional Tasks: Complete other tasks as assigned by the Clinical Director to support program objectives and client success.
Qualifications:
Education: Degree in Applied Behavior Analysis, Psychology, or a related field.
Certification: Must be Board Certified as a Behavior Analyst (BCBA) or meet the Behavior Analyst Certification Board (BACB) guidelines for Qualified Behavior Health Professional (QBHP).
Experience: Minimum of 1 year of experience in providing ABA (Applied Behavior Analysis) services is required.
Preferred Experience: Experience working with adult individuals with autism or other developmental disabilities is preferred but not mandatory.
Skills: Strong communication and interpersonal skills, ability to work collaboratively with multidisciplinary teams, and experience in data collection and analysis for behavior interventions.

The Social Service Worker under the supervision of the Center Manager plans, organizes and implements social services to Senior Community Care participants and families. Responsibilities include but are not limited to: assessment, treatment, teaching and counseling to participant, caregiver or other appropriate representatives. The Social Services Worker interventions could include individual participant contacts; appropriate collateral contacts; participant and family education, assessment and counseling; provision of resources; ongoing case management; advocacy to ensure participant and caregiver needs are met and addressed; and disenrollment procedures. The Social Services Worker is the liaison between the Interdisciplinary Team (IDT), caregiver representatives, and community agencies.

Masters' Degree from an accredited school of social work required

Essentials:
Performs in person initial assessments for enrollment of potential Senior Community Care participants to obtain a complete psychosocial history, which may include descriptions of cognitive status, social supports, family dynamics mental health and substance dependency and other issues and needs. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
Conducts in person re-assessment of enrolled participants every six (6) months and as needed.
Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participates in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
Provides ongoing support, counsel, and education to participants and family regarding a variety of issues, including but not limited to: the aging process, dementia, grief and loss, end of life, disease processes, difficult family dynamics and changing roles, PACE model and PACE health services.
Presents requests to Interdisciplinary Team for and coordinates admission/discharge to contracted facilities for temporary respites and permanent placement.
Acts as facilitator for meetings with participant, family, caregivers, and community agencies to clarify, or problem solves issues regarding the plan of care. Mediates discussions between all parties.
If hospice care is appropriate actively provides emotional support, grief work, education and funeral/financial planning referral. Facilitates hospice or nursing home placement as needed. Initiate referrals to external resources with community agencies such as Adult Protective Services, Housing Authority, or public utility companies. Advocates with these entities for purposes of maintaining community stability.

Position Duties:
Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
Complete psychosocial assessments within 72 hours of admission.
Responsible for interpreting, supporting, and executing hospital and department policies, and procedures with regard to the provision of care in the social services setting.
Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems.
Works closely with all members of the health care team in development of the Master Treatment Plan, based on the psychosocial assessment and accepted social work interventions.
Complete the Master Treatment Plan within 72 hours of admission.
Complete an update to the Master Treatment Plan within 7 days of the completion of the Master Treatment Plan. The purpose of the update is to outline progress towards goals listed in the Master Treatment Plan.
Provides group and family process-oriented therapies as well as conducts educational and other didactic groups for patients and family members using various professional treatment modalities. Provision of these services includes the prompt completion of ongoing progress notes.
Provides social work services including short-term individual counseling, community resource planning, and crisis intervention.
Develops and coordinates an individualized discharge plan for the patient by consolidating treatment team interactions and chart information to determine the patient's aftercare needs.
Attends regular treatment team meetings to provide social work perspective to total case management of the patient by reading and discussing written progress notes from charts and communicating any state or local agency legal requirement for case management with the interdisciplinary team.
Develops and maintains thorough knowledge of referral resources throughout the service area.
Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
Maintains up-to-date knowledge of Medicare, Medicaid, and other third-party reimbursement sources.
Maintains accurate and timely records, including clinical and progress notes, according to department policy. Completes all necessary filing by the close of business each day.

Key Responsibilities
Provide compassionate, person-centered care by addressing clients' needs with professionalism, empathy, and creativity, while delivering high-quality services in accordance with professional standards and the agency's policies and procedures.
Lead and facilitate daily therapeutic groups within the FACE to FACE Program, ensuring adherence to professional standards and facility guidelines.
Serve as the primary point of support for group facilitation, fostering a safe and effective therapeutic space for consumers.
Implement individualized treatment plans developed by Associate and Lead Therapists, utilizing a person-centered approach to meet each consumer's unique needs.
Assist with discharge planning and care coordination to support seamless transitions for consumers.
Maintain accurate and timely documentation in compliance with clinic policies and procedures.
Maintain consistent communication with clinical and administrative staff.
Demonstrate and maintain stable, professional, and appropriate boundaries with clients at all times.
Appropriately manage and resolve crisis situations all while maintaining professionalism and ensuring client safety.
Attend and participate in team meetings and contribute to a collaborative work environment.
Exhibit a positive attitude toward learning and adaptability in a dynamic work environment.
Demonstrate ability to accept constructive feedback, implement guidance, and show a willingness to grow and improve professionally.
Stay current with professional training requirements.

This role requires extended periods of remaining in a stationary position during HIPAA-compliant in-person sessions and requires the ability to perform physical activities such as bending and reaching. Requirements
