About the Role:

As a Field Care Manager - Behavioral Health, you'll play a vital role in supporting members' overall wellness by assessing needs, developing care plans, and connecting individuals with the right health and community resources. This is a remote position with 15-20% local travel to visit members in Wayne and Macomb Counties, Michigan.
What You'll Do:
Manage a caseload and conduct behavioral health assessments in members' homes, community settings, or by phone.
