When people need intensive, comprehensive treatment for psychiatric and chemical dependency issues but do not need inpatient or residential hospital care, Natchaug offers intensive, structured clinical treatment in programs that may include up to six hours of treatment a day, five days a week. These intensive treatment programs can help prevent the need for hospitalization. They can also help people make the transition to home after inpatient or residential treatment. Natchaug brings this clinical care closer to people's homes by offering programs throughout Eastern Connecticut. Job Summary Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Adult outpatient department. Clinical services include individual, group and family therapy. Key responsibilities of Behavioral Health Clinician: Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning.
