How to get a pay stub from a previous employer

In certain instances, you may be referred to the company's payroll or accounting department to get your previous pay stub.

You also may have to complete the necessary paperwork, like a pay stub request form. Fill this out and send it back to your previous employer to receive your pay stub in a timely manner.

Some companies may require you to create your own pay stub request form in writing. Check with a representative from the company as to what information they would like to appear on this request. In almost all cases, you will need to include:

• Your full legal name

• Your current address (and your previous address if you've moved since being employed at the company)

• Your SS#

• Your previous employee identification number

• The dates or pay periods you are seeking to get the pay stub(s) from

You may email them this document or provide it in person if you feel it will quicken the process.

If your previous employer does not honor your request for a pay stub, you may file a complaint with the state or federal labor department.
