We provide the most comprehensive range of surgical and medical services for cardiac care in South Texas. Our facility houses: 6 cardiac catheterization labs, 3 cardiovascular surgery suites, Cardiac rehabilitation unit, and Cardiovascular Intensive Care Unit.
Responsibilities:

The Clinical Team Manager is an RN position who performs management functions in coordination of all clinical disciplines including: skilled nursing, home health aide, physical therapy, occupational therapy, speech therapy, and medical social services. The Clinical Team Manager coordinates patient care on all activities and assumes responsibility for continuity, appropriateness and quality of services delivered within the home health department. The Clinical Team Manager assists with the day-to-day scheduling of staff, monitors performance of staff, assists in the orientation of staff and serves on committees to improve operations. The Clinical Team Manager performs all functions in a professional and ethical manner and collaborates with other members of the health care team to ensure the provision of quality care. The Clinical Team Manager is responsible for verification of appropriate incoming orders obtaining physician orders and completion of orders. Receive and respond to all incoming calls from referral sources or potential patients, exchange information to identify patients' needs, and determine the Company's ability to meet them. We are seeking an experienced and compassionate Hospice Clinical Liaison to join our team. In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services.
Essential Responsibilities
Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals
Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families
Educate patients and families on what hospice care includes, how it works, and why it matters-providing clarity, reassurance, and compassion
Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met
Serve as a liaison between referral sources and our hospice care team-ensuring seamless handoffs, clear communication, and rapid response to needs
Make daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians
Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services
About You:
Education
• Licensed Nurse with three to five years of hospice experience required.
• BSN with five or more years of hospice experience preferred.
Experience
• Minimum of three years of hospice experience required.
• Prior experience as a discharge planner, case manager, or educator preferred.
Licenses and Certifications
Current RN license to practice nursing in the state of employment.
CPR certification.
Valid driver's license and automobile insurance.
Skills and Knowledge
Strong knowledge of hospice care and the unique needs of patients and families.
Familiarity with community resources and referral networks.
Excellent patient assessment and documentation skills.
