Trainings * Attend and successfully complete all mandatory trainings/meetings required by DDS and the agency. * Successful completion of DDS Medication Administration Certification. The Bridge Family Center is currently searching for a full time Transitional Living Coordinator to provide overall case management, behavior management, and life skills instruction for its Moving On Project, a transitional living program for DCF involved adolescent males. Conduct apartment spot checks. III. Program Management Assist with intakes and the discharge of participants in the program, which includes data entry of required information for tracking purposes. Provide crisis intervention counseling as necessary. Transportation of clients, as required. Prepare monthly report of caseload for DCF as per assigned deadline, which includes summary of each individual relative to life skills, money management, interpersonal skills, etc. Administration of medication; maintenance of appropriate documentation. General maintenance of daily/shift documentation. Ensure program compliance at all times. Participate in weekly team meetings to review each participant and determines appropriate courses of action. Provide adequate supervision at all times (minimum ratio 1:6), to ensure resident support and safety. Demonstrate positive rapport with residents, including establishing professional boundaries. Exercise effective group skills and techniques. Other duties as assigned. Qualifications: Bachelor's Degree in Human Services or related field highly preferred. Experience working with adolescents as well as case management experience required.
