Enter/exit structures, crawl spaces, climb over and on top of structures, ascend and descend stairs and ladders.
Must deliver premier customer service to resolve pest issues.
Read and interpret pesticide labels to correctly mix products based on areas measured and quantities needed.
Record work activities and complete all paperwork after finishing services.
Perform residential pest management service routes with the highest level of customer service, professionalism, and efficiency.
Complete all duties required by a pest/mosquito/termite service professional: customer consultation, inspection, identify/eliminate conducive conditions and perform proper treatments.
Maintain and care for company equipment, vehicle and gas card and other job-related items.
Address and alleviate customer concerns and pest related issues by using integrated pest management strategies

Consultations and referrals are based on procedure and disease requisites and are clearly documented in the electronic health record (Epic).
Provides patient teaching and counseling aimed at illness prevention, anticipatory guidance, harm reduction, and accident risk factors related to health using guidelines set out by the clinic and other professional organizations.
Provides health education and supports wellness programs and health promotion services that meet the homeless population's unique needs.
Collaborates with the Medical Director and providers to monitor and improve processes and outcomes of care.
Documenting required clinic measures such as UDS, MU, HEDIS, etc., supports the integrity of data collection.

Leadership:

Qualifications:

The Medical Provider position delivers primary/urgent care in an ambulatory care clinic and mobile setting 100% of the time.
Duties and Responsibilities:

Core Responsibilities:
Provides comprehensive, trauma-informed, high-quality primary care medical services to homeless patients of all ages, with the exclusion of prenatal and obstetric care.
Conducts history and physicals, develops differential diagnoses and treatment plans, writes prescriptions, administers prescribed treatment, and dispenses medications: performs diagnostic tests, reviews and interprets laboratory findings.
Consultations and referrals are based on procedure and disease requisites and are clearly documented in the electronic health record (Epic).
Provides patient teaching and counseling aimed at illness prevention, anticipatory guidance, harm reduction, and accident risk factors related to health using guidelines set out by the clinic and other professional organizations.
Provides health education and supports wellness programs and health promotion services that meet the homeless population's unique needs.
Collaborates with the Medical Director and providers to monitor and improve processes and outcomes of care.
Documenting required clinic measures such as UDS, MU, HEDIS, etc., supports the integrity of data collection.
Demonstrates knowledge of the electronic health record and standards of proactive plus effectively communicates with healthcare teams.
Understand how to access and adhere to clinic protocols, policies, and standardized procedures as appropriate.
On-call consultation duties require availability by phone and are rotated amongst the provider staff approximately one week per month or less.
Precept Residents, Physical Assistants, APRNs, medical students, and other health care professionals in family practice and primary care.
Complies with the Utah Code and the Consultation and Referral Plan. A Copy of the plan is to be kept on-site.
Participates in quarterly peer review process.
Attend and participate in the weekly provider and all staff meetings.
Leadership:
Supports the Medical Director in establishing medical policies.
Collaborates with multidisciplinary teams to ensure appropriate services are provided within the clinic's guidelines and capabilities.
Qualifications:
The Family Practice/Internal Medicine/Dual Medicine/Pediatrics MD is a graduate of an accredited program who must be nationally certified within one year of completion of the program. The MD maintains national re-certification every two years and is licensed in the State of Utah with a current unrestricted Utah physician, DEA number, or eligibility, and NPI number.
Must have Suboxone waiver for Medication-assisted treatment.
Maintains current professional licensure.
