What You Will Do
Understands and discusses the client-defined healthcare and functional goals and anticipated post-discharge care needs to ensure a smooth transition home or to other level of care setting.
Works closely with the Care team to monitor and track client progress. Prepares detailed, accurate and timely documentation.
Facilitates periodic case record reviews and case conferences with the Care team for assigned clients.
Establishes and promotes an ongoing collaborative relationship with direct caregivers, hospital administration, physicians, nurses, discharge planners, case managers, social workers, unit clerks, homecare agencies and other facility personnel.
Documents all interaction with the interdisciplinary team in the EMR.
Follows up with clients and maintains expected timeframes as per workflow.
Qualifications
Licenses and Certifications:
Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs
Education:
Associate's degree in human services or a related field or the equivalent work experience, required
Bachelor's degree in a human services or related field, preferred
Work Experience:
Minimum of two years of customer service experience in the medical field (i.e., hospital, long term care, home care, medical office), required
Ability to adapt to change and work in a team-based environment, required
Strong follow up skills required, as well as the ability to manage multiple priorities, required
Demonstrated ability to educate clients, conduct phone interviews/coaching, and coordinate services for multiple chronically ill patients, required
Must be organized and detail oriented
Experience entering data, navigating, and retrieving information through computer systems, required
Proficient computer skills in Microsoft office, preferred
Bilingual skills may be required, as determined by operational needs
Pay Range
USD $25.46 - USD $31.86 /Hr.

The Position:
LMI is actively searching for an Associate to join the Direct Lending team. This individual will be an integral member of the leveraged financed platform within the Global Credit Markets business. Dedicated to the Direct Lending strategy, this Associate will have responsibilities that provide for significant ownership of the end-to-end private credit investment process including sourcing, underwriting and ongoing portfolio monitoring of private loans.
Responsibilities:
* Contribute as an integral member of the team to invest in private debt financings across the capital structure, with an emphasis on unitranche, first lien, and second lien loans.
* Assist in quarterbacking the investment process from start to finish, working closely with Team Leaders and Credit Research to conduct company and industry research, develop investment recommendations, and to manage loan positions post-closing of the initial investment
* Contribute to the preparation of the investment committee memorandums (complete with financial projections) and support Team Leaders in the presentation of the team's investment recommendations to internal investment committees
* Support Team Leaders in the evaluation, structuring, and negotiation of economic and legal terms at both the term sheet stage and during final documentation
* Actively monitor the performance of existing investments and maintain relationships post-close with key investment participants, including the Borrower / Sponsor and Agent; provide detailed updates to team members during scheduled portfolio reviews and as needed on an ad-hoc basis
* Collaborate cross-functionally with additional LMI teams including Legal, Finance, Accounting and Risk Management to execute transactions and manage the portfolio
* Engage externally with other market participants (e.g. other direct lenders and buyside investors, bankers, advisers, and third-party consultants) as part of the overall investment process
* Support relationship coverage with private equity firms across the lower, middle, and large market, as well as other deal sources to help meet the team's origination targets
* Take an active role in the training, development, and coaching of Analysts within the team
Qualifications
* 5+ years of investing experience, including a minimum of 3+ years in the direct lending / private credit space
* Previous experience in underwriting and monitoring credit investments
* Proven ability to interact with, assess, and build relationships internally as well as externally, with Borrowers, Sponsors and other deal sources
* Previous experience with credit agreement documentation and or other relevant legal documentation
* Ability to identify company, industry, and market risks and opportunities, and to evaluate risk mitigation strategies
* Excellent communication skills (written and verbal) with an ability to clearly articulate relevant key learnings and insights derived during the due diligence process
* Self-starter with a willingness to be flexible and take on additional responsibilities depending on the needs of the business

Maintain inventory of office supplies. This includes: completing approved purchased orders,

The Position:
LMI is actively searching for an Associate to join the Direct Lending team. This individual will be an integral member of the leveraged financed platform within the Global Credit Markets business. Direct Service Provider (DSP) Bilingual Mandarin
Salary Range: $16.52-$19/hr Part Time, Flexible Hours Approx 6 hours a week
Essential Duties and Responsibilities:
The role of a DirectSupportProfessional (DSP) is to work with children and their families to provide resources and support, as well as strengthen daily living skills. Applicants can choose to work in our OWPDD program with individuals who have developmental disabilities or in our HCBS program with individuals facing mental health challenges. The services are provided in the client's home or community setting giving the families respite while gaining important life skills.
Activities as a DSP for the OPWDD program recipients primarily involve teaching them the following: how to accomplish daily living tasks, improve socialization among peers, and acclimate to community activities (i.e. traveling, interacting with store staff/community helpers, and adhering to appropriate social boundaries in various settings).
Assist individuals in meeting their valued outcomes as outlined in their service plan.
Set schedule according to the needs of the client and the DSP.
Communicate effectively with families and supervisors.
Problem solve and communicate with supervisors as concerns arise.
Complete and submit documentation detailing the activities engaged with the individuals in a timely manner.
Bilingual in Mandarin
Complete mandatory trainings.
Education and/or Work Experience Requirements:
Prior experience providing direct services to individuals with a developmental and/or intellectual disability OR pursuing a degree in education, social work, nursing, or psychology.
Computer proficiency (MS Office Word, Excel and Outlook).
Can communicate in either Mandarin or Cantonese as well as English.
Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
Applicants must be at least 18 years old and legal to work in the US.
High school diploma or GED required, Associates or Bachelor's Degree preferred, in some cases a Bachelor's Degree may be required.

Ability to safely and successfully perform the essential job functions consistent with the ADA, FMLA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
