Position Summary
The Day Habilitation Specialist (Day Hab Specialist) will assist individuals with developmental disabilities to maximize their personal and vocational potential in their center and community. The Day Hab Specialist provides direct care support and assists in the implementation and documentation of each individual's program plan. Day Habilitation Specialists for designated programs may be expected to support other programs based on staffing and attendance. Floating Day Hab Specialists will be assigned based on need. The following position standards apply to both regular and floating Day Hab Specialists.
Full time schedule: Monday - Friday 7:45am - 3:30pm
Salary Range:
Day Hab Specialists I (drivers)- $17.90 to $19.90 per hour
Day Hab Specialists II (drivers)- 18.41- 20.41 per hour
Note: The rates outlined above are contingent upon being deemed eligible to drive by our insurance carrier. Should you be deemed ineligible to drive and the program is able to make a non-driver accommodation, a lower rate of pay will apply.
Position Standards:
Delivers and documents the services, supports, safeguards, behavioral documentation and activities identified by ISP/CFA and the Day Habilitation plan. Completes the monthly note and ensure it covers the individual's response to service within the seven days of the following month for assigned individuals.
Performs individual specific personal care as required, including but not limited to assistance with toileting, transferring, changing needs, handwashing and oral care. Documents any finding on appropriate forms including but not limited skin integrity, bowel and repositioning charts.
Ensures active engagement and appropriate communication with staff and individuals.
Assumes responsibility for medication administration and appropriate documentation in an accurate and timely manner.
Develops a trusting and friendly rapport with individuals served while maintaining professional boundaries.
Fulfills mandated reporter role by interrupting and reporting suspected abuse to administration immediately.
Ensures that the center is kept safe, clean and decorated in an age appropriate and comfortable manner and that is kept in good repair inside and out.
Demonstrates proficiency in all goals and skillsets outlined in the NYS DSP Core Competencies, on which performance will be evaluated.
Responsible for providing transportation to individuals receiving services driving agency vehicles with a capacity up to 14 passengers.
Ensures all required trainings and certifications are current, and seeks further support and training where needed.

SF2 also focuses on the child development of the child(ren) through various assessments and the Parents as Teachers curriculum. GENERAL DUTIES AND RESPONSIBILITIES
* Provides home visits to assigned families where children may have been identified through program screening criteria as being "at risk" for abuse and/or neglect.
* Provides home visits to assigned families who are identified as needing services including but not limited to parenting and child development education, advocacy and support and assistance with accessing community resources.
* Makes home visits based on the needs of families.
* Provides families with information that includes but is not limited to child development, parenting, safety, immunizations and community resources.
* Utilizes program-approved curriculum to improve parenting skills and increase the understanding of normal child development.

Little Flower Children and Family Services of New York is a human services nonprofit supporting the well-being of children, youth, families, and individuals across New York City and Long Island since 1929.
Our staff of more than 500 builds well-being by providing prevention services, foster care, residential treatment care, adoption services, programs and services for individuals with developmental disabilities, and medical and mental health services.
Program Summary
The Community Habilitation Program provides personalized habilitative services that empower consumers to direct their own support. Specialists partner with individuals to strengthen life management skills, promote self-advocacy, and expand meaningful participation in the community.
Principle Responsibilities
* Ensuring the safety and well-being of individuals in all settings
* Providing supportive companionship and serving as a positive role model
* Building trusting relationships with individuals and families
* Using clear, professional written and verbal communication
* Completing all service documentation within twenty-four hours to support compliance and billing
* Providing transportation to appointments, activities, and community outings as needed
* Supporting individuals to achieve their identified goals through skill development and encouragement
* Communicating promptly with supervisors, individuals, and familiesregardingschedule changes, lateness, or absences
* Supporting good personal care habits based on individual needs Identifying and reporting health or safety concerns to supervisors Participating in the development and implementation of service documentation and goal plans
Required Qualifications and Experience
* High school diploma or GED
* Valid New York State driver license
* Ability to travel to consumer homes and community locations
Preferred Qualifications
* Experience supporting individuals with intellectual or developmental disabilities
* Experience providing coaching, mentoring, or life-skills training
* Ability tomaintainprofessionalism, integrity, and responsibility in all interactions
* Demonstrated ability to promote physical and emotional well-being
* Ability to build and sustain positive working relationships
Salary Range
The salary range for this role is $18-20 per hour and will be calculated based upon education and experience.
Travel Requirements
This position offers a per diem, in the community schedule that requires travel throughout Suffolk County. It may require occasional work in our Wading River office.
