CASA provides volunteer advocacy to children involved in dependency and neglect cases and FC provides supervised visitation, safe custody exchanges, supportive case management, and parenting education. TBRI is integrated throughout CLC's programs and provides evidence based, trauma-informed, intervention. Across all programs, our mission is to advocate for safe, nurturing, and permanent connections to family and community so children who have experienced abuse and neglect have the opportunity to thrive.
Reports to: Program Manager
Full Time (40 hours/week)
Position Overview: The Family Connections Family Case Manager provides case management to strengthen families who participate in Family Time, parent coaching and exchange programs provided by CASA of Larimer County. The Case Manager is responsible for supporting families in a manner that is trauma-informed, culturally sensitive, and upholds best practices that lead to positive outcomes for children while keeping victims safe. This position is scheduled Sunday through Thursday and includes evening hours.
CORE RESPONSIBILITIES:
Case Management
Provides strengths-based support to caregivers and children on their assigned cases through direct, consistent and effective communication with caregivers, children, and related parties.
Provides Trust-Based Relational Intervention (TBRI ) lessons to caregivers to increase parenting knowledge and skills and improve the quality of connections developed with the child(ren).
Assists all families in scheduling and maintaining a consistent Family Time/exchange schedule to promote the best opportunity to strengthen family connections.
Supervises Family Time and exchanges with a focus on the child's physical, mental, and emotional safety.
Addresses concerns regarding child safety and Family Time compliance with urgency.
Clearly documents Family Time and any communication with family members in the case file and provides documentation to the courts as needed.
Updates and maintains case files accurately and to program standards.
Initiates and maintains communication with other agencies and parties to the case as appropriate.
Ensures strict confidentiality of agency and client information.
Assists families in accessing step-down services or in the transition out of the program.
Informs the Program Manager of changes and concerns on all cases.
Provides referral information to families for community resources, trauma information, and victims' rights/compensation information.
Tracks all resources and lessons provided, the progress of family connections over time, and additional metrics towards programmatic goals.

Helps to maintain conditions to promote a welcoming, safe space for families.
Reports safety concerns immediately and reports all maintenance issues in a timely manner.
Provides coaching to interns in case management duties.

Position Summary
The Youth Empowerment Specialist position will provide leadership and coordination for the development and implementation of a team of young adults with lived experience to provide interim youth voice to interested local system of care collaboratives and work with those collaboratives to identify strategies for increasing local youth participation through targeted awareness and engagement campaigns. The Youth Empowerment Specialist will provide guidance to the young adult leaders, to provide support to local SOC Collaboratives, to provide local youth engagement training, and to provide local youth leadership and advocacy training. Essential Duties and Responsibilities:

Qualifications and Skills:

Current BLS/CPR certification required
Must have valid driver license and appropriate liability insurance
Current TB test and maintain a negative test annually required

DEPARTMENT OVERVIEW
The Long-Term Foster Care (LTFC) program at Lutheran Family Services Rocky Mountains (LFSRM) provides care and support to Unaccompanied Children (UC) in federal custody as they wait for their immigration case to be resolved so they can transition to a more long term program or living arrangement. UCs are placed with licensed foster families and attend public school while they are in the program, allowing them to have more normalized experiences in family based environment.
JOB SUMMARY & RESPONSIBILITIES
The Placement Supervisor is responsible for the certification of foster care homes and the placements of UCs in foster homes. This position will provide direct supervision to the members of the case management team. Additionally, this position will be expected to provide casework and supportive services directly to a small caseload. This position will be required to maintain communication with stakeholders in an effective and timely manner to ensure case continuity. The Placement Supervisor is required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours and weekends.
REQUIRED COMPETENCIES

Reviews and signs home assessments including updates, renewals, and addenda; issue certificates; and post- placement reports.
Oversees appropriate medical services for the children placed in care.
Provides and documents monthly supervision of placement workers to review all home studies in process, certifications in renewal status, and current placement activity.
Assesses potential placements based on information provided by counties and other referral agencies.
Accompanies placement workers to pre-placement and placement visits between foster families and children, when possible.
Conducts file reviews of foster parents and foster children as scheduled.
Trains new employees on the placement of children, monthly paperwork, and job duties.
Provides crisis intervention when needed.
Supports the recruitment, retention and training of foster parents, as needed.
Performs the job duties of a case manager worker when carrying a caseload.
May participate in on call rotation.
TRANSPORTATION
Must maintain a valid driver's license and carry personal auto liability insurance at the level of $100,000/300,000/100,000.
Frequent local regional travel. May be required to transport clients in personal vehicle.
REQUIRED CERTIFICATIONS
A Bachelor's degree in the social or behavioral sciences and three (3) years of full-time child placement experience or Master's degree and one (1) year full-time experience in adoption or foster care and child placement.
Possess certification in the SAFE assessment tool and SAFE supervisor certification, or the ability to become certified within 3 months of hire. Meets regulatory requirements for Placement Supervisor per state regulations for Child Placement Agencies.
Bi-lingual English and Spanish strongly preferred.
