Family Support Specialist Utilizing personal vehicle, the Family Support Specialist provides transportation services, courier and administrative support to clients, case managers, supervisors, ADOP's and DOP, agency and its geographical area. Serves as a primary support to the case managers with administrative task.
Job Summary
Provides approved transportation to clients when requested (drives personal vehicle 90% of the time)
Transports children to various appointments and locations during hours indicated on the referral form.
Maintain appropriate and respectful communication with co-workers and leadership.
Respond to emails within 24 hrs., respond to text or phone calls within the hour from assigned supervisor or leadership.
Attends and participates in weekly and monthly meetings with staff and training sessions as required by the specific job or department
Observes and submits accurate written documentation on the service provided and the functioning of individuals and families as required by specific job or department
Serves as a liaison with collaterals such as DCF, schools, etc.
Shadows an experienced transporter within the first 30 days of hire.
Submits expense vouchers, renewed driver's license, and automobile insurance coverage on due date
Communicate any barriers to completing any request immediately to ensure other arrangements can be made. This should be communicated via email, text or face to face with the assigned supervisor.
Works cooperatively with other agencies, caseworkers, transporters, and family support services
Works in collaboration with Office Assistant and fellow Family Support Specialist to ensure the needs of the agency and clients are met for transportation; coordinates with assigned supervisor, client and caregivers to facilitate the transport
Ensure safety of the child during transports.
Courier documents to courthouse, agency locations, attorneys or other required areas and obtain signatures of completed deliveries when required.
Supervise and document parent/child/ visitation. Provide a written summary of the visit to the assigned case manager supervisor within 48 hrs of the visit. Immediately report any child safety concerns to the supervisor.
Maintain mileage log and documentation reports in a timely manner to receive authorized travel reimbursement for services provided.
Assist in file maintenance when requested, this will include preparing documents to be filed in the physical case file.
Must be able to lift, carry, and move a 25 lb child in a car seat
Required Qualifications Education: High School diploma/GED
Experience: up to 2 years' experience with transportation; 1-2 years in a childcare field
Other: Reliable transportation, valid driver's license and acceptable driving record with OHU required levels of coverage for auto insurance, geographic knowledge of the area

Role Specific Duties:
Maintain a caseload of 3-6 housing clients and 10-15 outreach clients.
Provide regular supervision to youth housed in the emergency hotel and assist them in obtaining more stable housing.

* Identifies and evaluates child abuse resources and obtains information relevant to families who are caring for a child abuse victim.
* Serves as an authority and provides expertise in the area of child abuse to physicians and other professionals.
* Provides specialized medical casework services to a caseload of 30 to 60 children per month, including intake screening, compiling all pertinent information and social history, and assessment of data needed for medical evaluation and safety planning.
* Represents the unit on at least one community child welfare committee and serves as a liaison to select DCFS offices in the central region.
* Foster professional and positive relationships with all agencies involved with the Pediatric Resource Center.
* Perform other related duties and participate in special projects as assigned.
Minimum Qualifications
* Bachelor's degree with a major in Social or Behavioral Science or related Health specialty from an accredited college or university with 5 years of relevant experience and significant specialized training in child protective services
* Or Master's degree in social or behavioral science or related health specialty area from an accredited college or university with two years of progressively more responsible full-time experience in the practice of social services or medical/clinical setting maintaining acceptable standards of social work practices.

Essential Functions:

Assists the caseworker with facilitation of community resources to prepare the home of parent.
Assists the caseworker with the post adoption/guardianship connection to the DCFS unit prior to finalization.

When reunification is not possible, assists the caseworker in listing children for adoption with Greenlight or the applicable Adoption registry.

Provides linkages between caseworkers, recruitment resource specialists, adoptions staff and community providers to expedite and finalize the permanency process and ensures all necessary actions and reviews have taken place to facilitate positive permanency outcomes
Completes and maintains client records and notes relative to achieving permanency with youth in care, compiles reports and other required documentation relative to achieving permanency according to agency guidelines for completeness, accuracy, and timeliness.

About Position:
On a daily basis, a Mobile Crisis Response Counselor will be available to complete psychiatric screenings and communicate with clients, families, and other mental health professionals the need for hospitalization, or the resources that will support the client and client's family in the community. The ability to understand the hospital admission and transportation process to the many hospitals in the state is essential for this position. Distinct challenges with this position include being able to problem solve in a variety of crisis situations, prioritizing tasks and resolving critical matters, and communicating effectively with clients, their families, and community partners. A Crisis Counselor provides crisis intervention services through a variety of services, including the MCR Crisis Program, the Community Trauma Team, and through the Community Crisis Line. A Crisis Counselor needs to project a positive image of the program and agency and use good judgment. Completion of documentation will need to be done on a daily/weekly basis.
A Crisis Counselor provides the client, the client's family, or mental health professionals with a sense of direction and/or hope in a crisis situation. This position also requires active engagement and participation in the Community Crisis Line Services. Position Summary
