Physical Therapist
JOB SUMMARY
Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include but are not limited to assessment; planning, providing treatment; and teaching participant, caregiver or other appropriate representatives/family to maintain participant support in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provides high quality care and serves as a member of the PACE interdisciplinary team (IDT).
Performs in-person assessments for participants at initial enrollment and as-needed ongoing as determined by the IDT.
Assesses the need for adaptive durable medical equipment, specialized shoes and prosthesis. Ability to assess participant's home for safety and mobility. Recommend home modifications and equipment.
Plans for physical therapy treatment programs according to recognized standards and policies.
Administers physical therapy treatment programs consistent in achieving identified goals and according to recognized standards and policies. Confer with interdisciplinary team on participant care issues.
Documents evaluation, assessment, plan and treatment rendered accurately and in timely according to policy.
Measures effectiveness of treatment, document and revise plan accordingly.
Educates and instructs participants/family in procedures, home programs and document training and understanding by participant/family.
Trains PACE staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing activities of daily living (ADL's).
Provides care and services in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care.
Assists participant to function at most independent level possible.
Develops and maintains positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Assists the interdisciplinary team members in understanding the significant rehabilitation needs of the participant and families
Collaborates with the interdisciplinary team to develop a comprehensive care plan for each participant.
Maintains regular attendance at and participates in daily Interdisciplinary Team meetings, communicates participant changes and collaborates with team members in care planning decisions and coordination for 24-hour care delivery.
Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed.
Actively participates in quality improvement projects and meetings.
Fulfills the role of other IDT member(s) as needed according to licensure and competency requirements.
Attends and actively participates in a variety of organizational meetings related to participant care or daily operations, in-services and community agency meetings.
Completes all documentation of clinical service in participants' medical records, including initial assessments; reassessments; change of status; and other significant events according to PACE documentation requirements.
Follows all Policies and Procedures and OSHA safety guidelines.
Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.
Practices standard precautions.
Maintains a safe working environment, following safety policies and procedures.
Participate in continuing education classes and any required staff and training meetings.
Maintains professional affiliations, required certifications and continuing education requirements.
Other duties as assigned.
EXPERIENCE EDUCATION AND CERTIFICATIONS:
Bilingual and fluent in Hmong preferred
Advanced degree in Physical Therapy from an accredited school of Physical Therapy
Current license to practice Physical Therapy in the state
Current driver's license and proof of auto insurance
Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).

Registered Nurse Care Manager
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 “on-call” process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.

Position Requirements:
5+ years plant management or multi-cell manufacturing operations leadership.
Proven record achieving delivery, quality, cost, and labor efficiency improvements.
Experience with fabrication/welding/heavy industrial or sanitary stainless-steel manufacturing.
Demonstrated success driving non-negotiable operational standards, standard work, and 5S.
ERP experience required (Epicor strongly preferred).
Effective change leadership and communication skills.
Exposure to ASME pressure vessels, food & beverage, sanitary fabrication, or process equipment (highly preferred).
Lean/CI certification (Green Belt or equivalent, preferred).
Experience in an ESOP or highly collaborative ownership culture (preferred).

SUMMARY: The Registered Nurse (RN) is accountable for providing competent nursing care. Responsible for coordination of the patient's plan of care through assessment, planning, implementation, and evaluation of nursing interventions that lead to established outcomes. As the only pediatric rehabilitation program in the region, our expert team offers comprehensive inpatient and outpatient care. Our multi-professional team provides specialized therapies and nursing care to treat conditions ranging from common to the most unusual and complex in patients from birth to young adult.
Our 20-bed unit is accredited as an Inpatient Rehabilitation Pediatric Specialty Program by the Commission on Accreditation of Rehabilitation Facilities (CARF) making Valley Children's one of only two pediatric facilities in the state with this impressive designation, and one of only five on the West Coast. CARF Accreditation is an internationally recognized award that is given to organizations that demonstrate commitment to providing the highest quality care and services.
POSITION REQUIREMENTS: A) a graduate from an accredited nursing program. BSN strongly preferred; B) an active California Registered Nurse licensure (RN); C) Life Support Certifications (see specifics below); and D) experience to correlate with the clinical ladder level. Bilingual skills desirable.
Life Support Certifications required: 1) Heart Code Basic Life Support (BLS) within 30 days and 2) Pediatric Emergency Assess, Resuscitation & Stabilization (Pears) within 12 months of hire or transfer into the position
