    Job Description Supports individuals with IDD in group and individual settings in pursuit of community involvement through leisure, volunteer and recreational activities in the community. Assists and supports individuals to contribute to and participate in community activities, locations and relationships of each individual's choice. ESSENTIAL JOB FUNCTIONS: As part of the team, plans community specific activities that are meaningful and adult appropriate including memberships in associations, opportunities for contributions and the development of relationships. Ensures supports, safety and participations in activities for all individuals in group services. Promotes individual choice in all activities, relationships and services. Schedule of activities is based on choice and interests identified in the ISSP. Ensures individual's health and safety is monitored and protected at all times. Follows operational procedures regarding safety. Reports unsafe conditions. Works with Team to address individual risks. Monitors and administers medications and ensures medical instructions and policies are followed. Implements Individual Service and Support Plans and supports Person-Centered Planning. Assists in identifying each person's strengths, interests, and needs in a proactive manner, by discussion with supervisor and team and ensures that alternative plans are in place to support where efforts are ineffective. Provides documentation of observation related to services and supports, i.e. progress notes, attendance, and incident reports. Ensures support is available to individuals with disabilities to participate in personal interests, community activities, and resources either provided directly or by friends, family, or other community members. Promotes individual choice in all activities, relationships, and services. Assists each person assigned in developing and maintaining relationships with community members. Understands and supports reciprocity in relationships. Participates as a team member to accomplish goals of the team and agency. Performs all other job duties as assigned. QUALIFICATIONS: Demonstrates ethical and respectful interactions; works with integrity and respects diversity. Manages difficult or emotional customer situations; responds promptly to customer needs; solicits customer feedback to improve service; responds to requests for service and assistance; meets commitments. Follows policies and procedures; supports organization's mission and goals in appearance and interactions; and commit to attention to detail. Meets productivity standards; completes work in timely manner; strives to increase productivity. LICENSES OR CERTIFICATES: Maintain current CPR, First Aid certification. Cheyenne Village will pay for any trainings needed. Must be able to operate a motor vehicle as part of your job duties as assigned. Most Cheyenne Village jobs require driving as part of their daily job duties. This requires maintaining a driving record acceptable to the State of Colorado, Cheyenne Village, Inc., and its insurance carrier's requirements. WORKING CONDITIONS: Work is primarily in community settings and transporting with multiple individuals. Environment can sometimes be hectic and noisy to include loud voices. Flexibility and ability to change priorities a must. PHYSICAL DEMANDS: Move about from location to location. Exerts force and/or lifts or carries object up to 10 pounds constantly, 10-20 pounds frequently and 20-50 pounds occasionally. COMPENSATION STATUS: Non-Exempt PAY TYPE: Hourly EDUCATION/EXPERIENCE/REQUIREMENTS: Experience in human service or related field required. Working knowledge of Microsoft Office applications and e-mail. Maintains criminal and driving checks acceptable with State of Colorado, insurance carrier and Cheyenne Village, Inc. requirements.
