Sign-on Bonus Available! The Social Work position for Mobile Health Services provides professional services to clients and their families utilizing the Mobile Health Clinic. This position will help to meet identified psychosocial, emotional, financial and environmental needs. The social worker provides psychosocial assessments, supportive counseling, emergent crisis intervention appropriate to setting, financial resource information, environment enhancements, advance directive planning and referrals to community agencies for clients and their caregivers/families in the mobile health clinic (MHC). Using an interdisciplinary team approach, the social worker ensures clear communication and helps to facilitate holistic care. The social worker identifies and implements interventions at the individual and systemic levels and provides expertise to high risk clients across the continuum. The social worker works collaboratively with the multi-disciplinary team to support the Mobile Health team performing at the highest level of their license in addition to maximizing the social worker's specialized training to address complex cases. Position: Child Welfare Worker
Qualifications: Bachelor's degree in Human Service field, plus two or more years of experience in child welfare. Must have reliable vehicle available for daily use and provide proof of a valid driver's license and vehicle insurance. Must pass the Child Care Act, Foster Home Licensing , CERAP and DCFS Licensure exams within first six months of employment.
Purpose: Perform functions conducive to maintaining children in foster home placements, assist with assessing future homes for suitable placements, and provide supportive services to both parents and children in the home.
Essential Responsibilities:
Monitor child's placement and services for safety, well-being and permanency.
Monitor and supervise foster home settings to ensure that appropriate care is being provided.

6. Functions as a member of the Interdisciplinary Treatment Team, by collaboration and consultation to provide appropriate care. When consulting with the interdisciplinary team, the Substance Abuse Social Worker will exercise good judgement when assessing these situations and making decisions regarding treatment issues on site and by phone.
7. Provides recommendations regarding mental health, socio-economic, and medical needs to the treatment team. The Substance Abuse Social Worker can provide brief intervention, or work with the patient for an extended period of time based on the needs identified.

Christie Clinic's department of OB/GYN is seeking a full-time Licensed Clinical Social Worker(Sign-On Bonus Available) from Monday-Friday, 8:00am-5:00pm at the University clinic. This position will allow for a 4 or 5 day work week and include 36 patient contact hours.
Duties include collaborating with the OB/GYN provider team to implement and manage individualized treatment plans for patients facing a variety of conditions, including but not limited to, mood disorders, anxiety disorders, PTSD, phobias, and addictive behaviors.
A Master's in Social Work and a Licensed Clinical Social Worker license required. 3-5 years of post-licensure social work experience, experience working with perinatal populations and women's health issues preferred.
Job Qualifications and Expectations

Performs mental health evaluations for referred patients, including assessing the need for ongoing psychotherapy or other services.
Provides psychotherapy services for patients in a healthcare setting, using techniques and interventions appropriate to the patients' needs.
Works with the patient to develop treatment plans, including specific goals and objectives to be addressed in psychotherapy.
With permission of the patient, collaborate with referring providers about patient diagnosis, treatment plan, and progress.
Demonstrates strong knowledge of community resources (12 step groups, support services, educational resources, etc.) to ensure appropriate supplemental care or follow up services after termination
Demonstrates knowledge of community resources for higher levels of care when required for mental health stabilization. Knowledge of petitions and referrals to higher levels of care.
Provide educational and informational resources to patients and families to optimize resources and care.
Complete mandated reports of suspected abuse and/or neglect of minors, disabled adults, and seniors. Cooperate and collaborate with the investigating bodies for safe plans.
Visit notes and orders are completed and transmitted in a timely manner according to policy. Corrections to care plans are entered and transmitted according to the timeline.
Maintain information from patients in written and electronic form.
Perform related work as required.
TYPICAL PHYSICAL DEMANDS:
Demands include sitting, standing, walking, bending, stooping, stretching, and lifting up to 75 pounds. Hearing within, or correctable to, normal range, vision correctable to 20/20 and manual dexterity for the operation of office equipment is required.
TYPICAL WORKING CONDITIONS:
Work is performed in a clinical setting with possible exposure to communicable diseases, toxic substances, hazardous waste, bodily fluids and other waste common to a health care environment. Ionizing radiation and medicinal preparations are also common. Requirements may include working irregular hours.

Position Purpose:
Provide continuity of program services in an ethical, legal and moral manner
within a safe and therapeutic environment, consistent with applicable regulatory and accreditation
standards. Participate as a team member in the delivery of addictions and mental health treatment
services to patients and their families and facilitate the issues of recovery into their daily living situations.
Qualifications:
