Community Habilitation offers a wealth of valuable, one-on-one services to children and adults with disabilities. The program offers choices and increased independence while encouraging productivity and shared community involvement. Through a variety of activities geared toward enhancing daily life-skills development, our knowledgeable, creative staff provide families with much-needed assistance by spending time with program participants in their homes and out in the community.
Independent self-starters needed to work directly one-on-one in the community with individuals with intellectual disabilities. Responsibilities include training individuals in the performance of personal hygiene and self-care tasks (dressing, grooming, etc.) general household chores, mobility to maximize independence in personal health care and travel, financial management, appropriate social behaviors. Duties include: following the Staff Action Plan based a person - centered approach, recording and monitoring progress, maintaining appropriate documentation, and assume driving responsibilities.
The ComHab/DSP/CCSM works one-on-one with individuals with intellectual and developmental disabilities by using strategies and community-based activities designed to promote skill building in the areas of communication, socialization, behavior, independent living, movement, etc. CHDSP's are involved in the creation and assessment of habilitative and behavioral plans that they will implement.
The CHDSP demonstrates knowledge of Richmond's quality mission, vision, and values. In addition, this position actively participates in quality improvement projects (i.e. communicating issues, offering suggestions) to ensure ongoing excellence of Richmond residents, staff, and programs.
Requirements:
Associates Degree/Human Services Degree or experience preferred
Minimum of 2 years of experience working directly with Individuals with developmental disabilities
Must have valid drivers license for at least 3 years and successfully complete and maintain Richmond Driver Authorization

Key Responsibilities

SCOPE
South Bronx United (SBU) is seeking energetic individuals with a passion for youth development and soccer to join our team of SBU Coach-Mentors. In the position, the individual will serve as a mentor and Head Soccer Coach for a travel team of boys or girls within the SBU Academy Program. As a mentor, the individual serves as the first point of contact for any issues regarding SBU student athletes on the team and as a positive influence promoting education, character, and community. As a coach, the individual will run twice-weekly soccer trainings and coach the team at weekend games. The coach will have the goal of keeping youth active, instilling fun, and helping youth grow as both players and students.
This is a part time position from September to June requiring approximately 8-12 hours/week during the fall and spring seasons. Hours will decrease during the winter off-season (January-February) to 4-6 hours/week.
RESPONSIBILITIES
1. Serve as a Mentor for a team of approximately 20 youth (mentees):
● Serve as first point of staff level contact for the mentees.
● Help plan, set, and keep students on their program schedule.
● Maintain weekly contact with each mentee in person and via phone.
● Maintain regular contact with mentees' parent/guardian.
● Help SBU Education Director monitor student academic performance and assist
in the collection of student report cards, transcripts, and school documents.
● Work with SBU Youth Counselor and Education Director to identify social,
emotional, and academic issues or concerns.
● Check email daily and maintain good communication with SBU office staff
● Hold a weekly call with SBU office staff to discuss progress of each
student-athlete on the team.
● Schedule one monthly in person meeting with program directors.
● Complete progress reports for each mentee two times per year.
● Attend program orientation/training (2 trainings per year)
2. Serve as Head Coach a travel youth soccer team
● Plan and lead training sessions at least two times per week during the season and at least one time per week during the winter.
● Coach and manage team at games.
● Coordinate team travel and traveling with students to games and events.
● Track attendance data from soccer and academic sessions.
● Attend coach training sessions and coach meetings
● Communicate effectively with youth regarding soccer and academic events.
● Enforce attendance and program policies on the soccer field.
● Work collaboratively with an assistant coach where applicable.
● Coordinate with the Director of Soccer Programs on all soccer matters
SCHEDULE
Coach-Mentors must be able to commit to coaching for at least one program year, September - June. Approximately 8-12 hours per week during the fall/spring season, and fewer hours through the winter. The precise schedule will be known ahead of each season. Activities include:
● Two practices per week (generally T/Th or M/W, 6-8pm in fall/spring but may vary)
● One or two practices per week in the winter (January-February)
● Weekend games in various locations
● Weekly phone call with Program Director
● Occasional team-building events
● Record keeping (Attendance, etc)
● Communication with youth and parents

General Description
Provides peer support groups and education for substance use disorder clients utilizing established Peer guidelines and assists in engagement activity. In this position, you will participate as a member of the treatment team to provide face-to-face peer services to people with substance use disorder and other behavioral health challenges and linking them to needed community resources. Using their personal lived experience this role promotes clients' recovery, offers support, advocacy, and skill development opportunities. There is 1 position available which focuses on a special initiative focused on State Opioid Response (SOR), which addresses the opioid crisis by increasing access to MAT; reducing unmet treatment need; and reducing opioid overdose-related deaths.
What You Will Do:
Represent Outreach's CRPA program in the community.
Provide peer recovery support to clients by sharing life experiences, assisting clients to develop a recovery plan, linking clients with community based programs supportive of recovery. Peer services may be provided onsite or in the community.

Core Responsibilities:
Provide one-on-one weekly and group mentor ship to help fathers enhance parenting and co-parenting skills (as needed). Support fathers in identifying goals and creating action plans for family and personal development. Achieve 50%, each quarter , of fathers reporting improved parenting confidence and communication skills (via pre/post surveys).
Attend community outreach events to recruit fathers and connect them to program services. Develop and maintain partnerships with community organizations and agencies. Represent the program in 2+ community meetings, fairs, and outreach events per month.

The Peer-to-Peer Mentor is an essential member of the Fatherhood Program team. This role supports fathers by providing lived-experience mentorship, assisting with group facilitation, helping participants navigate program requirements, and building community across all four program tiers. Job Description Summary:
The Peer-to-Peer Mentor is an essential member of the Fatherhood Program team. This role supports fathers by providing lived-experience mentorship, assisting with group facilitation, helping participants navigate program requirements, and building community across all four program tiers. The Mentor strengthens father engagement, supports retention efforts, and contributes to the program's father-friendly environment.
This role is designed for someone with personal experience as a father or caregiver who can connect authentically with participants, model positive fathering behaviors, and support fathers as they work toward reunification, improved relationships, and personal development.
Responsibilities
Provide peer-to-peer support to fathers across all program tiers.
Co-facilitate weekly Peer Support Groups (held twice per week).
Assist with parenting sessions (ProFathering15, Fathering in Action, SFI) as needed.
Share lived experiences to motivate and encourage participant engagement.
Support outreach and recruitment efforts within the community.
Help retain fathers by conducting follow-ups and encouraging participation.
Assist fathers in planning and co-leading required community projects.
Participate in family events and help fathers engage meaningfully.
Track attendance for groups, workshops, and program activities.
Support staff in maintaining incentive logs and participation records.
Communicate participant updates to Family Development Coaches and Program Director.
Attend DYCD-required training and participate in staff meetings.
Promote a father-friendly, trauma-informed, culturally competent program environment.
Conducts phone and field outreach and recruitment to locate and enroll fathers into the program.
