Responsibilities:
The PhysicalTherapist, School Based serves students with various physical, behavioral, and developmental challenges. Ages range from 3-21 years old. Age specific experience and/or special training and/or expertise is required to serve this population.
RESPONSIBILITIES/ACCOUNTABILITIES:
Implements treatment plans and programs through consistent daily and weekly schedules that focus on functional and measurable outcomes to be shared with the school and/or supervisors.
Delivers specialized individual and group physical therapy instruction as mandated in students' Individualized Education Plans (IEPs).
Collects data and completes reports regarding the student's goals and progress.
Consults with other professionals (Teachers, SLP, OT, PCP, etc) on the student's treatment team to ensure a cohesive and comprehensive therapy team.
Assesses and documents each student's progress through formal and informal tests and measurements.
Writes and prepares annual review reports and goals for each student in accordance with their IEP dates and requirements in order to remain in compliance with state guidelines.
Provides physical therapy expertise when developing FBA/BIP.
Provides parent and caregiver support and education to discuss the student's progress and strategies for continued support outside of school.
Attends all school related meetings as scheduled, such as, but not limited to, Triennials, Annual Reviews, Domain, Referrals, and pre-IEP meetings for each individual student.
Ensures a suitable learning environment by setting up and preparing materials and supplies as needed.
Supervises PTA(s) in accordance with state practice act guidelines, as needed.
Communicates and follows up with schools and families to obtain appropriate physician signatures/orders for school-based physical therapy referrals in order to initiate and continue with services.
Follows the safety guidelines and directives for the student in regards to functional mobility and orientation (i.e. transfers, wheelchair or walker mobility, etc.) and exercises provided by other related service providers (i.e. occupational therapist, vision itinerant, hearing itinerant, speech and language pathologist, orientation and mobility instructor, etc.).
Utilizes communication devices and assistive technology as outlined by the speech language pathologist and itinerants.
Follows any fire drill or safety guidelines outlined for each individual student.
Provides screenings and evaluations to determine requirements for skilled physical therapy intervention relative to student/client's level of function. This includes, but is not limited to the following areas: school function, range of motion, strength, positioning, contracture management, conducting home assessments, discharge planning, development of maintenance programs and other related client care issues.
Provides documentation to maintain the quality of therapy records, as it relates to reimbursement guidelines, accuracy of billing information, and school requirements.

What You'll Do:
• Educate patient on safe bed mobility, transfers and ambulation in consideration of post operative precautions
• Transfer, ambulate and perform stair negotiation with patients to determine they are safe for discharge to home
• Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third-party payers which reflects an understanding of reimbursement systems and their impact on patient care.

• Additionally, synthesizes evaluation results to formulate the diagnosis, goals, prognosis and plan of care.
• Establishes treatment plan based on evaluation results, goals and best practice. Collaborates with interdisciplinary team, patients and patient's support system. Assesses and re-assesses need for modifications to plan of care and goals.
• Provides education/training to patient and/or patient support system
• Functional Training-This includes but is not limited to ADLs/IADLs, therapeutic activities (e.g. transfers, bed mobility), rehabilitation equipment prescription and training and community reintegration
• Responsible for treatment, plan of care, exercise prescription for patient caseload
• Provides interventions to patients that are safe, effective, and in compliance with JAG
• Provide data, note writing using Prompt EMR for all treatments provided
• Attends annual JAG Billing and Coding seminar
• Communicate with Manager/Patient Access team for scheduling evaluations once demographics are received

Responsibilities:
Schedule: Full-Time, Mon-Fri 8am-4pm
Under the supervision of the Clinical Supervisor and the Mental Health Unit Chief on Rikers Island, the MHC will provide quality mental health care to incarcerated individuals who need and/or want services. This position is responsible for providing assessment and counseling services to inmates.
Conducts mental health evaluations, crisis intervention, assessments, and follow-up care.
Determines and coordinates disposition of patients for appropriate level of mental health care
Coordinates with Reentry Social Workers for appropriate discharge planning services

PURPOSE:
The purpose of a Trauma Systems Therapy (TST) Therapist in Preventive Service Department is to provide the TST model for families with child(ren) (ages 5 and over) who are impacted by trauma and there may be risk for foster care placement. The Preventive Services Department provides child welfare services using Evidence Based models to ensure that families are able to provide safe environments for their children, keep children safe in their homes and reduce the need for foster care. Prevention is an essential part of the child welfare continuum and requires a commitment to the wellbeing of all members of a client family. All services are provided in a family's home and community.
POSITION OVERVIEW:
The Therapist works with families to assess their needs, develop intervention plans, and deliver therapy sessions that are tailored to the family's unique circumstances. They collaborate with other service providers to ensure a comprehensive approach to the family's care. The therapist also plays a crucial role in monitoring the family's progress, adjusting treatment plans as necessary, and working towards successful case closure.
The Therapist is a key figure in helping families overcome challenges, strengthen relationships, and create a safe and nurturing environment for children within the child welfare system. The Therapist provides all services in a client's home and community.
KEY ESSENTIAL FUNCTIONS:
Provide Child Welfare Preventive Services exclusively or primarily within the client's home and community
Learn and implement the TST model
Improve caregiver-child relationship as a way to restore the child's functioning
Integrate case management services with an assigned evidence base model to help families meet their basic needs to promote stability and reduce stress
Collaborate and coordinate services with all providers working with the family to ensure the family's needs are adequately being addressed
Identify and evaluate risk
Conduct assessments including review of referral information, identify and engage key participants, identify strengths and weaknesses and collaboratively develop goals.
Engage caregivers and other key participants in active change-oriented treatment by identifying and overcoming barriers to engagement.
Provide services exclusively or primarily within the client's home and community.
Adhere to the fidelity of the model. Work within the model's structure, regarding length of treatment, case contacts, interventions, supervision, and documentation.
Participate in consultations with the model developer. Audio/video record sessions for training and supervision purposes.
Maintain clear and concise documentation in CNNX case record including progress notes of treatment efforts and Family Services Assessment Plans, enter data into PROMIS; prepare reports to Probation if required
Work a hybrid schedule based on program needs
Comfortable with group supervision and commitment to work on professional development goals to improve adherence
Work a flexible schedule to meet the needs of families
Outreach to generate referrals for program
Work with a team to meet client, program, and division needs

CORE COMPETENCIES:
Basic social work knowledge and understanding of stages of change
Ability to provide in home therapy and child welfare services
Ability to establish a strong therapeutic relationship with families.
