Seneca Lake Terrace is seeking a compassionate, organized, and motivated Case Manager to join our Wellness Department team. In this role, you will support older adults in maintaining their independence while ensuring they receive the care, services, and community connection they need to live with dignity and purpose. Seneca Lake Terrace Assisted Living is a 65 bed facility located in Geneva, New York.
Typical job duties include:
· Evaluating, initially and quarterly, the needs of a resident and the capability of the program to meet those needs
· Orienting new residents to the daily routine of the facility and participate in care planning meetings
· Identifying any resident adjustment issues and assisting to resolve those issues
· Assisting each resident to maintain family and community connections as well as establishing new ones
· Encouraging resident participation in community activities
· Identifying needs and arranging for services and entitlements from public and private sources for income, health, mental health, social services, and therapy services
· Assisting the resident in obtaining and maintaining a primary physician or source of medical care of his/her choice who is responsible for the overall management of the individual's health and mental health needs
· Coordinating the work of the other case management and service providers (Hospice, CHHA's etc)
· Assisting residents in need of alternative living arrangements to make and execute sound discharge or transfer plans
· Assisting residents, as needed, in the shopping for personal items such as toiletries, medical supplies, stationary, books, and clothing
· Oversee and document case management needs, and activities taken to meet resident needs to include, but not limited to, plan of care, aide care plans, medication assistance, fall management, and change in condition
· Works within the confines of the NYS DOH regulations and Resident Rights, always maintaining HIPAA
· Orient and supervise Charge Nurses and aides while maintaining the required staffing schedule
· Support the Wellness Program and Fall Prevention under the direction of the Wellness Director/ Director of Resident Services
· Conduct meetings with families and/or responsible party to foster good communication, exemplary resident care, and great customer service
· Attend marketing events, as required
· Assists in maintaining on-call schedule and on-call responsibilities to include availability for consultation to the nursing staff
· Assist with the interview, hire, discipline, termination, and supervision of nursing staff through participation in staff meetings, orientation, and required in-services
· Follow Viva Senior Living's Policies & Procedures
· Assist with QA activities
· Assist in maintaining all Viva Senior Living's required reporting logs
· Assists in maintaining the department budget, as set forth by Viva Senior Living

Summary: As a vital member of the Sanctuary team, the Caregiver, Animal Wellness provides essential daily needs to the residents. Qualifications:
Minimum of one year as a Farm Sanctuary Assistant Caregiver, Animal Wellness OR Minimum two years' experience at animal care facility; volunteer history at animal advocacy organization(s); candidates with only one year of experience may still be considered but would start as an Assistant Caregiver, Animal Wellness

Summary: In close collaboration with the Director, Animal Care and the staff Veterinarian, the Manager, Wellness supervises caregivers to apply evidence-based, holistic care practices to optimize physical, emotional, cognitive, and social health for all residents. The Manager, Wellness provides leadership, structure and support to ensure excellent communication within the Wellness team as well as with Environment, Research and Site Ops teams. This position collaborates closely with the Research & Animal Welfare team in implementing goal-oriented animal welfare programs, including the provision of daily, goal-oriented enrichment. The Manager, Wellness reports to the Director, Animal Care.
Responsibilities:
Provide Leadership and Team Development: Supervise Wellness team members through regular one-on-ones, coaching, performance management, and weekly team meetings to cultivate a culture of transparency, growth, and compassion.
Lead Staff Training and Hiring: Train team members on caregiving shifts and healthcare skills; participate in hiring by conducting interviews and evaluating candidates in collaboration with Director, Wellness.
Manage Scheduling and Team Coverage: Oversee staffing schedules and fill in for 1-2 shifts per week as needed to ensure continuous, high-quality resident care.
Oversee Daily and Emergency Health Care: Conduct routine health checks, administer advanced care, and serve as the primary decision-maker in emergencies when the veterinarian is off-site, following Farm Sanctuary protocols.
Coordinate Veterinary Services and Medical Oversight: In collaboration with the Veterinary Assistant, schedule external veterinary appointments, initiate diagnostics and treatments in the veterinarian's absence (excluding antibiotics), and enact biosecurity protocols to safeguard resident well-being.
Guide Resident Enrichment and Learning: Collaborate with Research and Sanctuary Animal Welfare team to implement daily enrichment and care programs that support residents' physical, emotional, and cognitive health.
Support Strategic Resident Care Planning: Work with Environment, Research, and Rescue teams to plan resident movements, evaluate rescue intake capacity, and implement strategic animal care initiatives aligned with Farm Sanctuary's long-term goals.

Participates in agency orientation of program operations, policy, procedure, goals and objectives
Facilitates the integration of the residents into the community, using community-based services whenever possible
Maintains accurate case file records and reports based on the requirements of each funding source/program
Is responsible for the documentation, development, and/or implementation of the following: assessments, service plan goals, service plan reviews, utilization reviews, hospitalizations, discharge planning, progress notes, and income in accordance with each residential housing program, insuring that the completion of these documents is in accordance with agency policy, procedure, or business practice. Documentation includes both written and data entry as required
Submits resident files to the SHS within 28-days of an admission for a complete file audit and review, ensuring compliance with program regulations, policy, procedure, or business practice
Is responsible for the renewal of file documents with expiration dates that include; Release of Information (ROI) and/or Consents to Release Information, Physician Authorization's, Housing Quality Inspection packets, Tenant Calculation Worksheets, Income Verification
Ensures that licensed apartments are maintained at standards established for apartment recertification; reporting the need for household furniture/supply replacement or property maintenance in accordance with agency policy, procedure, or business practice
Ensures resident fees related to rent and phone bills are paid in a timely manner and in accordance with agency policy, procedure, or business practice. The HCM will assist as appropriate with delinquency issues
Assists, as needed, residents in complying with treatment/service providers, ensuring that there is no lapse in service/funding
Serves as an advocate/mediator for residents experiencing difficulties with landlord/tenant disputes or provider, employment, funding related issues
Acts as a resource and assist residents transitioning from the program to a more/less restrictive level of care, ensuring a continuity of care with providers and funding streams
Serves as a resource to ensure that the resident has access to services and to ensure that the rights and confidentiality of the resident are not overlooked or denied
Conducts routine housing inspections to see how residents are doing and to identify those who need help. Communicates concerns with the SHS and work with the resident to maintain their unit or assist them to access homemaker or other needed services. Intervention should be timely
The HCM is required to attend staff meetings, treatment team / provider meetings, and staff development/training seminars in accordance with agency policy, procedure, or business practice
Completes other duties as assigned.

Qualifications:
EDUCATION: Bachelor's degree in human services or related field preferred
