Summary:
We're seeking a passionate Care Assistant to join the Unified Care team as both technical and clinical roles to develop an innovative, team-based chronic disease management program. You will utilize iHealth devices and Unified Care platform to help manage patients with chronic conditions on-site and online with other clinical team members.
Under general supervision in a fast-paced Primary Care clinic setting, the Clinical Assistant will assist with direct patient care as dictated by established protocol and coordinate the team to achieve team goals.
*This role is eligible for clinical hours, which can be used when applying for medical school, PA, nursing school, etc.
Responsibilities:
This role requires you to be tech-savvy with the ability to work with our internal messaging channels (Google Workspace, Zoom, Google spreadsheet, pdf, printers, etc. ). Also requires you to be patient and able to explain technology to a non-tech savvy population. You will be responsible for teaching our elderly patients how the mobile app and medical device work, as well as helping them with troubleshooting any tech issues.
Communicate effectively with both tech-savvy and non-tech savvy individuals, conveying technology concepts clearly and patiently.
Communicate well with teammates, providers, and staff in the clinic.
Assists with direct patient care procedures and related tasks; checks in patients assists in obtaining patient histories, takes vital signs, prepares charts, and assists with medical examinations.
Maintains accurate documentation in electronic health records.
Performs routine specialized procedures, such as setting up the iHealth products for the patients under established protocols.
Obtains pertinent labs and other tests as needed by the provider.
Manages clinic inventories and medical supplies.
Educates and advises patients on specific medical issues within established parameters including obtaining prescription information.
Directs patient flow following provider schedules/patient appointments.
Maintains assigned examination rooms.
Coordinates routine office activities and administrative functions to support clinic activities.
Practices safety, environmental, and/or infection control methods.
Conduct patient outreach calls following provided scripts to schedule appointments, provide follow-up care information, and promote health education
Other clinic related duties as assigned.
Qualification and Skills:
Bachelor's degree or above is REQUIRED
Exceptional communication abilities and meticulous attention to detail
Demonstrated patience and empathy, especially when interacting with the elderly
Familiarity with fundamental medical procedures and terminologies is a plus

In addition to meeting the minimum qualification requirements, you must show possession of the ability to type at least 40 words per minute based on a 5-minute sample, with three or fewer errors. You may self-certify your proficiency in the Occupational Questionnaire.

Schedule: Monday - Friday. This position requires availability between the hours of 6:00am-6:00pm.
Compensation: $24.31 - $25.52 an hour
Duties and Responsibilities
Communicates patient care needs to the LVN and supervising RN.
Provides direct personal care to participants as directed or assigned.
Assists in training new hire PCA's and other staff as needed or directed by management.
Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants. Duties will include showering, toileting, laundry and feeding assistance and companionship services to and from medical appointments.
Provides assistance with ADLs and IADLS, transfers, companionship to doctor's appointments, etc.
Provides support and assistance with meal and snack service in the Day Center.
Assists in maintaining patient areas in a clean and orderly manner. Cleans and organizes program areas including cleaning of equipment and shower areas.
Communicates effectively with participants, families, staff, other departments and agencies.
Works effectively in a team setting.
Documents treatments/procedures, observations, nursing care and patient activities on appropriate forms.
Participates in teaching/learning process with others.
Follows guest relations policies and procedures.
Abides by all established policies, rules and regulations.
Participates as a member of the interdisciplinary team (IDT).
Attends staff meetings and takes part in participant care planning.
Observe each participant for any change in physical, mental, emotional and social functioning and shall report such changes to the licensed nurse.
Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
Performs patient care activities appropriately, reporting any circumstances or change in patient's condition.
Performs clerical and administrative work as required including making and answering phone calls, assisting with the implementation of closure policies as per instructed by executive leadership . Entering participant data in EHR system and assisting covering reception.
Assist and have ability to escort participants that require assistance in their care plan.
Transport participants on an as needed basis.
Other projects and duties as assigned.
Physical Demands
Ability to lift up to 30 pounds. Moving, lifting or transferring patients may involve lifting or pushing greater than 30 pounds, should be done with assistance as appropriate.
Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff.
Must have vision with or without lenses that is adequate to read memos, a computer screen, personnel forms and clinical and administrative documents.
Must have high manual dexterity.
Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, twisting/turning, fingering and feeling.
Work Environment
Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
Subject to unpleasant odors.
The noise level is usually quiet to moderate, but may at times be noisy and crowded.
Minimum Qualifications
Valid California Driver's License, acceptable driving record and vehicle insurance.
Must be able to treat people of all ages including geriatrics.
Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.
Education/Experience
High School Diploma or equivalent required.
Current unencumbered CA licensure as a Certified Nurse Assistant, Home Health Aide, Emergency Medical Technician, Advanced Emergency Medical Technician or Paramedic; or certification from an accredited program for clinical medical assisting.
Current BLS CPR Card certified by the American Heart Association
Prior personal care assistance experience, or ability to pass internal competencies within six (6) months of hire.

Expected Outcomes or Key Accountabilities for the Position:
Develop and demonstrate understanding of trauma informed services for youth as outlined in the Art Restores Kids Training Handbook, by providing positive mentorship, redirection and promoting non-violent behaviors.
Demonstrate a thorough understanding of the Art Restores Kids (ARK) Program and how it operates within Cricket's Hope by accurately presenting program's purpose, components and procedures to clients, community organizations, school sites and the public.
Become familiar with youth-related difficulties as well as appropriate interventions by learning how trauma, violence, substance use, mental health and poverty impact children and referring clients to appropriate community resources.
Complete intake process and maintain client files using Parent Permission to Participate forms, photo releases and other necessary documentation.
Facilitate workshops with youth participants using AWBW Curriculum, utilizing a variety of materials identified in curriculum including paint, oil pastels, beads, clay, etc.
Display ability to work as a part of a team by co-facilitating workshops and progressively learning to lead all workshop components including warm ups, activities, check out and providing back up to address individual crisis and/or behavioral needs.
Plan workshops using A Window Between Worlds (AWBW) curriculum to effectively address clients' needs. Including selecting workshops according to topic, number of participants, time and materials available.
Prepare for workshops by identifying and gathering supplies and materials as outlined in our curriculum.
Foster social cohesion within groups by promoting respectful relations and focusing on nonviolent behaviors.
Track workshop attendance using Excel Spreadsheets to be used in reports for Board of Directors and grant purposes.
Promote Pathways to Purpose program by acting as mentor and acknowledging participants' strengths, talents and gifts and encouraging them to find ways to use them.
Provide a model for a healthy, trusting relationships through clear communication and setting appropriate boundaries
Participate in Camp Cricket by helping children prepare for a weeklong camp experience and attending camp.
Assist with event coordination by contacting donors, vendors, organizing donations, preparing venues, etc.
Work as part of a team, taking some leadership roles when appropriate, but also helping behind-the-scenes and helping with the operation of Cricket's Hope.
Keep a consistent and regular schedule as agreed upon with the Executive Director and/or Program Specialist.
Follow/adhere to the Art Restores Kids Training handbook and employee handbook as appropriate.

Duties include but are not limited to:
Officiate intramural sports contests.
Assist with registration for each sport prior to each sport season.
Educate participants and enforce intramural sports rules and regulations.

Assist with pre and post-game logistics and administrative functions.
Assist with preparation, inspection, and closing of intramural sports activity sites.

Resolve participant conflicts and disputes with tact and diplomacy.

Provide coverage for SRC front desk as needed. Perform other duties as assigned.
Comply with industry standards for safety and participation.

Provide general level of knowledge of Campus Recreation programs and activities to customers.
Always represent the SRC and CSUSTAN in a professional manner and act as an SRC ambassador with an extensive knowledge of intramural sports playing rules, policies, and procedures.
