Clinical Care

Use ASL, tactile sign, written communication, adaptive technologies, and other appropriate modalities to ensure accessible communication for all patients.
Develop individualized care plans while addressing social, sensory, and accessibility needs.
Coordinate care with specialists, audiologists, vision rehabilitation services, behavioral health providers, and other community resources.
Maintain culturally competent, trauma-informed practice tailored to Deaf, DeafBlind, Blind, and Hard of Hearing populations.
Collaboration & Care Coordination
Work closely with the Alder Springs and High Country Community Health teams, including resident support staff, care coordinators, and disability-access specialists.
Participate in interdisciplinary case reviews and quality improvement initiatives.
Collaborate with interpreters, support service providers (SSPs), and Certified Deaf Interpreters (CDIs) when appropriate.
Patient & Community Engagement
Promote health literacy within the Deaf, DeafBlind, Blind, and Hard of Hearing community.
Support outreach, education, and wellness initiatives within the Alder Springs community and the broader Morganton area.
Administrative & Compliance Responsibilities
Document all encounters accurately in the electronic health record.
Adhere to federal, state, and organizational policies, including ADA requirements and accessible-care best practices.
Participate in staff training on communication accessibility and inclusive practices.
Qualifications:
Required:
Current licensure as a Medical Provider - Physician, Nurse Practitioner (FNP/ANP) or Physician Assistant in North Carolina.
Board certification in applicable discipline.

Document the Individual's progress and unusual events or occurrences. To assist the company in meeting the company's obligations under state and federal law and regulation, Provider shall complete and submit reports to the company.
Arrange and establish routine or emergency medical and dental care for the Individual consistent with the Individual's Service Plan and with due regard for the Individual's physical and emotional needs.
Provide transportation for the Individual to service appointments and /or community activities, consistent with the Individual's Service Plan.
In the event of serious illness, serious injury, or death, to immediately notify the company's Clinical Supervisor or appropriate on-call staff as soon as possible and after emergency medical or dental care is obtained for the Individual.
Communicate fully, promptly, and regularly with the company about matters affecting the care or placement of the Individual, as provided by state law and regulations and the Independent Contractor Agreement.
Maintain all information and records (including logs, notes, etc.) regarding an Individual in a confidential and privileged manner at all times.

Get orders for Adaptive Equipment from necessary physicians (i.e., glasses, hearing aids, breathing machines/treatments, shower/grab bars, walkers/wheelchairs, etc.) and specific diagnoses.
Have Physician orders signed by the Primary Physician (yearly with annual)
Complete labs as ordered by physician or as requested
Have Physician Orders signed by Psychiatric Physician (upon each visit)
Any Medical changes need to be updated on the Physician Orders including (Medication changes, Adaptive Equipment, diets, allergies etc.)
When labs are completed; follow-up and obtain results from Physician (Psychiatric or Primary)

This role includes providing hands on care, physical and emotional support as outlined in each resident's Personalized Service Plan (PSP) while maintaining a safe, comfortable home like environment. The Resident Care Associate is responsible for demonstrating all elements of wellness, remaining in accordance with federal, state and local standards and regulations and Phoenix policies to promote the highest degree of quality care and services to our residents.
PRINCIPLE DUTIES AND RESPONSIBILITIES
RESIDENT CARE
Resident Care Associate | CNA
Participates in the development of the Personalized Service Plans and updates; attends service plan review meetings with families as requested
Follows the hydration schedule established for the residents is maintained during the shift
Completes resident meal attendance sheets have been completed and any resident who has not dined in the dining room has been provided room service and / or accounted for.
Knows and complies with all resident rights
Initiates, assigns, and assist with activities, as appropriate
Check all assigned residents daily for cleanliness, appropriate clothing, any continence issues and change in condition
Uses proper sanitary procedures and universal precautions
Checks on assigned resident rooms daily for general cleanliness and neatness and takes appropriate action; ensures high risk rooms are double checked
Assist with conducting laundry rooms, common areas - including bathrooms - checks for cleanliness and any exposed hazardous chemicals; and takes appropriate action (i.e. contacting housekeeping, personally cleaning the room, securing the chemicals)
Completes all ADLs for assigned residents according to the resident's Personalized Service Plan (PSP)
Responds to security system and resident call bells promptly & immediately; takes appropriate action including resetting call bells
EDUCATION/EXPERIENCE/LICENSURE- CERTIFICATION
Resident Care Associate | CNA
High School education preferred and may be required by the state
Previous experience working with elders or disabled individuals, preferred
Must be at least 18 years of age to perform the personalcare aspects of the RCA position
Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing, preferred
Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times
