DUTIES:
1. Functions as the Physician Team leader of their Patient Aligned Care Team (PACT), Primary Care Services, Community Based Outpatient Clinic (CBOC) setting at (MTVAHCS). May perform additional duties (temporary or permanent) as a float provider [Face-to-Face (F2F), Telephone, or VA Video Connect (VVC)] as those duties are assigned.
2. Responsible for providing medical care in an outpatient setting following PACT principles. Will routinely provide medical care using F2F, VVC, and/or Telephone appointments. When performing duties as a float provider (F2F or VVC) for remote patients, will provide care to outreach clinics as needed. Float provider duties (temporary or permanent) may require travel to the supported CBOC(s) periodically to cover a provider vacancy.
3. Assures that Veterans receive compassionate, efficient and safe patient care in their respective care line.
4. Assures that clinical care provided meets all timelines and required documentation as required by VA. Providers must review alerts daily and have no more than 200 alerts at any given time. They must encounter their notes within 72 hours. Encounter errors must be completed within 7 days.
5. Participates in multi-disciplinary team meetings to achieve a patient centered, holistic approach for clinical care and needs of the Veteran. This includes daily and weekly huddles with the Physician's respective PACT.
6. Leads by example. Attend either by Microsoft Teams or in person all Primary Care statewide meetings and whenever possible, participates in the clinical and teaching activities at MTVAHCS Primary Care statewide meetings.
7. Comply with all VA directives and policies related to space management to achieve optimal resource utilization (space, professional and administrative) to maximize clinic utilization within the PACT model.
8. Improves quality of care by achieving
a. Excellence in meeting national and Veterans Integrated Services Network (VISN) 19 patient safety goals.
b. Participate in PACT huddles and manage PACT patients' medical comorbidities with the goal to optimize PACT quality improvement efforts and obtain excellence of Healthcare Effectiveness Data and Information Set (HEDIS) metrics.

SUPERVISION RECEIVED: Under the general administrative direction and medical supervision of the PHC Chief Executive Officer or designee.
SUPERVISION EXERCISED: No direct supervision exercised. May supervise students during rotations.
WORKING RELATIONSHIPS: Has numerous contacts with the general public, PHC clinic staff and other employees and professionals in the allied health fields. Acts as an expert in a professional field to inform people of their health status and problems and to influence patients to comply with health care recommendations.
REQUIRED KNOWLEDGE, SKILLS, & ABILITIES:
KNOWLEDGE: Considerable knowledge of the practice of primary care medicine including preventive medicine. Working knowledge of community health care and social service resources. Working knowledge of the principles and practices of HIPAA compliance. Working knowledge of the PHC Corporate Compliance Guidelines. Basic knowledge of the principles and practices of clinical assessments and health care management.
SKILLS: Skill in diagnosis and treatment including use and interpretation of diagnostic tests, current medications and therapies. Skill in basic lab procedures and interpreting lab results; skill in clinical examination procedures, charting and the use of related instruments and equipment.
ABILITIES: Ability to work with a multi-disciplinary team in an evolving program. Ability to support PHC's mission and provide culturally sensitive care. Ability to develop and maintain effective working relationships with diverse individuals and groups, the Missoula medical community, staff, clients and the public. Ability to communicate effectively in the English language, orally and in writing. Ability to assist in developing, and practice in accordance with, protocols approved by the PHC Chief Executive Officer, Chief Medical Officer and Board of Directors.
MINIMUM QUALIFICATIONS:
EDUCATION: Requires the degree of Medical Doctor or Doctor of Osteopathy from an accredited medical school; Board certified, board-eligible or residency trained; family practice is required.
SPECIAL REQUIREMENTS: Holds or will hold a current license to practice medicine in the State of Montana with full prescriptive authority with no history of licensure suspension or disciplinary action. Eligible for malpractice/liability coverage. Requires immunizations or proof of immunity to certain infectious diseases and a TB test. New employees will be asked to volunteer vaccination status for required vaccines upon hire and will be offered assistance during hiring to receive necessary immunizations. Employees who have not received the vaccines required for their positions or who are unwilling to voluntarily provide vaccination status for required vaccines will receive a reasonable accommodation where such accommodation does not require an undue hardship or endanger the health or safely of any person.
PHYSICAL/ENVIRONMENTAL DEMANDS:
The work is commonly performed in the PHC clinics and office settings. Requires bending and lifting in the medium range (20 to 40 pounds). Involves a high risk of exposure to blood borne pathogens. May risk exposure to communicable diseases. This role may require evening and weekend hours, participation in on-call coverage, and Saturday clinic shifts. Travel to affiliated satellite locations may also be necessary. This position is not eligible for full-time remote work.
