Case Worker

Classification: Part-Time, Non-Exempt, 25 hours/week, Monday - Friday, 9:00a.m. - 2:30 p.m. with a 30-minute unpaid meal break.
Salary: $16.00 per hour
Job Focus: This position is responsible for Material Assistance Services at the Chapel at Worthington Woods Worship and Service Center location. Material Assistance services include operating the food pantry, completing rent and utility assistance applications, and registration for Christmas Cheer. This position will work in harmony with the Corps Officer to carry out The Salvation Army's mission and ministry.

Qualifications
An understanding of and passion for The Salvation Army's mission and ministry
HS Diploma or equivalent
Volunteer or work experience in a social work/ministry setting strongly preferred.
Strong written and verbal communication skills
Able to maintain strict confidentiality.
Strong organizational skills
Proficient in Microsoft Word, PowerPoint, and Excel
Ability to lift at least 50 lbs. and move a loaded pallet of food.
Able to work well with individuals with varying needs.
Have ability to handle difficult situations in a professional manner.
Valid Ohio Driver's license and the ability to obtain and maintain driving privileges per Salvation Army insurance standards.
At least 21 years old with the ability to drive a box truck.

Works in conjunction with the Recreation Department to assist in developing leisure activities and incentives to reinforce Trainee Employee Assistance Program lifestyle
Acts as liaison to Center Mental Health Consultant and Center Standards Officer for Trainee Employee Assistance Program issues
Performs quality assurance record audits. Maintains Trainee Employee Assistance Program statistics for quarterly reporting to Department Of Labor
Assures collection and handling comply with chain of custody procedures
Supports Center Tobacco Use Prevention Program through education and training

Qualifications

Qualifications:
Degree in Social Work or bachelor's degree in finance or legal professional preferred
LSW or LPC licensure is beneficial
Prior experience working with probate court, estates, financial accounting and or retail banking operations

A minimum of a monthly visits to the ward to monitor the care the ward is receiving in the community, group home or nursing home. Collect and review medical information and make informed decisions regarding proper medical care.
Apply for all benefits and entitlements for the ward that shall include VA benefits, Medicaid, Social Security and Pension benefits.
Communicate and coordinate with the ward's family on decisions regarding their loved one that can include preneed and end of life decisions.
Assess and advocate for any cultural and religious practices that the ward and family may require in providing for their spiritual needs and making end of life decisions.
Assess for signs or verbal reports of abuse or neglect and report all incidents to the proper authorities.
Informs the court of significant changes in the case and files required reports with the court in a timely fashion.
Maintain a basic understanding of entitlements and benefits programs and how to apply for needed benefits.
Understanding Medicare and Medicaid rules as they pertain to nursing home placements, spend downs, and medical services provided under the ward's applicable insurance carrier.
The caseworker will assume on call duties as assigned during non-business hours. Periodically they may need to provide transportation for a ward. On occasion, the caseworker may need to respond to a crisis matter on a weekend or during the evening hours. Other duties as assigned by supervisor.

Understand a mental status exam as it relates to cognitive impairment.
Gather third party information during the assessment to collaborate need to Guardianship.
