• Acts as a preceptor for newly hired field RNs and LPNs, giving individualized instruction and guidance during the onboarding period as well as providing field support to incumbent clinicians as identified by management.

• Conducts/facilitates/participates in regularly scheduled preceptor team meetings that are focused, productive, promote ‘consensus' decision-making and enables the team to achieve desired performance results. SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 “on-call” process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.

What You Will Do:

Medication Administration
Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents.
Receive medication updates from Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
Ensure that medications are passed according to times utilizing a mobile medication cart.
Document and initial as medications are given and ensure that appropriate documentation is completed for refusal or missed doses.
Maintain confidentiality of all resident information including resident medication among other residents.
Report all resident concerns and unavailable medication while administering the medication to the RCD or Wellness Nurse.
Restock medication cart after all medication passes.
Assist in checking medication regardless of packaging system.
Assess the residents to determine need for "as needed medication" and appropriately document and report to supervisor.
Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse.
Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
Follow re-fill process for medications.
Help residents maintain independence and promote dignity and physical safety of each resident adhering to the Sunrise Principles of Service.
Strive to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns, history, and basic human needs.
Practice routinely good standard care precautions of cleanliness, hygiene, and health.
Audit medication carts.
Resident Care
Notify RCD of any resident and/or family concerns.
Attend and actively participates in daily Cross Over meetings facilitated by the LCM.
Conduct Service and Health Updates as directed by RCD.
Participate in the development of the Individualized Service Plans (ISP).
Transcribe orders.
Risk Management and General Safety
Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
Report all accidents/incidents immediately.
Report all unsafe and hazardous conditions/equipment immediately.
Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes.
Comply with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.
Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
Report occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.
Ensure oxygen tanks are stored safely, exchange guest/resident's tanks when empty, and monitor to make sure liters of oxygen are at prescribed levels.

Lifting/Push/Pull - Up to 50 pounds of equipment, baggage, supplies, and ability to lift patients safely and using OSHA guidelines, etc.

Overview
The Registered Nurse will be responsible for either (A) admitting the patient to the Center, (B) assisting in the procedure room, and (C) monitoring the patient during the recovery post procedure. In addition, the Registered Nurse may be in charge of maintaining instruments and inventory in order and other responsibilities as identified by management.
Responsibilities
Admitting Role
Identifies self to patient and family and assures the correct identity of patient and procedure to be performed.
Checks patient's vital signs and does pre-procedure assessment.
Select correct IV catheter and insert IV catheter within two attempts.
Offers emotional support to patients.
Demonstrates knowledge of procedures when instructing patients.
Documents and communicates any specific patient concerns.
Procedure Room Role
Assures the correct identity of the patient and procedure to be performed.
Correct documentation of procedure and medications.
Knowledge of Medications and the ability to administer through various routes.
Able to utilize all equipment to monitor patient.
Offers emotional support to patient during the procedure.
Utilizes safety measures, e.g. masks, gloves, goggles.
Keeps Physician aware of patient's condition during procedures.
Accurately documents specimens.
Knows and performs if needed the emergency protocol.
If not previously completed, can insert IV catheter with no more than 2 sticks.
Chooses appropriate site for I.V. insertion.
Accurately and completely documents I.V. insertion information.
Assist with injections and similar procedures as required.
Recovery Room Role
Follows standing Recovery Room orders.
Accurately documents post-procedure progress in the Recovery Room.
Checks and follows through on any additional physician orders.
Communicates discharge instructions to patient/escort, and answers any questions.
Keeps patient until discharge criteria met.
Arranges or instructs patient on obtaining a follow-up appointment.
Makes follow-up calls to patient on the next working day following the procedure.
Knows emergency protocol.

Other responsibilities:
Demonstrates and uses appropriate safety measures.
Performs duties related to completion of center logs.
Attends all staff meetings and in-service classes on regularly scheduled days.
Participates in quality improvement initiatives.
Keeps informed of changes in endoscopy procedures through reading or attendance at local SGNA meetings
Complies with the various incident reporting systems of the Center, such as adverse events and incident reports.
Performs assigned duties as needed for the center operation.
Job Relationships
Reports directly to the Director of Nursing.
Works collaboratively with Center co-workers and physicians.
Interacts with patients, visitors and staff in a courteous manner
Qualifications
BS or equivalent college degree and current state RN license.
4 years of solid experience in either Endo, Surgery/Ambulatory or OR
Prior experience in an admitting, procedure, or recovery role in an ambulatory setting, preferably in a GI setting.
ACLS certified.
If BLS certified, willing to become ACLS certified within center specified time period.
Strong organizational and analytical skills.
Excellent verbal and written communication skills

Possesses good physical health, capable of working in a demanding environment.
Able to perform the job's essential functions, including extended hours, standing, bending, and stretching with frequent trips to all areas of the center.
Able to lift and carry supplies and equipment up to 25 lbs as needed.
Possesses manual dexterity, hearing acuity, and visual understanding, which is essential to the performance of the job tasks.
Speaks clearly and distinctly to perform extensive communication with other staff members, patients, their families/escorts, physicians, vendors, and other third parties.
Always appears professional and wears professional attire as dictated by center policy.
