About the Client/Patient: Early 70s. Struggles with mobility, essentially uses a walker full time. Mild memory issues, repeats himself often, experiencing a cognitive decline. The individual in this position assists the RN/Case Manager by providing general personal care to all assigned patients. The incumbent interacts with the patient and family to provide for the patient's needs and to promote safety and well-being. In addition, the incumbent works cooperatively with other members of the health care team.
