social, emotional, and psychological needs and strengths. Assesses each patient/resident for
discharge.
Identifies patient/resident discharge goals at admission and documents initial discharge plan and
involves the patient/resident and family throughout the process.
Educates staff regarding the role of the Social Services in the facility and the psychosocial needs
of the patients/residents and their families/significant others including the problems of aging and

Completes comprehensive Individualized Service Plans (based on medical model and assessed need), documents according to requirements, interprets program policies and monitors consumer situations and provision of services on regular basis via home visits and phone contacts with consumers and caregivers to ensure that services meet the needs of the consumer. JOB SUMMARY:
The socialworker facilitates effective referral coordination and information exchange with the Interdisciplinary Team (IDT), prospective and current hospital-based general inpatient (GIP) patients, their caregivers, and hospital and community teams.
Provides explanations about the hospice benefit and organization services in a manner consistent with the patient and family level of understanding.
Records information necessary to assist the hospice physician in determining eligibility, plans and coordinates discharge needs for the prospective patient, and collaborates with community senior living community staff and hospice teams to facilitate a safe and effective discharge plan.
Utilizes SW interventions, education, counseling, and support services to address the identified psychosocial needs of patients and caregivers.
ESSENTIAL FUNCTIONS:
Collaborates with hospital providers and staff IDT to develop a plan of care, complete initial and ongoing comprehensive assessments, and plan/evaluate SW interventions.
Conducts admission assessments/information visits and follow-ups on hospitalized patients
Conducts high-risk admission assessments/follow-ups in hospitals where psychosocial issues are present, including but not limited to patients living alone, inadequate caregiving/decision-maker situations, and significant mental health/substance abuse concerns.
Documents in accurately and concisely at the time the service is provided.
Initiates, coordinates, and revises the plan of care on an ongoing basis with input from the IDT, patient, caregiver, hospital providers, and staff.
Participates in hospital discharge planning, when needed, collaborating with hospital case management on the plan.
Participates in IDT meetings to report on the plan and update the comprehensive assessment.
Provides education and support to patients and caregivers regarding diagnosis, care, treatments, medications, and end-of-life expectations in a method or manner that they understand.
Provides education and support to hospital staff.
Supports special needs of patients and families including the pinning of Veteran patients through the We Honor Veterans program.
Communicates effectively with organizational staff, physicians, patients, families, hospital staff, outside contractors, and community agencies.
Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
Works in collaboration with IDT members, managing the psychosocial care of assigned caseload.
Identifies patient and caregiver stressors and strengths to direct interventions throughout the patient's illness, including initiating post-death bereavement support,
Demonstrates proficiency in navigating systems, knowledge of community resources, advocacy, individual/group counseling, and rapport building,
Understands reimbursement and payer source implications to the plan of care and utilizes resources wisely and effectively.
Assists with the orientation of new staff and visiting professionals.
QUALIFICATIONS:
MSW required
LSW or LCSW is also required or must be obtained within 2 years of hire date.
Two years' experience in the health care setting preferred.

Job Description:
JOB SUMMARY
Perform initial intake, screening, and problem identification of inquiries from those seeking information/assistance or assessments and referrals for home and community-based services.
REPORTING RELATIONSHIPS
Reports to and consults daily with Information and referral (I&R) Supervisor who monitors work via observation of interactions, reviewing of referrals, and thoroughness of case documentation/data entry.
Attendance at I&R and Care Management unit meetings also occurs wherein worker receives regular updates on program policies and procedures.
ESSENTIAL JOB FUNCTIONS
Receive inquiries (in person, phone, or email) from older adults, families, potential referral sources and public at large.
Conduct problem identification by screening and thoroughly exploring inquires to determine most appropriate response to the need(s) presented.
Provide information/assistance about agency and community resources or programs requested and conduct eligibility determinations accordingly.
Take/make referrals for appropriate programs and services such as level of care assessments and screening for Protective Service Reports of Need, demonstrating ability to receive/process such abuse reports in a timely, knowledgeable and professional manner.
Follow-up as needed ensuring that all required documentation/data entry is thoroughly and clearly completed. 50%
Assist with completing applications for Lottery-funded and other special programs such as Property Tax/Rent Rebate, PACE, LIHEAP, Healthy Horizons, etc. 25%
Maintain updated community resource information for access by and distribution to agency units and general public.
Participate in various public education opportunities, expos and community speaking engagements by representing the agency and providing information on available programs/services. 25% (Actual percent of time spent on these essential duties may vary with the needs of the agency.)
OTHER SPECIFIC TASKS OR DUTIES
Maintain records of contacts with consumers as needed.
Attend relevant and/or required trainings as specified.
Serve as advocate for older consumers.
Perform other related duties as required.
MINIMUM QUALIFICATIONS
Successful completion of six (6) months of experience with the County as a Caseworker Trainee; or
One (1) year of experience in public or private social work and a bachelor's degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences; or a bachelor's degree with a social welfare major; or
A bachelor's degree with a social welfare major; or
Any equivalent combination of experience and training including successful completion of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences.
