Program Summary Natchaug Hospital's Northeast Treatment Center in Danielson, CT is conveniently located to both northwestern Rhode Island and south central Massachusetts. Natchaug's Joshua Center Programs treat children as young as six and as old as 17 who are struggling with psychiatric or substance abuse issues, but do not require inpatient care. Services Include: Comprehensive assessment, which may include: Comprehensive aftercare planning, family, group, and individual therapy, occupational and creative rehabilitation therapy. Natchaug Hospital provides transportation from school to the Joshua Center Program, and then to the student's home. Job Summary Lead Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Child/Adolescent outpatient partial hospitalization program (PHP). Clinical services include individual, group and family therapy. Maintains ethical principles and professional standards of practice in adherence to the discipline's professional code of ethics. Assumes leadership responsibilities as assigned by clinical leadership. May assume clinical oversight of daily operations in the absence of Program Manager. Key responsibilities of Lead Behavioral Health Clinician: Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate. Provides direct patient care that may include case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, and emotional support to patients and families coping with illness, trauma, or end -of-life care, referral and discharge planning. Refers patient to appropriate levels of care and available resources that promote the individualized care plan. Assists in the coordination of programming which may include staff scheduling, scheduling clinical activities, program development, utilization review, case load assignment, and attending meetings as directed by manager. May assist in the hiring process for new staff, supporting expansion of initiatives, supporting clinician productivity, leading team meetings, conducting staff education and training. Independently able to provide clinical social work interventions to all patient populations. Provide social work field supervision to second year social work students. ***Lead Behavioral Health Clinicians are eligible for a Sign On Bonus up to $2,500.00, in accordance with program guidelines*** Qualifications Master's degree in social work, counseling, or marriage and family therapy or related Human Service field required. Minimum of one year of experience as a licensed Clinician working with behavioral health and/or chemical dependency population. Experience in leading clinical care teams preferred. Documents all services provided in the clinical record in accordance with state and federal regulations and Agency policy: a) All records are complete, accurate, and done in timely fashion: · admissions within 24 hours from first visit. · subsequent visits within 48 hours. b) Obtains physician verbal/written orders and complies with all reporting requirements. c) Participates in coordination of care with physicians and with other disciplines as appropriate. 3. Manages work assignment in effective and efficient manner: a) Reports any cases to the Hospice/Palliative Clinical Supervisor where goals can not be met or at-risk situations exist and responds appropriately. b) Remains flexible in daily planning, allowing time for emergencies or unexpected visits. c) Maintains productivity as established by the Agency. d) Utilizes payor sources correctly, reflecting changing levels of care. e) Delegates patient care responsibilities to Home Care Nurse (LPN) as appropriate. f) Participates in the development of the hospice interdisciplinary team care plan. 4. Establishes the plan of care to be delegated to the Homemaker/Home Care Aide/Companion, orients and supervises the home care aide in accordance with state and federal regulations and Agency policy: a) Develops an initial written plan of care for the HMK/HCA/Companion and updates this plan as patient's condition warrants but no less often than every 60 days. b) Obtains input from HMK/HCA/Companion regarding patient progress. c) Prepares written performance evaluations of home care aides under his/her supervision. d) Provides supervision of home care aides at a minimum of every two weeks or as required by payor source. 5. Develops and maintains sufficient professional skills and credentials to meet the needs of patient care: a) Attends a minimum of 12 hours of in-service per year. At least six of those hours shall deal with aspects of hospice care. b) Participates actively in self-appraisal and the formation of a plan of self-development. c) Knowledgeable of agency policies and procedures. Adheres to Agency Corporate Compliance Plan and HIPAA Privacy Plan as per written agreement. d) Seeks appropriate clinical input to ensure appropriate patient care. e) Projects professional image through attitude and compliance with dress code. f) Capable of precepting new nurses and students. g) Functions as a team member with consideration to all staff. h) Provides Agency with current copy of professional license. i) Maintains CPR certification. Ancillary Duties 1. Performs other duties as necessary to support Agency programs as assigned by the Supervisor of Clinical Services. a) Agency committees as requested. b) Special clinics as needed. c) Adult Consultation Hours as assigned. d) Special agency or community programs during and/or after regular working hours. e) Participation in Agency focus groups. 2. Performs other duties as necessary to maintain current Agency personnel policies: a) Gives adequate time and attention to administrative duties. b) Abides by Agency weekend/holiday rotation of nurses. c) May provide back-up for on-call service. Responsibilities: • Repair and/or refurbish on equipment such as: frozen beverage, fountain soda coffee brewers, gas dispensing equipment, refrigerated cases, and other food service equipment. • Perform preventative maintenance on equipment such as frozen beverage units, fountain soda, coffee brewers, high speed ovens and other food service related equipment. • Responsible for managing parts inventory on their assigned service truck by conducting weekly cycle counts and periodic physical inventory. • Must be able to travel to several locations throughout the day and work independently the majority of the time. • Receive dispatches via email & phone, and works with dispatchers to ensure prioritization of work with the goal of maximizing equipment up-time. • Updates job status and notes via online work order management system, provides detailed resolution notes • Maintains truck inventory accuracy by performing weekly cycle counts and annual inventory. • Ensures service vehicle is maintained, clean, and well stocked with service parts. • Perform other job related duties as assigned by Department Manager Working Relationships: Internal Facilities Managers, dispatchers and parts department, to include Retail Store team members. Will also interact with other technical peers including: HVAC Technicians, Fuel Maintenance Technicians and General Maintenance Technicians. Minimum Education: a high school diploma or its equivalent Preferred Education: Trade school and/or Associates Degree Minimum Experience: Requires 0-2 year's related experience.
