The primary focus of a Family Support Specialist is to deliver, coordinate, and document supportive services to families in our prevention, shelter, and stabilization programs, ensuring services are effective, trauma-informed, culturally responsive, and focused on long-term housing stability. Essential Job Functions
Direct Services (50%)
Coordinate, deliver, and document family-focused services as part of a team.
Work alongside referred families to create, or continue, a housing stability plan, setting mutually agreed upon goals and identifying the steps required to reach each of those goals.
Support Intake procedures and walk-ins as needed.
Provide support to the program team where needed.
Resource Connection (10%)
Connect families with appropriate resources and services.
Establish and maintain effective relationships with key stakeholders.

Administrative (40%)
Maintain online case file records for every family, including documenting all case management activities and summary of services provided in compliance with our established policies and procedures.

Document self-sufficiency and health assessments in HMIS as appropriate.

Experience & Qualifications
Minimum two years' experience in social work or related field required.

* Ensure compliance with hospital financial resolution policies, including conducting financial interviews with patients and families to assist with resolution of their account. Initiates and completes charity applications on accounts at risk. This includes Nemours Financial Assistance Programs and Uninsured discount Programs.

The Life Enrichment Specialist is a full-time travel position responsible for providing on-site leadership support to Grace Management, Inc. communities by filling open Life Enrichment and Village Program leadership roles on assignment. This position ensures consistency in programming, hospitality excellence, and resident engagement during times of transition, vacancy, or performance improvement. Essential Functions of the Life Enrichment Specialist
Serve as an interim Life Enrichment Director or Village Program Life Enrichment Manager during community vacancies, transitions, or leadership changes, ensuring a seamless continuation of engagement programs.
Travel to communities nationwide, providing hands-on leadership, training, and mentorship to Life Enrichment teams.
