Duties will include:
Places an introductory call to the client within the first 24 hours of receiving the case assignment. If the client is unavailable a message will be left on their voicemail and an email or text will follow.
Assessing the completeness of the case documentation and obtains missing documentation, if required.
Reviews documentation and accuracy of information with client.
Sets client up with necessary medical treatment and/or ensures the client is treating through their own providers. -
Maintains client file with progress reports and claim status (while complying with established organizational guidelines).

Updates case management system at every stage of the case, documenting all pertinent information and conversations.
Timely files any governmental claims via certified and return receipt mail and requests a conformed copy of each claim filed.
Calendars the “45 day from date the claim was mailed out” in the case management system.
Assesses if additional treatment is needed and/or approved on each case.
-Reviews the police reports and medical records obtained by the Records Clerk. On Serious Injury Cases (SI), forwards these reports to the Team Attorney.
For Serious Injury (SI) Cases: Follows up with Records Clerk in order to obtain the police report, establishes liability, requests the policy limits of the defendant, obtains the client declarations page, obtains any important evidence (i.e. PD photos, injury photos, videos, witness statements, etc.) -
For Pre-Procedure Evaluations: Drafts the PPE questionnaire, obtains the reports, bills and
estimated cost or each procedure, obtains the defendant's policy limits, runs and ISO on the
client, and provides all information to the Lead Attorney for their review. For surgery evaluations runs a Criminal Background Check (CBC).
-Contacts client to over the ISO and/or CBC with client to confirm the information and/or obtain any pre-ret treatment regarding any previous accidents. Adds a note regarding the ISO/CBC review.
-CM will obtain medical provider full contact information, prior attorney information and provide this to their respective RC for cases where there are pre-existing accidents as they will request the records.
Consults and advises clients regarding loss wages claims and credit issues.
Resolves problems. Consults with Pre-Lit Manager regarding claims process and special, unusual, or complex circumstances.
As required and under the direction of an attorney, assists in the negotiation process for a
client's case (case by case)
Seeks opportunities for clients to submit favorable online reviews about their experience with the firm.
Maintains utmost professional standards towards the Firm and its clients.
Achieves high customer satisfaction in the claims process.

JOB PURPOSE:

Coordinate and facilitate safe exits from the property for individuals experiencing crises, ensuring a supportive and respectful process.

SCOPE OF RESPONSIBILITY:
Specialists will work on a team providing services to children and their families/caregivers involved in the Los Angeles County foster care system using a Wraparound services delivery model. In Wraparound, the team assists high need children to be successful in school, home and their communities with the goal being to maintain children in community settings. Specialists work together with Facilitators and participate on the Child and Family Team, work directly with the children and family/caregivers per the Plan of Care in their respective environments, including but not limited to in homes, out-of-home placement(s), schools and/or in the community to help achieve permanent placement for the child(ren). Services provided will include but are not limited to mental health, crisis intervention and case management per requirements of a Los Angeles County Department of Mental Health Services contract. Specialists who understand and demonstrate independent initiative to perform in a team environment that endorses the family-driven, group decision making process at the heart of this model are preferred. Use of own car, and timely completion of duties including but not limited to paperwork, keeping records, and performing other, similar paraprofessional job duties are required.
ESSENTIAL FUNCTIONS:
Provides community linkages and resources for the child or youth.
Encourages the child or youth to utilize their formal and informal supports appropriately.
Promotes functional skills building and teaches coping skills that will allow the child or youth to participate in CFT meetings and reduce functional impairments (academic, social, emotional, medical, and occupational).
Collaborates in gathering information regarding the youth or family's past trauma experiences.
Consults with the clinician on the child or youth's underlying needs and trauma and collaborates with the clinician on improving the symptoms and behaviors of the child or youth.
Works with school personnel to recognize signs of trauma that may be causing behavioral issues at school.
Assists formal and informal supports with re-labeling behaviors associated with trauma and provides increased awareness of the emotional needs of children and youth.
Advocates for positive behavior interventions such as support, guidance, social and emotional learning, mediation, and conflict resolution.
Other duties as assigned.
REQUIRED QUALIFICATIONS:
Bachelor's degree in social work, counseling or psychology from an accredited college or university is required. A minimum of two (2) years of experience working with children or adolescents and their families is required.

WORKING CONDITIONS:

POSITION SUMMARY:
This position is under the Thriving Families Program (TF). TF is designed for families to obtain self-sufficiency by teaching them critical skills that empower them to resolve crises and avoid future hardships while achieving long-term stability, including employment and economic goals. ESSENTIAL FUNCTIONS:
Identify strengths and areas of improvement for new clients and working clients as needed, through information and assessments.
Completes the discovery process, determining personal attributes, interests, skills, talents, and preferences, resulting in a solid profile for program participants.
Develops a personalized plan and determines vocational, educational and/or employment opportunities in conjunction with participants, to create and maintain a clearly written Thriving Family Stabilization Plan to include educational, housing, and employment goals.
Assists participants with educational support including high school completion, online educational opportunities, college preparedness programs, college scholarships etc.

COMPENSATION:
$23.88 USD - $28.66 USD
In accordance with California law, the expected pay range for this position is included in this posting. The actual compensation will be determined based on factors permitted by law. New hires will be offered a salary at the starting point of the range to maintain internal inequity and allow opportunity for future salary growth. CII salaries are determined based on comprehensive internal and external market analysis, as well as the skills, education, and experience relevant to each position.

POSITION SUMMARY:
This position is under the Thriving Families Program (TF). TF is designed for families to obtain self-sufficiency by teaching them critical skills that empower them to resolve crises and avoid future hardships while achieving long-term stability, including employment and economic goals. The position will work with families to assess participant's interest, abilities, and personality characteristics, for vocational planning purposes and developing and executing job placements. The program's primary objectives are to motivate clients through financial education, academic and employment retention support, asset-building opportunities, and community linkages.
ESSENTIAL FUNCTIONS:
Identify strengths and areas of improvement for new clients and working clients as needed, through information and assessments.
Completes the discovery process, determining personal attributes, interests, skills, talents, and preferences, resulting in a solid profile for program participants.
Develops a personalized plan and determines vocational, educational and/or employment opportunities in conjunction with participants, to create and maintain a clearly written Thriving Family Stabilization Plan to include educational, housing, and employment goals.
Assists participants with educational support including high school completion, online educational opportunities, college preparedness programs, college scholarships etc.
Develops vocational educational relationships with trade schools, community colleges etc. and connect participants as appropriate.
Generates employment opportunities for participants through regular employer contacts, public relations, and community interaction. Follows up on leads with potential employers and develops jobs.
Assists with ongoing job development including but not limited to: resume and cover letter development, interview preparation, individual job searches, job coaching, preference assessments etc.
Conduct financial literacy workshops and awards families who complete the financial literacy program.
Works with TF team in the implementation of the savings program and UBH match.
Support clients in achieving realistic stabilization goals by coordinating supportive services tailored to meet family needs.
Encourage clients to work with community partners and utilize local resources that will support the clients' growth.
Engages clients in regular meetings, trainings, and events to further develop clients' skills for self-sufficiency.
Facilitate relationships with internal and external community entities that will support clients' goals including but not limited to educational institutions, employment and work source centers, etc.
Serves as a positive role model to participant, modeling appropriate self-sustaining skills, behavior and social interaction.
Supports participants in locating, applying, and partaking in the appropriate programs, trainings, and/or institutions to achieve individualized goals as identified in the Thriving Family Stabilization Plan.
Provides services/interventions to empower participants in retaining employment, academic achievement, and overall family functioning betterment. Interventions may include counseling, advocacy, job coaching, skill training, assessment and referral.
Participates in training to improve skills in the use of structured training techniques for supportive services in education, employment, and life skills.
Collects & documents “success” verifications from participants (i.e. academic enrollment, training certificates, employment offer letters, etc.)
Research and remain informed on opportunities that can benefit clients including, but not limited to job fairs, educational trainings, seminars, etc.
Identifies potential employers with a focus on developing employment opportunities for program participants.
Identifies areas where participants can apply to obtain or increase entitlement benefits such as Calfresh, CalWorks, SSI/DI, etc.
Works collaboratively with UBH's case management and housing team members to eliminate participant barriers that can interfere with the Thriving Family Stabilization Plan.
Documents and maintains current information on services provided to participants in the agency's client tracking database and Homeless Management Information System (HMIS).
Participates in regular case conferencing meetings and attends staff trainings as required.
Maintains electronic and hard copy case records in participant file.
Prepares regular written reports of clients' activities as assigned.
QUALIFICATIONS:
Bachelor's degree from an accredited college in human services or equivalent combination of training, education and experience to perform the job successfully.
Three years' experience providing services to persons experiencing homelessness or at risk population, including experience working with youth.
Knowledge of: general goals and purposes of public social services programs; techniques for interviewing and gathering information from varied populations; arithmetic skills sufficient to interpret participant income and expense information to identify general eligibility for services and benefits within program requirements.
Demonstrated experience working with challenging populations; including those with criminal backgrounds, mental health, chemical dependency, emotional/behavioral issues, special education, and/or literacy issues.
Ability to read, apply, interpret, and explain State and Federal regulations, policies, and procedures governing assigned employment program areas and program guidelines.
Strong verbal and written communication skills as well as time management skills.
Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook) and any other database software(s) used to track service delivery.
Strong ability to effectively resolve and cope with immediate conflict and/or crisis situations.
Experience using a data management system to record client information and interactions.
Knowledge of Trauma-Informed Care and Practices a plus.
Ability to work collaboratively, but independently.
Ability to maintain clear personal and professional boundaries.
Ability to work effectively in a constantly changing and sometimes demanding or chaotic environment.
Ability to solve problems, make decisions, resolve conflicts and listen.
Exceptional organizational skills.
Experience working with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age, and physical and mental illness; must demonstrate cultural competency. Experience in Special Education, Alternative Education or related programs, a plus.
