POSITION SUMMARY:
Under the general supervision of Mental Health Supervisor and Site Program Supervisor (Mobile Response and Stabilization Services, MRSS), provides mental health services as a member of a mobile, community-based crisis intervention team to residents of Saginaw County. Serves all target populations including adults with mental illness, youth with emotional disturbance, individuals with developmental disabilities, and individuals with co-occurring substance use disorders. In addition, provides crisis intervention services within community settings such as schools, adult and youth foster care homes, private residences, and others, with the purpose of assisting those experiencing a mental health crisis and/or who may need assistance accessing ongoing mental health services. Provides (via telehealth or in-person contact) urgent screening, short-term clinical intervention, crisis stabilization, and safety/support planning with the goal of diverting hospitalization or legal involvement when appropriate. Works as a member of a multi-departmental group with SCCMHA Crisis Intervention Services, Central Access and Intake and Care Management with shared performance goals related to access, consumer satisfaction, and risk management. Coordinates with adult and youth mental health service providers to ensure consumers may access the most appropriate level of care in an outpatient setting. Provides consultation to law enforcement and co-responds to crises as appropriate. Available for assignment to the schedule seven (7) days per week, overnight (from 10 PM-8 AM), including holidays. This position will be knowledgeable about and actively support culturally competent recovery-based practices; person-centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma-informed culture of safety to aid consumer in the recovery process.
ESSENTIAL DUTIES AND RESPONSIBLITIES:
1. Respond to any non-imminently life-threatening psychiatric or behavioral symptoms that are disrupting a consumer's functioning or the family system by providing immediate intervention to assist in de-escalation and stabilization of the presenting situation.
2. Provide crisis intervention for consumers in crisis which may include suicidality, emotional dysregulation, family interpersonal conflict, youth noncompliance with parental/identified guardian's directives, substance use, emergency shelter, domestic violence, or other psychosocial issues.
3. Respond to emergency phone calls (via the MRSS or 24-hour crisis telephone lines) from community agencies, consumers (youth, families, and adults) to help alleviate mental health crises.
4. Utilize crisis de-escalation techniques with an awareness of the safety of self, other clinical staff, and consumers in the environment.
5. Complete assessments and interventions that may be provided in the office or in the community as the situation warrants.
6. Mobile Response and Stabilization Services are available 24 hours per day, 7 days per week, with weekends and holidays included. Staff should demonstrate a high level of flexibility in available work hours, with no specific work schedule guaranteed. The function of the program necessitates a strong team adherence to standards of attendance. Short-notice time-off requests are especially disruptive and, to the greatest extent possible, use of Paid Time-Off (PTO) must be arranged in advance.
7. Function as a member of a triage team providing urgent screening and intervention with the goal of diverting inpatient psychiatric hospitalizations and youth incarceration when appropriate.
8. Function as a member of a community-based mental health team in diagnostic formulation and implementation of mental health services, short-term crisis interventions and stabilization plans for consumers, including adults, children, adolescents, and their families.
9. Mobilize security, police, medical response, or crisis response as appropriate.
10. Work closely with SCCMHA Crisis Intervention Services to provide follow-up care and assistance to consumers diverted from an inpatient treatment setting.
11. Collaborate with clinicians from Central Access and Intake, Crisis Intervention Services, and Care Management departments regarding the immediate needs of the consumer.
12. Convey information clearly and concisely to team members at the end of scheduled shift.
13. Coordinate services of the MRSS team with other community agencies; police, schools, hospitals, courts, to identify and obtain emergency resources and services for youth, families, and adults in a mental health crisis.
14. Work within the unit and on Sentri II electronic scheduler to provide maximum flexibility for scheduling of crisis follow-up appointments and eligibility screening appointments to meet consumer needs.
15. Demonstrate an understanding of the intentional and unintentional effect of his/her of own professional demeanor in response to individuals who are demonstrating signs and symptoms of a psychiatric or behavioral crisis.
16. May provide a warning contact to individuals who have been threatened with harm. May make reports to Child and Adult Protective Services according to state law.
17. Maintain records, reports, and legal documents in the electronic medical record.
18. Adhere to the mission, vision, core values and operating principles of SCCMHA at all times.
19. Adhere to state crisis response time requirements (within 2 hours of initial request for intervention).
INCIDENTAL DUTIES AND RESPONSIBILITES:
1. May attend meetings, workshops, or other public mental health activities to assist the community.
2. May perform various administrative functions such as preparing reports and appearing in court.
3. Attends meetings, seminars, workshops, and community events related to the public mental health mission to sustain professional competency.
4. Perform various administrative/clerical functions such as preparing travel vouchers, making copies of documents, scanning documents, filing, etc.
5. React productively to change and handles other tasks as assigned.
6. Provides outreach services with supervisor approval.
7. Performs Quality improvement duties as interested or assigned.
8. Speaks on mental health issues to public groups with supervisor approval.
9. Provides psychoeducation to consumers, families, and community members as needed.
10. Will require working overnight, weekday and weekend shifts and holidays as needed.

QUALIFICATIONS:
Education: Bachelor's degree in a human service-related field from an accredited school.
Experience: A minimum of one (1) year of experience in the examination, evaluation, and treatment of youth, families, and adults, including dealing with individuals in crisis situations.
Has a thorough knowledge of mental health service options within and outside the community mental health system. Professional experience working with adults and children with serious mental health disorders, Intellectual or Developmental Disabilities, or co-occurring substance use disorders.
Licenses and Credentials: Valid Michigan Driver's license with a good driving record.

Works as a member of multi-departmental group with Preadmission Screening, Care Management and State Facility and Forensic Intensive Case Management units with shared performance goals related to access, customer satisfaction and risk management. This position will be knowledgeable about and actively support culturally competent recovery based practices; person centered planning as a shared decision making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process. The individual in this position must be bilingual (English/Spanish). The individual in this position will be a resource to the general community providing bilingual services as needed to both persons meeting SCCMHA entry criteria and the general public (through State grant dollars).
ESSENTIAL DUTIES AND RESPONSIBLITIES:
1. Functions as the Hispanic bilingual outreach worker on a triage team providing urgent assessments and interventions with the goal of diverting hospitalizations when appropriate.
2. Provides brief focused interventions to individuals requiring a bilingual outreach worker.
3. Serve as first point of contact to Hispanic individuals that are seeking behavioral health services.
4. Participate in community events that are focused on the Hispanic population.
5. Connect with community partners to educate agencies on behavioral health services and care of individuals with the Hispanic population for the Saginaw County area.
6. Functions as a member of a triage team providing urgent assessment and interventions with the goal of diverting hospitalizations when appropriate.
7. Completes tracking of services as required to meet the requirements of the grant dollars provided for this position.
8. Assessments and interventions may be provided in the office or in the community as the situation warrants.
9. Completes a thorough assessment and interim service and support plan based on the persons served/families desired outcomes and needs. Provides an integrated assessment of co-occurring substance use disorders.
10. Supporting the concept of recovery by developing the initial transition plan upon admission to services.
11. Determines eligibility based on Saginaw County Community Mental Health Authority (SCCMHA) eligibility criteria.
12. Makes recommendations to Care Management regarding which unit and team can best meet the needs of the persons served.
13. May provide the first service after intake to advise the person served of the findings of the intake assessment and provide the initial orientation to SCCMHA services.
14. Responsible to assure that intakes occur within 14 days of the request and that the first service after intake is scheduled within 14 days as well.
15. Assures that all required demographic information is accurate and compete prior to transferring the case.
16. May be required to work flexible hours to accommodate the needs/desires of the person served/families.
17. Completes the original CAFAS for children with serious emotional disturbances or LOCUS for adults with serious mental illness and other assessments as prescribed by agency policy.
18. Works within the unit and on the Sentri electronic scheduler to provide maximum flexibility for scheduling of intake appointments to meet the person served needs.
19. Works closely with the Inpatient Pre-admission Screening and Episode Management Unit referring persons served when an adequate hospital diversion plan cannot be developed.
20. Completes all necessary documentation in the electronic medical record within the time frames established in policy.
21. Adheres to the mission, vision, core values and operating principles of SCCMHA at all times.
INCIDENTAL DUTIES AND RESPONSIBILITES:
1. Perform various administrative/clerical functions such as preparing travel vouchers, making copies of documents, filing, etc.
2. Attend meetings, seminars, workshops, and community events related to the public mental health mission.
3. May occasionally transport persons served to and from agencies and community resources in personal automobile or agency car.
4. React productively to change and handles other essential tasks as assigned.
5. Ability to use computer hardware and software.

This is a primary care position where the newly hired candidate will see 20 patients per day. The newly hired candidate must be Board Certified and an active Michigan license is preferred. The provider will be building their own patient panel and eventually need to work with autonomy. Duties include histories, medication management, new patient consults, must be able to see all ages, etc.Hours are Monday - Friday 8a - 5pm schedule with no weekends/call.Familiarity with Epic is preferred.
