Essential Duties Include: Arranges and completes home visits with parents to address child development through the DCY approved curriculum Parents as Teacher. Home Visitor is to complete Family Plans, Parent & Family Assessments, Developmental Screening, and assist in planning additional developmentally appropriate play activities for children and their families. Familiarizes self with community resources or services; refers clients to such resources or services as needed and documents such referrals. Organizes the curriculum's activities and keeps the supplies in stock. Clean supplies after use. Works cooperatively with other agencies that provide parent education. Takes training online and on site to better educate parents and as required for continued training hours. Prepares and submits periodic reports on program efforts and needs to the Ohio Department of Children and Youth, Family and Children First Council, the Early Childhood Collaborative and others as needed. Monitors and enters OCHIDS data, monitors referrals to ensure accuracy and proper utilization. Assists in activities to create public awareness of the Help Me Grow Program. Completes office work such as documentation and data entry, scheduling visits, making phone calls and preparing letters to clients or referrals, receives and responds to inquiries; etc. Attends program staff meetings; participates in regularly scheduled supervision and attends HCHD staff meetings and trainings. Maintains the required licensure and certification after gaining certification with the support of the HMG supervisor. Perform laundry duties such as washing, drying, folding and putting away Empties waste containers, collects and disposes of recyclables and garbage. Change bed linens and make beds. Collects soiled laundry: washes, dries, folds, irons, performs minor clothing repairs and distributes laundry. Delivers mail and packages to client. Transport client when needed. Assists with all client needs for personal care services. Inform supervisor of client needs and concerns. Attends and participates in Brethren Care At Home team meetings as required. Attends in-services outside of Brethren Care Village and/or Brethren Care At Home for support and needs of the client Other duties as assigned. What do I need for this role? High School Diploma or equivalent preferred but not required. Training in geriatrics (nursing, social work, rehabilitation or therapeutic recreation) or sufficient related prior work experience is preferred but not required. Onsite presence required.
