The ideal candidate will have a strong background and experience in diagnosing and treating a wide range of geriatric illnesses and chronic disease with a focus on comprehensive patient care and quality of life. This full-time position involves working collaboratively with other healthcare professionals within the hospital-based system to provide high-quality medical services, ensuring the best outcomes for our patients. Responsibilities: 1.0 FTE clinician alternating weeks between in a typical 5-day M-F workweek outpatient clinic and working in the residential long-term care facility Care for adult patients in the outpatient and residential long-term care setting One in three telephonic call for nurse advise at the LTC facility.
