QUALIFICATIONS AND EXPERIENCE REQUIREMENTS
LSW preferred
High school graduate or G. E. D. equivalent
Previous social services experience in a long term care environment.
YOUR MISSION
As Social Services Designee, you will:
Develop and maintain a good working rapport with other direct caregivers to assure their participation in and support of all programs/services to benefit the residents' psychosocial well being.
Report all grievances and complaints and make necessary oral/written reports to the Director of Social Services.
Serve as the resident's advocate at all times working in harmony with all direct care giving staff to assure that the resident's needs are being met at all times (i.e. appropriate dress and clean appearance). Be aware of any changes in a resident's condition and report immediately to the RN Charge Nurse.
Provide information to the Director of Social Services that would help to solve the problems of residents and better meet their needs.
Improve the quality of services and/or activities and increase a variety of opportunities and choices for the resident.
Work with the activity department to involve family members and guests in activities whenever possible.
Maintain confidentiality of all resident/family information at all times.
Perform general social services duties.

GENERAL STATEMENT OF DUTIES: The Social Service Assistant will be responsible to the Program Manager of the locality within which they are employed and work to provide a trauma-informed, safe and stimulating environment for clients. They will be expected to work within the policies and procedures of Family & Community Services and work toward the maintenance of excellent service standards and professional practices.
ESSENTIAL RESPONSIBILITIES:
1. To assist in the efficient running of the facility and its therapeutic programming and other general duties associated.
2. While utilizing trauma-informed care, assist clients upon shelter entrance with required paperwork, continued assistance throughout their shelter stay by providing supplemental case management, and assist in communication and coordination with various agencies to secure appointments and resources.
3. Provide a level of support necessary to ensure all clients experience an improved quality of life while in our shelters.
4. Use a strength-based approach to help clients in working towards their goals of obtaining independent living.
5. Accompany clients to appointments, as needed.
6. Organize, plan and lead groups for clients in shelter to help promote positive interactions and group work (i.e. affirmation boards, book club, yoga, etc.), on a voluntary basis, when time allows.
7. Complete daily administrative duties such as answering phone, directing inbound calls, keeping office clean and organized, tracking supply lists and facility needs, completing check requests and therapeutic programming attendance of clients.
8. Maintain facility documents and records and ensure all requirements to operate a facility are being met, such as conducting fire drills, kitchen temperatures, and other facility needs.
9. Prepare rooms and meals (as needed) for clients.
10. Work with team members to foster a positive working environment.
11. Promote anti-discriminatory practice ensuring that no resident is discriminated against on the grounds of race, class, gender, religious beliefs, disability, or sexual orientation.
12. Attend staff meetings and training courses as required by management.
13. Report timely for all assigned shifts.
14. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
15. Other duties as assigned.
Requirements
QUALIFICATIONS:
1. Ability to utilize the trauma-informed care model to provide open communication, fair and equitable treatment, and display care and compassion.
2. Excellent in-person, phone, email and written communication skills for team members and clients, even in stressful situations.
3. Knowledge of community services.
4. Must possess Microsoft Word skills.
5. Valid Ohio Driver's License and ability to meet agency requirements for driving insurability.
6. Must be able to meet agency requirements for criminal background check (Ohio Bureau of Criminal Identification & Investigation)
7. Must have reliable transportation and a phone.
MINIMUM EDUCATIONAL/LICENSURE REQUIREMENTS: High School Diploma or GED preferred. CPR/First Aid Certification preferred.
MINIMUM EXPERIENCE REQUIREMENTS: Experience working in crisis intervention preferred.

Responsibilities
The Social Services Designee will assist the Social Service Director/Consultant/Administrator in the planning, developing, organizing, implementing, evaluating, and directing of social services programs of this facility.
The Social Services Designee will meet with administration, medical and nursing staff, and other related departments in planning social services, as directed.
The Social Service Designee will participate in discharge planning, development and implementation of care plans and resident assessments.
The Social Services Designee will encourage residents/responsible parties to participate in the development of their care plan and invite them to care plan meetings accordingly.
The Social Services Designee will assist residents in obtaining transportation to medical appointments, upon discharge, etc.
Required Qualifications
All facilities must provide medically related social services to residents. It is not required that a qualified social worker necessarily provide all of these services, except as required by State law. The regulations do not require a Social Worker when a facility has equal to or less than 120 beds. The Social Services Designee must possess:
A minimum of a high school diploma or its equivalent.
