Assist with provision of a wide range of practical, personal, social, and general care services for the veteran resident and/or veteran household. 3. Assists veteran residents and/or veteran households in developing realistic and achievable housing plans that focus on maintaining housing stability and include measurable action steps and timetables. 4. To undertake monitoring, reviewing, and recording of the agreed upon support plan according to the changing needs of the veteran resident and/or veteran household. 5. To promote veteran resident and/or veteran household involvement by maximizing their participation in the decision-making process within the services. 6. To communicate and liaise with various agencies (e.g. Department of Veterans Affairs, Veterans Services Commission, Probation Services, Housing Authority, Landlords, Department of Jobs and Family Services, Legal and Consumer Credit Organizations). 7. To accompany veteran resident and/or veteran household to appointments with any of the above agencies as necessary. 8. Maintains regular contact with the veteran resident and/or household including childcare assessments, crisis intervention, referrals, and ongoing life skills development. 9. Assures that veteran resident future and/or veteran household housing is habitable according to established organizational standards. 10. Collects, provides and updates required data for the HMIS database. 11. Supports Social Service Worker (SSSW) and their duties in their absence or as needed 12. Travels to community locations where homeless and at-risk veterans congregate to ensure effective local outreach. 15. Help SSSW, all staff, and veteran's residents maintain up-to-date knowledge of internal fire regulations and procedures and any other emergency procedures ensures health and safety standards are maintained. 22.
