Responsible for overseeing the collection and distribution of key clinical, demographic, and treatment information for patients who may be treated with non-surgical or surgical modalities. Position Overview: A member of the PACT team sharing responsibility for the treatment, rehabilitation, and prevention of emotional disorders. The PACT nurse is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and support services.
Job Duties:
Provides case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team (ITT); assumes primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the ITT, provides individual supportive therapy and symptom management, ensures immediate changes are made in the treatment plans as clients' needs change, educates and supports clients' families, and advocates for clients' rights and preferences.
Conducts comprehensive assessment of psychiatric history.
Consults with community agencies and families to maintain coordination in the treatment process.
Provides on-call crisis intervention during regular hours and serves as a backup to evening and weekend Many Rivers staff.
