Responsibilities:
A Day in the life of an Early Intervention CaseManager:
As an Early Intervention CaseManager you will provide service coordination/casemanagement services to children, birth to three years of age, with developmental delays, diagnosed conditions or atypical developmental & their families by assessing, planning, linking, monitoring, and advocating for them in response to their changing needs. Completes clinical documentation in accordance with agency and regulatory guidelines.
Requirements for the Early Intervention CaseManager:
Required: 2+ years experience required in early intervention working with infants/children with developmental delays
Required: Bachelor's degree in human services or a related field
Required: Must be certified or certifiable in in Basic First Aid and CPR
Required: Valid driver's license and acceptable driving record as reported by the Department of Motor Vehicles
Required: Ability to pass Background Check and Drug Screen
Required: Must be reliable and a quick learner and have ability to multi-task with accuracy in a very busy environment
Required: Excellent written and verbal communication skills

GENERAL STATEMENT OF JOB Under general supervision, performs casemanagement, service coordination, screening and assessments and linking for mentally ill youth and adults. Employee serves as a primary casemanager in providing individual and family counseling and make referrals as needed. This position reports to the Clinician Supervisor/Program Administrator.
ESSENTIAL JOB FUNCTIONS
Assist consumers by identifying individual needs; coordinating available resources and monitoring service delivery. Work involves ensuring the physical, mental, medical and social well-being of the consumer.
Employee supervises consumers in the daily unit operations; documents youth, family and adults progress and maintains records in accordance with program policies and procedures.
Provides consultation to youth, families and adults to assess their social, health, and vocational needs; assists consumers' in obtaining information regarding appropriate community services programs. Visits foster care home providers and consumers; refers consumers to foster care homes, as necessary.
Consults regularly with therapists, staff members, and doctors and families to discuss consumer progress; maintains accurate and complete clinical charts; updates consumers' progress records. Monitors mental status of consumers through home/office visits; records observations; evaluates and monitors progress through service delivery. Plans workshops/training/meetings related to developing programs that meet consumers' needs for youth and adults.
Attends case reviews, documents; and implements service plans accordingly. Implements goals and objectives by linking consumer to appropriate services; transports consumers in pursuit of employment, living arrangements and educational opportunities. Ensures departmental policies and procedures are followed.
Responds verbally and/or in writing to inquiries from general public/prospective consumers regarding program goals and objectives. Prepares statistical reports and presents findings to supervisor. Adheres to all billing procedures regarding Medicaid, insurance and third party companies.
Provides outreach services, and serves as a liaison on various City boards regarding the rights and needs of citizens.
Coordinate groups and make referrals to primary care and other resources, as needed.
Work closely with Clinical Therapists and Psychiatrist in coordinating services and interventions.
Performs other related work as required. Individual assignments will be determined by supervisor based on current workload and department needs.

EDUCATION & TRAINING
A Bachelor's Degree in Social Work, Psychology, Education or a human services related field. Requires 1-3 years of experience working in mental health; or any equivalent combination of training and experience which provides the required knowledge, skills and abilities. Experience working with youth and adults in a clinical setting.
SPECIAL REQUIREMENTS
Requires a professional certification in the State of Virginia as a Qualified Mental Health Professional (QMHP) Adult or Child.
An acceptable general background check to include a local, state, and federal criminal history check and sex offender registry check.
Must possess Red Cross First Aid and CPR certifications or have the ability to obtain certifications within 90 days from date of hire and maintain them as a condition of employment.
A valid driver's license with an acceptable driving record.
Individuals in this position cannot be listed as having a founded child use or neglect complaint.

Specific Duties:

* Serve as an after-hours standby supervisor on a rotating basis.
