Position Summary Holon Health is seeking a compassionate and highly organized Case Manager to join our collaborative care team. The Case Manager will play a crucial role in proactively monitoring client progress, ensuring adherence to treatment plans, and providing ongoing support to facilitate positive treatment outcomes. Schedule & Location This is an onsite role, and the individual must be based in Colorado (Denver or Ft. Collins area). Hours will typically be 40 per week with some flexibility around when those hours occur. What You'll Do Actively review client activity within the program, including: drug test results, system logins and lesson completion rates, and appointment attendance and adherence. Monitor client progress against their individualized treatment plan. Identify any deviations from the plan and collaborate with the clinical team to develop appropriate interventions. Identify potential barriers to client engagement and proactively address them with the client. Verify client insurance coverage and eligibility for treatment services. Coordinate client appointments with providers, including scheduling and rescheduling as needed. Provide support to the clinical team when follow-up with clients is needed. Assist clients with identifying and accessing necessary community resources and referrals (e.g., housing, transportation, employment). Coordinate referrals with external agencies and providers. Assist clients with troubleshooting technical issues related to our proprietary technology platform. Provide guidance on navigating the platform and accessing program resources. Respond promptly to client inquiries and concerns. Provide ongoing support and encouragement to clients throughout their treatment journey. JOB SUMMARY & RESPONSIBILITIES The UC Case Lead will be responsible for providing casework and supportive services to children in foster care, which includes facilitating integration into the local community and supporting the UC's access to education services and legal services in addition to required medical and dental services. Additional casework responsibilities include facilitating any family members identified or referred for potential sponsorship and assisting with sponsorship application as needed. The UC Case Lead will also be responsible for communicating with stakeholders in an effective and timely manner to ensure case continuity.     Summary/Objective The Case Manager will provide support to assigned clients and facilitate/link clients to appropriate community service resources. Assist and support clinical services as needed in cooperation with the treatment team. Essential Functions Receives referrals for Case Management services/treatment from therapists; provides an initial and ongoing assessment of client needs for case management, treatment, support, and/or skills training. Is familiar with community resources and makes appropriate referrals and/ or links clients to those resources. Provides case management monitoring of the Client's condition(s) and progress or lack of progress and communicates this to the primary therapist. Advocates on behalf of the Client to ensure his/her access to needed resources and ensure their rights are respected. Provides case management emergency services during work hours and after hours as assigned. Provides psychiatric rehabilitation services to all clients in need, including readiness assessment, skills training on an individual and group basis, assisting Clinicians who facilitate group activities, and case management to improve the Client's functioning and assist the Client in being successful in his/her treatment goals. Carries out all psychiatric rehabilitation services by Client's treatment plan and goals and identifies possible service needs to the primary Clinician in support of comprehensive treatment planning. Provides liaison services to state and local hospitals as needed. Provides liaison services to community partners, such as attending meetings, facilitating communication, and answering questions about services within the agency. Provides program and case consultation and education services as needed. Responsible for tracking supported employment activities. and assists with the development and implementation of supported employment programming as directed by the supervisor and Clinic Director. According to the Policies and Procedures for Volunteers of America Northern Rockies. Provides documentation reflecting the course of treatment on time. Actively participates in supervision meetings, clinic meetings, case discussions, planning for treatment provision, and agency development required training activities. Maintains a suitable professional demeanor that includes: contributing to a positive work environment within VOANR through dependability, flexibility, cooperation, supportiveness and respect for others, communication, creativity, ethical behavior, and appropriate appearance.
