JOB SUMMARY
The CaseManager I - Emergency Services (CME-I) reports to the Emergency Services Supervisor or Emergency Services Manager and provides casemanagement services, crisis intervention, support and referral services for clients assigned to contracted emergency services programs. The CME-I will provide timely outcome reports per program guidelines and requirements. Monitors progress of residents, and refers them to appropriate agencies for assistance as needed. Schedule may include some nights and weekends.
CORE DUTIES/RESPONSIBILITIES:
Maintains personal and professional integrity, and ensures clients are treated with respect and dignity
Handles confidential information with discretion and in accordance with HIPPA guidelines
Provide casemanagement services designed to assist clients to obtain housing and stability
Builds rapport with clients and facilitates group discussions effectively
Maintains up to date listing of community contacts, links clients with community resources in a timely manner and follows up with client's progress once linked with community resources
Enter and maintains client-related data including case notes and documents
Completes HMIS entries accurately and in a timely manner
Keeps client files organized and with all documentation completed accurately
Keeps supervisor informed of challenges and accomplishments when working with difficult cases, or if client refuses to accept housing
Provide support for clients as they transition from homelessness to housing
Ensures contact information is captured when client departs (address, phone, email, etc.)
Closes files appropriately and in a timely manner, reviews files for accuracy and/or missing data, and updates the database accordingly for discharges
Attends collaborative meetings and networks with other agencies, coalitions, and local community meetings as assigned
Communicates well with clients and coworkers
Will lead and/or participate in devotions, prayers, or other ministry activities ex. outreach events

EDUCATION/EXPERIENCE REQUIREMENTS:
Requires a High School diploma or GED

The CaseManager is a non-exempt position responsible for the delivery of casemanagement services to at-risk families of child abuse and neglect, including parenting skills training, home safety and life skills, job placement, housing, and other support services in order to reduce the likelihood of any child abuse or neglect incidents.
Provide casemanagement services in accordance with guidelines and manual requirements as established by the funding source.
Screen, assess, and evaluate clients in their homes to assess their basic needs and establish advocacy relationships with the client, family/guardian, and or significant other.
Provide information, referral, and follow-up for appropriate ancillary and support services.
Resolve service delivery problems.
Maintain established productivity requirements.
Must ensure concise and timely completion of all paperwork or documentation, including but not limited to, administrative, clinical, or case load requirements.
Ensure and safeguard the human and legal rights of clients and their families and co-employees at all times.
Perform all other duties as assigned.

We are currently seeking a FT CaseManager I at The Caring Place Center for Men in Miami, FL,
JOB SUMMARY
The CaseManager I (CM-I) reports to the CaseManagement Supervisor, and will provide casemanagement services, crisis intervention, support, and referral services for residents. The CM-I will also provide timely outcome reports per program guidelines and requirements; monitors progress of residents; and refers them to appropriate agencies for assistance as needed. The CM-I provides guidance to clients using scriptural references, and acts as a role model by personal testimony. Duties include discipleship of residents based on sound scriptural/biblical principles, incorporated into established casemanagement techniques.
CORE DUTIES/RESPONSIBILITIES:
Makes correct assessments and drafts appropriate care plans for clients
Keeps appropriate up-to-date case notes, and documents files in a timely manner
Builds rapport with clients and facilitates group discussions effectively
Keeps supervisor informed of challenges and accomplishments when working with difficult cases
Maintains an up-to-date listing of community contacts
Links clients with community resources in a timely manner
Follows up with clients progress once linked with community resources
Closes files appropriately, and in a timely manner
Keeps client files updated in compliance with established procedures
Attends company sponsored events to support the residents we serve
Keep up to date progress notes as per clients Phase Guidelines and status change in the Rommel System.
Submits accurate on time client monthly reports
Will lead and/or participate in devotions, prayers, or other ministry activities ex. outreach events
Must be in agreement with and sign The Caring Place Statement of Faith
Performs other related duties as assigned
EDUCATION/EXPERIENCE REQUIREMENTS:
Requires a High School Diploma or previous on-the-job training in Social Work or related field; Associate's degree, CAP training, and 2 years' experience preferred
At least 1 year experience in working with the homeless population
Requires the ability to lead and facilitate support groups using biblical scriptures and principles, and incorporate scriptures into the rehabilitation process
Must be able to exercise independent judgment within the general framework of approved casemanagement techniques, existing laws and within agency guidelines
Must have the ability to recognize symptoms of common mental, physical, co-occurring, alcohol/ substance abuse addiction
Must be able to make proper assessment for care plans, provide casemanagement of client services, crisis intervention and referral services
Must possess knowledge of local community agencies, programs and resources available for clients
Must be able to establish and maintain effective working relationships with clients and staff of diverse cultural and linguistic backgrounds regardless of race, religion, sex, disability, political affiliation and sexual orientation
Must have the ability to react quickly and calmly in an emergency
Must be organized and computer literate
PHYSICAL REQUIREMENTS:
Must be able to sit for lengthy periods of time
Must also be able to see, hear and speak, in order to interact with staff and the general public
Must be able to lift and/or carry up to 25 pounds
Must also be able to move around as needed to perform essential job duties

Essential Job Responsibilities
Recruitment and Screening:
Maintain assigned billable hours per week on average throughout the year.
Screen clients for mental health and substance abuse disorders as well as for available counseling interventions.
Informed Consent:
Obtain informed consent utilizing agency forms prior to providing services.
Conduct client orientation to treatment program according to applicable laws and agency procedures.
Assessment:
Conduct In-Depth Assessment to determine diagnoses and/or course of treatment for both individuals and groups.
Complete assessment requirements for various available counseling interventions and programs.
Treatment and Service Planning and Documentation:
Provide mental health and/or substance abuse treatment, including diagnosis, crisis intervention, treatment planning and reviews for both individuals and groups.
Adhere to agency procedures and protocols in the provision of effective delivery of mental health and substance abuse counseling and maintenance of standards of care.
Serve and maintain an active caseload, as assigned with concomitant productivity.
Ensure documentation is timely, accurate, legible and clear.
Maintain updated and complete treatment plans, treatment plan reviews, progress notes and other required information in client records as specified in performance standards.
Intervention & Crisis:
Conduct interventions with clients including individual, group and other appropriate counseling interventions.
Determine client needs for specialized services within or outside of the agency, such as medical treatment, psychiatric care, psychological testing as well as with other health-related organizations in accordance with the client's treatment plan and communicate all such actions to the psychosocial team as applicable.
Assess clients in crisis for appropriateness of Baker Act proceedings, utilize Baker Act documentation when necessary and coordinate with police, family, and other staff as required to place client when necessary.
Supervision:
Provide daily documentation of work performed (e.g., Informed Consents, Assessments, Treatment Plans & Reviews, Case notes, Discharge Summaries)
Attend at least weekly supervision with supervisor to review work, obtain signed documentation, discuss clinical issues and staff critical cases until such time as licensure under FS 491 is achieved.
Provide your supervisor with productivity reports weekly.
Manage Resources:
Coordinate with client casemanager for clients support services as needed.
Coordinate court referred programs by writing reports and progress notes as required.
Input client information using specific software as required.
Provide clients with information about bill-coverage, services and procedures as required.
Perform quality assurance reviews bimonthly achieving a minimum score of 90%.
Participate in staff training sessions as required by the health center.
