Essential Duties and Responsibilities

What You'll Do

The Intake Case Manager is integral to ensuring the success and support of clients undergoing medical treatment and property damage claims. This position involves being a crucial link in helping clients obtain the maximum compensation they deserve to overcome challenging situations. Treatment and Service Planning and Documentation:
* Provide mental health and/or substance abuse treatment, including diagnosis, crisis intervention, treatment planning and reviews for both individuals and groups.
* Adhere to agency procedures and protocols in the provision of effective delivery of mental health and substance abuse counseling and maintenance of standards of care.
* Serve and maintain an active caseload as assigned with concomitant productivity.
* Ensure documentation is timely, accurate, legible and clear.
* Maintain updated and complete treatment plans, treatment plan reviews, progress notes and other required information in client records as specified in performance standards.
Intervention & Crisis:
* Conduct interventions with clients including individual, group and other appropriate counseling interventions.
* Determine client need for specialized services within or outside of the agency, such as medical treatment, psychiatric care, psychological testing as well as with other health-related organizations in accordance with the client's treatment plan and communicate all such actions to the psychosocial team as applicable.
* Assess clients in crisis for appropriateness of Baker Act proceedings, utilizes Baker Act documentation when necessary and coordinates with police, family, and other staff as required to place client when necessary.
Supervision:
* Provide daily documentation of work performed (e.g., Informed Consents, Assessments, Treatment Plans & Reviews, Case notes, Discharge Summaries)
* Attend at least weekly supervision with supervisor to review work, obtain signed documentation, discuss clinical issues and staff critical cases until such time as licensure under FS 491 is achieved. Provides supervisor with productivity report weekly.
Manages Resources:
* Coordinate with client case manager for clients support services as needed.
* Facilitate drug urinalysis for court mandated clients following agency procedures.
* Coordinate court referred programs by writing reports and progress notes as required.
* Input client information using specific software as required.
* Provide clients with information about bill-coverage, services and procedures as required.
* Perform quality assurance reviews monthly achieving a minimum score of 90%.
* Participate in staff training sessions as required by the agency.

Job Description
We are seeking a Personal Injury Case Manager to support our attorneys by managing a caseload of personal injury claims and acting as a key point of contact for clients. The Personal Injury Case Manager will guide clients through the claims process, collaborate closely with providers and insurance companies, negotiate settlement for case under authority supervisor and ensure cases move efficiently from intake through settlement or litigation. This role requires a detail-oriented and client-focused professional with deep knowledge of personal injury cases.
Key Responsibilities
Client Communication:
Serve as the primary point of contact for clients, guiding them through the claims process and ensuring they feel supported at each stage.
Present and explain initial settlement offers to clients, providing recommendations on next steps.
Educate clients about additional medical treatments (e.g., injections, surgeries, TBI treatment) and legal procedures, such as filing lawsuits.
Maintain bi-weekly proactive communication with clients and ensure all inquiries are addressed within 24 hours.
Case Management & Documentation:
Manage the full lifecycle of personal injury cases, including intake, negotiations, resolution, and post-settlement tasks.
Accurately maintain and update client files, records, and case notes in Litify to ensure seamless case progression.
Track and log demands, offers, and client authority, ensuring proper documentation throughout the negotiation process.
Ensure timely case resolutions while prioritizing client satisfaction and attention to detail.
Insurance & Provider Coordination:
Follow up with insurance companies to obtain timely responses to settlement demands and negotiate under attorney supervision.
Verify insurance coverage details (UM, PIP, BI) and request necessary documents from insurers.
Communicate with medical providers to gather treatment estimates, lien information, and outstanding balances.
Coordinate with providers and attorneys to support mediation and case settlement processes.
Operational Support:
Prepare mediation summaries, global settlement documents, and related case materials.
Conduct weekly audits to ensure all case files are current and compliant.
Draft and file lawsuits or dismissals under attorney guidance, ensuring accuracy and timeliness.
Review surgical case files and related documentation prior to client procedures.
Working Schedule: Monday through Friday 9:00AM - 5:30PM

JOB SUMMARY
The Case Manager I (CM-I) reports to the Case Management Supervisor, and will provide case management services, crisis intervention, support, and referral services for residents. The CM-I will also provide timely outcome reports per program guidelines and requirements; monitors progress of residents; and refers them to appropriate agencies for assistance as needed. The CM-I provides guidance to clients using scriptural references, and acts as a role model by personal testimony. Duties include discipleship of residents based on sound scriptural/biblical principles, incorporated into established case management techniques.
CORE DUTIES/RESPONSIBILITIES:
Makes correct assessments and drafts appropriate care plans for clients
Keeps appropriate up-to-date case notes, and documents files in a timely manner
Builds rapport with clients and facilitates group discussions effectively
Keeps supervisor informed of challenges and accomplishments when working with difficult cases
Maintains an up-to-date listing of community contacts
Links clients with community resources in a timely manner

EDUCATION/EXPERIENCE REQUIREMENTS:
Requires a High School Diploma or previous on-the-job training in Social Work or related field; Associate's degree, CAP training, and 2 years' experience preferred
At least 1 year experience in working with the homeless population
Requires the ability to lead and facilitate support groups using biblical scriptures and principles, and incorporate scriptures into the rehabilitation process
Must be able to exercise independent judgment within the general framework of approved case management techniques, existing laws and within agency guidelines
Must have the ability to recognize symptoms of common mental, physical, co-occurring, alcohol/ substance abuse addiction
Must be able to make proper assessment for care plans, provide case management of client services, crisis intervention and referral services
Must possess knowledge of local community agencies, programs and resources available for clients
Must be able to establish and maintain effective working relationships with clients and staff of diverse cultural and linguistic backgrounds regardless of race, religion, sex, disability, political affiliation and sexual orientation
Must have the ability to react quickly and calmly in an emergency
Must be organized and computer literate
