Summary of position:
This position reports to the Deputy Director of Social Work Reentry within the Division of Correctional Health Services and is responsible for working with the Deputy Director on the management, supervision and innovative development of staff (social workers, casemanagers, administrative) who work to address the social service needs of patients on the mental health service upon re-entry into the community. These needs include but are not limited to linkages to benefits, housing, and treatment for mental illness, employment, and family/community supports. This position will require strong collaboration with all relevant clinical services and with the courts.
Responsibilities include: (Detailed Task)
Develop and evaluate performance standards and guidelines for social workers and case workers in the forensic mental health treatment setting.
Provide direct social work services to individuals, families and communities; including the development and implementation of comprehensive clinically driven discharge plans.
Assist social workers and casemanagers in identifying and utilizing community resources for mental health, substance use, entitlement benefits, housing, and medical care.
Assist with referrals/appointments to appropriate community-based aftercare services.
Promote and support Social Work Department collaboration with an inter-disciplinary treatment team approach to patient care through excellent verbal and written communication skills.
Conduct quality assurance reviews of patient care to ensure clinically appropriate, tailored comprehensive care, and in compliance with Brad H stipulation requirements.
Generate and reconcile reports and analyze data related to patient care and Brad H stipulation compliance.
Conduct appropriate follow-up to ensure connection to care; and participate in the orientation and in-service training of staff and other agency personnel.
When required, participate in program planning and policy making with regard to community health programs and facilities and discharge planning.
Supervise and evaluate all aspects of social worker and casemanagers' work performance; take part in counseling and disciplinary processes when necessary.
Minimum Qualifications:
1. Licensure (LMSW or LCSW) issued by The New York State Education Department (NYSED); and,
2. Six (6) years of experience in Clinical Social Work, of which three (3) years are in a health care or medical care setting including psychiatric and medical social work, and three (3) years in a supervisory, consultative and administrative capacity in a organization adhering to acceptable standards with knowledge of administration, with administrative and managerial skills and abilities; or,
3. A satisfactory completion of training and experience. However, all applicants/incumbents must possess licensure as described in (1) above at the time of appointment; and,
4.Demonstrated ability to function productively and cooperatively with Director of Social Work, Medical Staff, and Administrative Staff and to supervise effectively.

The goal of the Family Enrichment Center (FEC) is to enhance child and family well‑being by providing access and support to enable the strengthening of family protective factors inherent in its residents and community by building mutual community connections, capacity, and resilience.
In partnership with FEC staff, families will drive the planning, development, and sustainable co‑ownership of the centers. Families who participate in the FEC programming will be encouraged to share their expertise by volunteering at the center and given meaningful opportunities to engage with neighbors and peers. The goal of the FEC is to strengthen families' protective factors and build community connections, capacity, and resilience.
Position Summary
The Family Advocate is responsible for contributing to the day‑to‑day operation of the FEC including building connections with other staff, residents, and community partners; co‑designing and delivering relevant programming and activities (offerings); providing advocacy and making referrals for participants; and creating a warm and inviting gathering place for families and individuals using the protective factors model to support community resilience.
Responsibilities

Qualifications
Minimum qualifications include a bachelor's degree and three (3) years of professional experience; or a high school diploma and five (5) years of professional experience in community-driven programs working directly with children, youth, and families.
Reside in or near the FEC community, or reside close enough to arrive promptly, in the event of an emergency.
Bi‑Lingual either /Mandarin &/or Cantonese is required.
Strong interpersonal skills and the ability to warmly connect with and engage children, youth, and adults individually and in groups.
Strong verbal and written communication skills.

Statement of the Job
The Early Head Start (EHS) Family Worker is responsible for the programs' parent involvement. The job responsibilities include but not limited to maintain necessary records, make referrals to appropriate agencies as needed, and work with families in a supportive fashion. The Family Worker in consultation with Family Service Coordinator and other staff serves as a liaison between the center and parents from the community the program serves. Under the supervision of the Family Service Coordinator, the Family Worker will be responsible to assist in recruitment, interviewing, and registration efforts in accordance with the Agency's policies and Head Start Regulations to determine eligibility of qualified individuals.
Conducts In-Takes and Family Partnership Agreements; shows parents around the center.
Elicits social and developmental information on the child, and shares this information with teachers and other staff keeping with the requirements of the family's confidentiality.
Responsible for inputting information in the data master, completing documentation, preparing required reports to document compliance with guidelines on in-takes, eligibility, priorities, classroom attendance and records; responsible for In-Kind Reports.
Receives and verifies financial and other information needed to determine eligibility as required.
Contacts the families to receive information, referrals, and other services such as employment mental health, counseling, housing, etc. as needed.
Performs other duties as assigned.
Qualifications
High School Diploma required.
2 years of experience in Human Services required.
College Degree or Family Development Credential preferred.
Bilingual Spanish/English; Russian/English; Chinese/English; Korean/English; English/Creole speaking preferred.
Must be able to occasionally lift and/or move up to 25 pounds.

JOB SUMMARY
The CaseManager assesses the needs of unaccompanied children in care, develops Individual Service Plans, screens for human trafficking concerns, facilitates the safe and timely release or discharge of children and youth, documents the provision of services in case file according to ORR requirements, state licensing standards, and Abbott House's policies/procedures.
GENERAL RESPONSIBILITIES
Assess each case and adapt service plan throughout the life of the case.
Practice sound and productive counseling skills.
Documents all contacts appropriately in case record. Writes timely, clear, concise progress notes, assessments and other documents as required.
Screens for human trafficking concerns and works closely with the PSA compliance worker.
Identify and screen prospective sponsors, URM programs and Post 18 plans. Manages unification/ discharge process. Concurrent planning to expediate safe and timely releases.
Effectively coordinates inter and intra-agency services to clients by working as a team with the providers (foster parents, clinicians, teachers, medical, legal, FFS, CFS, CC etc.).
Creates and gives each child a complete discharge package.
After 30 days (and a 7-day call for a medical follow up) calls and documents the 30-day call. Closes the complete UC file.
Establishes effective collegial relationship with Lead CaseManager, Senior CaseManager and Lead Clinician.
Attends weekly staffing meetings and requests meetings with the FFS for complex cases.
Follows up on timely and appropriate referrals for medical services, health screening and mental health assessment of young people.
Meets with the child face to face at minimum of once per week (or more if needed) to provide child with case updates, phone calls and check in.
Conducts home visits as needed and as required by city, state and federal requirements.
Participates in the 7day casemanagement rotation.
Makes meaningful contributions to agency and department programming.
Carry out the job responsibilities in a professional manner.
Complete all required training.
Mandated Reporter.
Participates in individual, group, and department supervision.
Attend and complete ORR & Abbott House Mandated trainings on a timely basis.
Any other related duties as required.

Report critical incidents immediately to the Director/Supervisor of Social Services
Refer clients to internal and external resources and serve as an advocate when necessary
Provide casemanagement and counseling periodically; one year beyond the client's departure
Develop and maintain connections with other programs and units within the organization to facilitate inter-agency referrals
Responsible for escorting clients to housing viewings, DHS or HRA appointments
Responsible for placing clients into suitable housing
Case notes must be completed within 24 hours after meeting with clients
Substitutes for other staff when need arises
Perform general clerical duties
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer.
Ability to sit for extended periods and perform repetitive tasks.
Must be able to lift and carry up to 5 pounds.
Must be able to travel to multiple NYC sites as needed.
Must be able to access and navigate each department at the organization's facilities.
Ability to climb stairs.
Work Environment / Schedule Requirements:
Office setting with regular exposure to computer screens and moderate noise levels.
May be required to work shifts, including evenings or night tours and/or Saturdays, Sundays and holidays with holiday pay differential.
Transitional Housing Program staff are required to continue their shift until relieved by another staff member or authorized to leave by a supervisor.
Qualifications:
Baccalaureate degree in Social Work, Psychology, Sociology, or related field with 2+ years of direct social service experience
Associate degree in Human Service, or related field with 4+ years of direct social service experience
High School Diploma/General Equivalent Diploma with 8+ years of direct social service experience
Strong interpersonal skills with the ability to be compassionate and firm and maintaining confidentiality at all times
Knowledge of community resources
Ability to solve problems, make decisions, resolve conflicts and listen
Ability to deal calmly in crisis situations.
Proven ability to work collaboratively well with diverse groups
Proven ability to handle multiple tasks effectively under pressure
Strong organizational skills, detail-oriented, and efficient
Maturity, integrity, and sound judgment
