Key Responsibilities
Provide individual casemanagement under clinical supervision, meeting productivity and quality requirements set by the Agency and within the limits of services permitted by position and scope of practice. This may also include periodic on-call duty.
Meet or exceed standards pertaining to services provided.
Accumulate the required number of billable hours of clinically allocated time based on Agency standards for position and profession.
Ensure ACT CaseManager-canceled appointment rate does not exceed 10%.
Meet or exceed standards pertaining to assessments and documentation.
Identify Individualized Service Plan supportive services authorized by Utilization Management and provide linkage to these services within authorized duration and frequency, including working with Mental Health and Alcohol and Other Drug clinicians to develop multi-disciplinary services to ensure continuity of care.
Ensure each progress note demonstrates client progress and logical casemanagement action steps.
Ensure progress notes and other documentation clearly identify consumer and ACT Team member input services.
Complete progress notes at the end of each session and within 24 hours of completion.
Develop and maintain quality, accurate clinical charts for all clients served.
Develop and maintain positive collaboration with other social service agencies serving clients.
Maintain knowledge and familiarity with referral sources within the community and provide consultation when needed.
Serve as a liaison between the hospital, criminal justice system, and other treatment services in the community.
Maintain attendance at assigned clinical and administrative committee meetings and training.
Maintain license or registration appropriate to the profession and job requirements, including the appropriate amount of continuing education at all times of employment.
Complete agency-required annual training including client rights and grievances, safety training, and infection control.
Remain free from citations by other providers or consumers for unethical practice and meet professional standards.
Adhere to all organizational and departmental policies, including compliance with all behavioral and ethical expectations.
Participate in at least 6 hours of supervision for every 3-month period or meet the supervision requirements of the license/registration (whichever is greater).
Perform other clinical duties as assigned.
Qualifications
Minimum Qualifications:
Education: bachelor's degree in human/social service field, or associate degree with equivalent field experience.
Experience: 3 years working with SMD population
Certification: First Aid; CPR; De-escalation
Other: Valid Ohio driver's license; proof of automobile liability insurance; meets OMHAS licensure and certification requirements; competencies for serving the SMD population.

Overview
This classification is responsible for professional casemanagement work of routine difficulty, and performs related work as required. This is the trainee/entry level class in the DCS CaseManager job series. An employee in this class learns to perform a variety of casemanagement duties for children under State supervision, in State custody, or at risk of State custody, and their families. This class differs from DCS CaseManager 2* in that an incumbent of the latter is fully trained and functions at the working level.
* An applicant appointed to this flexibly staffed class will be reclassified to the next higher class in the series after successful completion of a mandatory one-year training period; inadequate or marginal performance during the training period will result in automatic demotion or termination.
Responsibilities
ALL LISTED RESPONSBILITIES WILL BE PERFORMED AT THE LEARNING/ENTRY LEVEL:
* Determines if abuse or neglect has occurred, who the abuser is, the level of risk or harm to the child, determines the need for a safety plan, makes recommendations regarding permanency, monitors adoptive and foster families, recruits foster parents, conducts home visits, and coordinates/observes visitation.
* Maintains documentation for case files and other required documentation. Prepares for, participates and testifies in court proceedings.
* Conducts interviews with alleged child victims, individuals reporting alleged abuse or child neglect, child's family, and collateral contacts; gathers required information such as school records, demographic information, medical exam/psychological information, etc., identifies supports for children and families, administers client drug screens; and conducts required assessments/assessments of home. * Works flexible hours including weekends, holidays, and after hours. Schedules appointments, meetings, visitation with children on caseload. Transports children to various appointments safely, assists children in preparing for placement, and sitting with children who are awaiting placement.
* Communicates effectively and timely with others, engages with children and families to build trustful relationships, encourages clients to overcome barriers and achieve permanency, explains parental rights and other relevant documents to families, responds timely to families, providers, co-workers and supervision.
* Convenes and identifies members for Child and Family Team Meetings to work collaboratively with all Child and Family Team Meeting participants. Develops action steps and goals to be accomplished by the Team.

Unusual Working Conditions:
Service and Support Administrators are required to drive extensively within Hamilton County to perform their duties. Therefore, access to reliable transportation is a necessity and an essential function of this position. Planning meetings will be held in individual/family homes and throughout the community.
Start Date:
