Provides ongoing patient information to the interventionist and clinical team during procedures to deliver vascular access care to the patient including but not limited to cardiac monitoring, alderte score, pain status
Monitor patient vitals (HR, BP, O2Sat, EKG) during procedures
Assist in recovery room patient care following vascular access procedures
Gather patient clinical information prior to procedures including patient assessments and patient history
Maintain up-to-date narcotic drug log
Maintain clear and accurate patient medical records
Communicate with dialysis facilities to monitor post-care as directed by center manager
Participate in access center community education programs
Monitor and maintain adequate inventory of medical supplies
Assist in scheduling patients for vascular access procedures and provide assistance in transporting patients within the center
Participate in community education activities regarding the Access Center
Experience
RegisteredNurse: 2 years (Preferred)
Scrub: 1 year (Preferred)
2 years experience in general diagnostic, and special procedures, cardiac catheterization
Experience in IR and Cath Lab is a plus

Key Responsibilities:
Review medical documentation for all admissions, complete medication reconciliation, and obtain admission orders from physicians.
Facilitate emergency transfers for patients with medical conditions beyond our facility's capabilities.
Review referrals from hospitals, facilities, and community agencies, assessing medical and behavioral acuity and responding within designated timeframes.
Respond to inquiries about our facility, providing appropriate recommendations and information.
Conduct assessments to determine the appropriate level of care within our facility.
Collaborate with medical and psychiatric personnel to ensure appropriate recommendations and admissions.
Facilitate the admissions process, including assessment, consent, and inventory of patient belongings.
Perform insurance benefit verifications and secure initial pre-authorization for treatment and admission.
Monitor patient status throughout the admissions process, following facility policies and physician orders.
Utilize web-based programs to track and review referrals.
Communicate projected admissions promptly to designated internal representatives.
Ensure timely receipt of all clinical information, including medical comorbidity, from referral sources or patients.
Schedule and complete pre-admission assessments, consult with admitting physicians, and communicate disposition recommendations to patients or their families.
Prepare and maintain accurate medical record documentation to facilitate payor authorization at the requested level of care.
Complete initial pre-authorization for treatment and admission within payor timeframe guidelines.
Admit patients to the registration and accounting system, completing all necessary admission and consent forms.
Demonstrate a positive, empathetic, and professional attitude towards patients, prioritizing their safety and promptly addressing concerns or complaints.
Coordinate care for patients who are not being admitted, ensuring they receive appropriate follow-up care and referrals.
Conduct safety checks and provide supervision as required by special precautions and individualized guidelines.

As an After Hours Hospice RN, you'll respond to patient needs during evenings, nights, weekends, and holidays-bringing peace of mind and compassionate care when it's needed most.
What You'll Do as a Hospice RN - After Hours / On-Call:
Report directly to the Administrator, Executive Director, or Patient Care Manager Senior
Provide direct patient care and support after normal business hours
Serve as the primary liaison between patients, families, physicians, and the hospice team during your shift
Respond promptly and compassionately to urgent calls, patient visits, and symptom management needs
Evaluate patient and family needs to guide clinical decisions and implement the plan of care
Support and collaborate with an interdisciplinary group including LPNs/LVNs, CNAs, and other support staff
Accurately document clinical visits and communications
Participate in quality assessment and performance improvement programs
Work independently while ensuring continuity of care and coordination with the daytime care team
About You:
Experience - What you'll bring:
Current RN license in the state of requested employment (or eligibility to obtain)
1+ years of nursing experience
A hospice heart: compassionate, calm, and dependable in urgent care situations
Excellent clinical judgment and the ability to provide support in emotionally intense settings
Comfortable providing care in patients' homes, facilities, or wherever they reside
Valid driver's license, automobile insurance, and reliable transportation
Current CPR certification
Preferred Experience (Not Required):
1+ years of hospice, home health, oncology, or palliative care experience

Prior experience in on-call or overnight nursing roles
Strong documentation and electronic medical records (EMR) skills

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity.
