We are seeking a licensed Co-Occurring Substance Abuse Disorder Therapist or Co-Occurring Therapist-in-Training to join our team. This full-time position will serve clients in our Menasha Counseling Clinic by providing therapeutic counseling. Key Responsibilities * Provide a comprehensive assessment by collecting all information outlined in the program's Biopsychosocial Assessment Form. * Provide and updates diagnostic impressions based on information collected from the assessment. * Develop a treatment plan by establishing treatment goals with the client based on the assessment and diagnosis(es) and obtaining client/parent's signature on treatment plan. * Implement treatment by providing psychotherapy in accordance with diagnosis and treatment plan, applying a variety of psychotherapeutic methods with individuals, couples, families, and groups. * Monitor progress toward treatment goals by evaluating and adjusting services provided, completing initial and ongoing clinical case review requirements as necessary. Qualifications Education Requirement: * Master's degree in social work, counseling, psychology or other closely related mental health discipline. Licensure Requirements: * Wisconsin state certification as a Licensed Clinical Social Worker or Professional Counselor, or In-Training Licensure. * CSAC (Clinical Substance Abuse Counselor), SAC (Substance Abuse Counselor), or SAC-IT (Substance Abuse Counselor In-Training * Must be eligible for Medical Assistance, Medicare (if applicable) and other third-party reimbursement. Other Requirements: * Family Services is able to obtain malpractice insurance on your behalf. * Maintain eligibility to practice and active status with your licensing board through the State of Wisconsin Department of Regulation and Licensing. * Maintain eligibility for reimbursement from Medicaid, Medicare (if applicable) or other insurance panels. * Demonstrated commitment to diversity, equity and inclusion. Assists consumers in learning independent living skills, which are defined in the resident's Individual Service Plan. (i.e. laundry, housekeeping, cooking, time and money management.). As well as encouraging management of symptoms through support services and education. Participates in ensuring that nutritious community meals are planned, prepared, and served within the home, involving training residents in food preparation serving and storage. Monitors consumer medication adherence as directed by their physician. Reports to Master's Level Clinician, WHCG RN, or physician any changes in client behavior and functioning. Ensures that residents maintain acceptable standards of personal hygiene. Ensures that residents leave for their day program, job or school appropriately groomed. Facilitate transportation as necessary. Ensures that the house is clean at all times and complies with DHS 83 CBRF regulations. Ensures that all physical repairs are reported to the Property Management Department.
