This is a critical, hands-on role responsible for the day-to-day operation, maintenance, optimization, and compliance of the site's wastewater treatment processes. The successful candidate will be the subject-matter expert for wastewater operations on site, ensuring system reliability, environmental compliance, and continuous performance improvement. We are a child abuse prevention program focusing on families who are at high risk for entering the child protective services system due to addiction and/or mental illness. Our goal is to identify and serve pregnant and postpartum women who have known histories of substance use, current substance use, and mothers of substance-exposed infants. We accomplish our goals by surround vulnerable mothers, infants, and families with supportive services that are sustainable and grounded in the community. We provide healing and support by addressing mental health and trauma, offer in-home services, case management, linkage to concrete supports, counseling, education, parenting, baby bonding, support groups and more. The Clinical Home Visitor will provide strength-based mental health services with high-risk pregnant mothers or parents of an infant 0-24 months old. This is a child abuse prevention program focusing at-risk parents due to addiction or mental illness. Through our collaboration with Pomona Valley Hospital, the Clinical Home Visitor will provide client engagement, home visitation, clinical assessment, individual counseling, psycho-education groups, and prenatal/postpartum workshops. Key Responsibilities Conduct outreach and engagement of pregnant or postpartum mothers at Pomona Valley Hospital, local OBGYN Clinics, or the community. Position Summary: Under the direct supervision of the assigned Supervisor, the Family Support Counselor/Mental Health Specialist has responsibility for providing support services to individuals and families as identified in the designated program and/or individualized plan. Essential Functions: Provides individual and family support services in accordance with the individualized plans. Able to effectively engage, develop relationships with appropriate boundaries with assigned clients. Effectively manages a flexible community based schedule. Completes all required documentation according to Agency and program standards within required time constraints. Is an active member of assigned treatment teams. Meets Agency service percentage standard reflecting expected time spent with clients, if applicable. Minimum Required Education and Experience: Must possess a high school diploma/GED. No previous experience required. Bilingual in Spanish preferred. Position/Program Requirements: Must possess a reliable mode of transportation to travel to assigned client homes, schools, offices, etc. If personal vehicle is the mode of transportation, must meet Agency's driving requirements. Must be physically and mentally fit. Must be willing to complete a Tuberculosis (TB) and drug screening test. Must be willing to complete a personal background investigation conducted by the State of California. Must be flexible to work a varying schedule according to operational needs. Physical Requirements: Must be able to work in an office or outdoor environment including standing, walking, and running, sitting for extended periods of time, bending, twisting, reaching, balancing, occasional carrying and lifting up to 25 pounds and up to 10 pounds frequently. Must be physically able to perform Pro-Act Restraint Techniques, CPR and First Aid as trained. Must be able to sit for prolonged periods of time in a vehicle for traveling up to 100 miles driving distance.
