Responsibilities
The Family Support Specialist provides best practice, quality clinical services, ensures all documentation is completed per Payer's, state licensing agent's, COA, & HCA's standards and established guidelines and assist with marketing for new referrals.
Communicate with customers, colleagues, and other individuals to answer questions, disseminate, or explain information, and address complaints/concerns.
Develop treatment plans with the input of the treatment team and the Clinical Supervisor
Provide individual, group, and family therapeutic interventions and skill building as outlined in the Treatment Plan
Implement Health Connect America approved therapeutic interventions as delineated on the treatment plan with clients and their families
Complete all client documentation within established time frames
Attend and actively participate in weekly Treatment Team meetings
Link the client and family with specific services and resources as identified in the treatment plan
Serve as a liaison with other professionals and agencies as related to the client
Actively recruit new referrals to ensure that your caseload stays full
Provide 24/7 on-call support for clients as per program requirements
Ensure compliance with all state regulatory bodies & COA standards.
Participate in and complete all assigned training and professional development activities, including E Learning courses.
Qualifications
A Bachelor's degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice, along with 1 year of experience working with children (including internships or volunteer experiences) is preferred, OR a BA/BS degree with 3 years of relevant experience.

JOB DESCRIPTION:
SUMMARY OF POSITION
This position provides in home visitation services to enrolled participants, providing linkage and referral to other services and support to enhance family functioning by: building trusting relationships, teaching problem-solving skills, and improving the family's support system.
ESSENTIAL DUTIES & RESPONSIBILITIES
Responsible for initiating and maintaining regular and long-term (up to five years) contact and support with families. This activity will occur primarily within the family's home; with visits lasting around an hour.
The interventions should be family-centered, strength-based, and directed at establishing a trusting relationship; strengthening the parent-child relationship; promoting healthy childhood growth and development; and enhancing family well-being by reducing risk and building protective factors.
Partners with families and honors diverse family structures and parenting practices.
Activities may also include but are not limited to: administering the Family Resilience and Opportunities for Growth Scale (FROG) Scale; administering screening tools such as the ASQ, ASQ-SE, Depression Screens etc.; identifying and referring families for other supportive services, including health care services.
Responsible for assisting the family in establishing goals and supporting them throughout this process and is responsible for implementing activities outlined on the Family Service Plan to address areas of concern that were identified on the FROG scale.
Assist in preparation of required monthly, quarterly and annual statistical reports.
Maintains up-to-date and accurate records for families on GGK curriculum, HFA data, and other required records.
Participate in training according to requirements of HFA guidelines, GKI and DOH contract.
Implements GGK curriculum and HFA strategies to enrollees through weekly home visits.
Provides case management to families on caseload.
Attend training and other mandatory meetings.

Experience in working with or providing services to children and their families.

JOB SUMMARY: Provide continuous treatment team care management services to children and youth with serious emotional disturbance or severe mental illness, their families, and other team members' consumers. Mentor new care managers on basic skills.
RESPONSIBILITIES:
Continual assessment of consumers' needs, and development of individualized service plan and crisis plan with measurable goals to ensure needs are met.
Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
Ensure consumers are seen upon discharge from the hospital and Diversion Services within Agency standards.
Ensure consumer's daily psychiatric, medical, and environmental needs are planned for and coordinated.
Ensure efficient transition of consumers when needed within the Agency and the community.
Meet productivity standards as indicated in the Policies and Procedures.
Meet regularly with Care Management Supervisor (individual and group).
Attend morning and weekly team meetings as scheduled. Follow-up with the plan(s) developed in each meeting. Attend quarterly All Care Management meetings/trainings.
Facilitate education of consumer and/or involved collaterals to psychiatric diagnosis and appropriate treatment options.
Respond to urgent/crisis consumer needs as appropriate to position with assistance of supervisor, crisis services, and clinic staff by developing plans and interventions
Develop and maintain relationships within family unit and supports.
Ensure timely and appropriate documentation as required by the Agency.

What Is the Opportunity?
Bond & Specialty Insurance (BSI) provides management and professional liability, cyber, crime insurance, and surety bonds to or on behalf of businesses of all sizes. The BSI Claim Counsel team conducts thorough investigation, analysis, evaluation, and disposition of claims and claim litigation to achieve superior customer service and to optimize claim results. Travelers' Cyber Claim Team is responsible for handling claims focusing on cyber privacy and security breach incident response, computer crime, infrastructure attacks and restoration, notification to affected parties and regulators, business interruption, consumer privacy liability, and identity fraud/theft.
