The Patient Access Representative II is responsible for efficiently and accurately processing patient registrations, scheduling appointments, and verifying insurance coverage. **ESSENTIAL JOB DUTIES/RESPONSIBILITIES:**
+ Interacts appropriately with patients and records information while greeting, checking-in, checking-out or scheduling appointments for patients.
+ Utilizes computer equipment and programs to add new patient information, prints out relevant forms for processing and schedule appointments. Ensures patient contact and emergency contact information is accurately documented in the system.
+ Answers telephones, takes detailed and accurate messages, and ensures messages are directed to the appropriate individual.
+ Clearly communicates information about ChenMed clinical personnel to patients and other individuals.
+ Verifies patient insurance via phone or website and collects any necessary copays for services. Assists with scheduling follow-up and in-house Specialist referral appointments, when necessary.
+ Escorts patients to exam rooms and ensures their comfort and well-bring prior to leaving.
+ Acts as HEDIS champion for the PCP by providing visibility of HEDIS needs for scheduled patients.
+ Presents patients with customer service survey during check out and escalates if needed for immediate service recovery.
+ Participates in daily and weekly huddles to provide details on patients.

Job Description:

Assist patients in providing required registration information, including demographic, insurance and clinical data, as necessary.
Support clinical staff and clients by collecting assessment measures at regularly scheduled intervals.
Update client information in the customer service database during and after each interaction.
Collect and audit departmental data on a weekly basis to investigate and assess current processes and areas for improvement.
Support LOA's clinicians through thorough and timely communication and assistance with any questions.
Handle confidential information consistent with Federal HIPAA legislation, and all other applicable federal, state, local, and institutional regulations with regard to confidentiality
Ensure problem resolution and corrective action for long-term solution, coordinating such efforts across departments.

General Information for all Pool positions
The Pool season is from May - August 2026
Hours vary between 8:00am and 9:00pm depending upon programs and events. Shifts may vary.
Candidates will be interviewed and may be selected for a pool position based upon candidates' qualifications.
The pool environment is an outdoor setting. Staff will be exposed to weather conditions, including cold, rain, and extreme heat. Inclement weather is monitored daily and safety precautions taken when necessary.
