Position Summary: Homemakers (HM) services are provided to clients who require physical assistance and verbal queuing with homemaking tasks. The goal of services is to maintain a client's optimal level of functioning while maintaining dignity and independence. Services include client assistance with shopping, menu planning, laundry, general household tasks such as meal preparation and general household care when the client is temporarily, or permanently, unable to perform such tasks for him/her self or others in the home. The Aging Services Access Point Registered Nurse (ASAP RN) will assess the client's needs, the type of and amount of care assistance needed, the client's environment and support system in determining the appropriateness for services. The ASAP RN collaborates with the RN to ensure an individualized, comprehensive and effective plan for each client. The RN supervises and orients the HM to the service plan and the client. In-home Nurse Supervision is conducted not less than once each quarter. Licensed Practical Nurses may supervise and orient the HM as allowed by the Elder Affair's Provider Agreement. Education, Experience and Qualifications: Successful completion of an approved 40- hour HM training program in accordance with Federal Regulations. A current, valid certificate must be presented upon hire. A Home Health Aide, certified in another state or territory of the United States, may be deemed to have completed a training program as required by the Commonwealth of Massachusetts provided the Home Health Care Agency has sufficient documentation in accordance with the ASAP's provider agreement.
