ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Creates a resident-centered approach to the planning and delivery of services to residents, supporting residents' rights to participate in decision-making and treating residents with dignity and respect.
2. Performs assessments on health, functional and psychosocial status of residents. Participates in the development of individualized service plans. Proactively manages care and services for each resident, maximizing the resident's opportunity to remain in the assisted living environment. 5. Ensures that resident care needs are met by coordinating services with other professionals, including physicians, discharge planners, nurses, home health agency staff, case managers, hospice, and state agency personnel. 6. Communicates regularly with residents' families regarding health care needs. Provides education and support to residents and families.

* Acts as the lead cook during assigned meals. Prepares, cooks, and serves food in accordance with the menu and food and nutrition standards.
* Manages the department budget including spend downs, expense reporting, managing food and supply cost and labor management to PPD.
* Implements dietary changes as directed thru PCP Diet Orders. Ensures all dining services team members are aware of diet changes and that all dietary entries in the resident records are accurate and timely.

* Keeps Executive Director and department heads up to date on changes in the dining services department.

Responsibilities
Complete required record keeping in regards to your role in admitting, transferring and discharging residents.
Coordinate with nursing staff and support personnel daily to plan the shifts' services, programs and activities to ensure the resident's total regimen of care is maintained.
Prepare and administer medications and review medication cards for completeness and accuracy.
Review resident care plans for appropriate resident goals, problems, approaches and revisions based on nursing needs.
Plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures that are necessary for providing quality care.
Provide leadership to nursing personnel and make daily rounds of your unit to ensure staff is providing direct care in accordance with resident's care plan and wishes.
Develop and maintain rapport with nursing staff, other department personnel, residents, family members, and all other individuals involved in resident's care.
Participate in facility surveys by authorized government agencies.
Qualifications
Must possess a nursing degree from an accredited college or university.
Must possess a current, unencumbered, active license to practice as an LPN in this state.
Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities.
Must chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident and resident's response to care.
Experience with Electronic Medical Records and computer documentation systems.
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
Must speak, read, and write English fluently
Must have an active CPR/BLS certification

Essential Duties and Job Responsibilities:
Oversee and coordinate all aspects of resident care, including daily activities, personal care and medical needs.
Develop and implement individualized care plans based on resident needs and preferences.
Develop methods for coordination of nursing services with other resident services to ensure the continuity of the resident's total regimen of care.
Maintain open and effective communication with residents, families, staff and other healthcare professionals.
Ensure compliance with all applicable regulations and facility policies and procedures.
Monitor and evaluate the quality of resident care to ensure it meets standards and expectations.
Train, supervise and evaluate direct care staff to ensure they provide quality care.

Participate in state and federal surveys of the facility; assist the Administrator in reviewing deficiencies and developing plans of correction.
Collaborates with the Medical Director to assure quality of care.
Attend continuing education programs designed to keep you abreast of changes in your profession and participate in/provide leadership for in-service training for nursing

Requirements and Qualifications:
Must possess a nursing degree from an accredited college or university.
