The CNA should notify the case manager or clinical supervisor of any deviations from the Activity Plan.
The CNA should report changes in patient's condition, including pain control to case manager or clinical supervisor within one hour of assessment
The CNA should accurately complete appropriate clinical records related to assigned duties and observations of patients within 48 hours of the provision of care
The CNA should work effectively as a member of the interdisciplinary team and contributes to plan of treatment based on knowledge and experience.
Requirements
CERTIFIED NURSING ASSISTANT QUALIFICATIONS:
Satisfactory completion of training program or competency test for Home Health aide which meets the requirements of the State of Wisconsin Department of Health as well as Medicare (CNA).
Home health and/or hospice experience preferred.
Current Driver's license.
Name is listed on the Wisconsin registry as current and eligible to provide home health and hospice care.
Excellent communication skills.
Evidence of attendance of hospice training class or willingness to attend.
Believes in and has a working knowledge of the hospice philosophy of care.
Basic Computer skills.

What You'll Do
The Medication Technician/Medication Passer is a leader and a role model for the shift and is responsible for maintaining a safe and secure delivery of medications. The Medication Technician/Medication Passer is key in developing, coordinating, and performing the tasks on the Growth and Wellness plan according to the elder's preferences and abilities. Leadership responsibilities include ensuring Care partners complete their assignments and supporting and guiding them.
As a Medication Tech/Med Passer, you can expect a range of responsibilities and tasks. Here are some general expectations:
You will pass medication and assist with self-administration of medication as required by state law.
You will guide and support the Med Techs on their shift.
You will lead the Care Partners on their shift as a role model of kindness, accountability, and collaboration.
You will participate in interviewing new Care Partners and Med Techs as needed.
You will provide the orientation of new Care Partners and Med Techs.
You will provide training and re-training of Care Partners and Med Techs as required by state regulations or as needed to maintain skills.
You will coach team members to be the best they can be in collaboration with the Administrator.
You will work in collaboration with the RCC and Administrator for scheduling.
You will supervise the storage, distribution, and discontinuation of residents' medication.
You will maintain continued compliance with the policies and procedures of Compass Senior Living, including those as outlined in the Quality Counts Policy and Procedure Manual.

You will be helping our patients live well by:
Prepares and rooms the patient for exam by obtaining vital signs and gathering/documenting/updating pertinent health information (i.e., chief complaint, allergies, and/or medications). Monitors and completes Clinical Integration/ Quality Improvement measures with notification to clinician for opportunities within their scope.
Performs laboratory procedures (i.e., strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions/infection control guidelines.
Assists the Physician/ Advanced Practice Clinician (APC) with procedures such as pelvic exams, allergy scratch testing, EMG, cautery, colposcopy, etc.

Follows through with necessary procedure or test requests, pre-approved care algorithms, new appointment times and referrals to other facilities or services. Refers questions to Registered Nurses and Physician/APC per scope of practice guidelines and relays information back to patient as directed.
Communicates with Physician/APC and other members of the health care team to ensure smooth clinic flow and adjusts as necessary. Effectively communicates accurate and timely information with the patient.
May perform basic and advanced clinical support tasks or skills based on the specialty and appropriate competencies such as but not limited to removal of sutures and staples, laboratory procedures (throat/nose culture, drug screen), phlebotomy - butterfly, venous heel stick, EKG, spirometry, Holter monitor application, prescription refills using approved protocols, etc., under clinical supervision.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. OVERVIEW
Our Clinical Care Staff will primarily be responsible for providing compassionate care and services to our residents in accordance with each resident's service plan. RESPONSIBILITIES
Caring for residents in the community by adhering to all community policies and procedures.
Reviewing all tasks before performing cares to ensure updated cares are being completed.
Assisting residents with Activities of Daily Living (ADL's) to include but not limited to: bathing, showering, dressing, grooming, shaving, nail care, hair care, eating, making bed, laundry, ambulating, and helping to activities and other functions.

Maintaining daily records by documenting tasks completed in the resident electronic medical record and notifying the Clinical Care Team of any changes.
Monitoring the Clinical Care Call System for emergency calls and responding appropriately.
Assisting all community staff, as needed, if own tasks are completed.
Providing effective and courteous service to all residents, guests, and co-workers.
Administering medications as ordered by physicians and documenting medications administered on MAR.
Signing in medication deliveries by the pharmacy.
Assisting fellow team members in maintaining a clean, homelike environment that is free from hazards by tidying up spaces, cleaning up after residents, and ensuring that they always look presentable.
Obtaining resident vitals.
Informing Management Team Directors of unsafe conditions in the building and/or reporting any suspected or actual abuse/neglect immediately.
Completing daily cleaning task as assigned.
Arriving for scheduled shifts on time.
Following protocol for calling-in or trading shifts.
