A day in the life of a Patient Care Coordinator:

Position Summary:
The Patient Care Coordinator - I (PCC-I) supports clinic growth through excellence in execution of the practice management role and patient intake processes. This individual will work in collaboration with the Clinic Director (CD) to carry out efficient clinic procedures. The PCC-I position is responsible for supporting the mission, vision, and values of Upstream Rehabilitation.

Physical Requirements:
This position is subject to inside environmental conditions: protections from weather conditions but not necessarily from temperature changes; exposed to noise consistent with indoor environment.

Support ongoing in-service education for Memory Care team members related to dementia care, disease processes, and best practices.
Collaborate with Dining Services to ensure individualized dining preferences and needs are met; actively participate in the dining experience with residents.
Work closely with community leadership to ensure Memory Care operations align with state regulations and company standards.
Assist with developing specialized approaches and programming tailored to residents with Alzheimer's disease or related dementias.

Customer Service Representative (CSR) serves as the first impression of Classic Collision's customer experience.
Responsibilities
* Explain and educate customers on repair process on a high level, including insurance claim information and processes.
* Maintain a broad knowledge base of insurance partner requirements regarding authorizations, billing, and load level processes.
* Coordinate Rental Car /Tow companies to provide one-stop service to all customers.

Summary Description:

A typical LifeVest patient is in the hospital awaiting discharge, and the Patient Service Representative sets up the equipment and trains the patient and caregivers on the use and care of the device. They also follow up with patients as needed to assure patient understanding and satisfaction. PSRs retain the flexibility to accept or decline assignments as their schedules dictate. Most assignments can be scheduled during free time - before/after work, and on days off.

Travel to patient's homes and health care facilities to provide services
Train the patient and other caregivers of patient (if applicable) in the use of LifeVest
