Summary:
The Emergency Room Registered Nurse (ER RN) delivers rapid-response, high-quality patient care in a fast-paced emergency department setting. Essential Work Functions:
Rapidly assess and triage patients upon arrival, prioritizing care based on the severity of their condition
Explain emergency procedures and treatments to patients and families
Administer prescribed medications and treatments in accordance with approved emergency nursing techniques and protocols
Prepare equipment and aid physicians during emergency treatments and examinations
Monitor patient comfort and safety throughout their time in the emergency department, responding to immediate needs
Observe and document patient conditions as required within scope of practice
Take and monitor vital signs using clinical judgment to address deviations and prevent complications
Respond to life-saving situations based upon emergency nursing standards, policies, procedures, and protocols
Document comprehensive nursing assessments, interventions, and outcomes in electronic medical records (EMR)
Initiate patient education plans according to individualized needs, considering the acute nature of emergency care
Collaborate effectively with interdisciplinary teams including physicians, specialists, paramedics, social workers and other support staff to ensure comprehensive care
Perform other duties as assigned within the scope of practice
Adhere to hospital safety protocols, infection control guidelines, and regulatory standards

The Case Manager possess social service and engagement skills and an ability to promote a culturally sensitive, performance-driven culture to meet child welfare programmatic goals.
Essential Functions:
Supports the case management supervisor in promoting efforts to achieve team and performance goals
Maintains a caseload of children and families and ensures dependency milestones are achieved timely and with a high degree of quality.
Accepts cases assigned by unit supervisor and assesses the safety of children in their primary residence within two working days of case transfer staffing and with identified parents. Completes a Family Assessment within 15 working days of case transfer staffing.
Negotiate and develop a case plan through Family Team Conferencing based on identified strengths and needs of the family, the circumstances bringing the family into care, recommendations provided by the Comprehensive Behavioral Health Assessment and other relevant data. This plan is to be created in collaboration with the parents, Guardian Ad Litem, foster parents and other pertinent parties with the goal of reunification or other permanency for the child.
Visits the child in their primary residence according to required frequency, but no less than once every 25 days. At least one visit each quarter is to be unannounced.
Accurately documents all case activities in the Florida Safe Families Network database within 48 hours. Maintain the physical case file in chronological order, by subject, in accordance with the standardized case file format.
Submits service requests to the lead agency and provides clients with timely referrals to services. Develops and maintains knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources. Maintains regular contact with service providers and documents service progress in FSFN.
Complete and submit court documentation within required time frames. Prepares for, attends, and participates in all court activities as necessary.
Arrange for, attend, and participate in individual case staffings as necessary. Completes and presents at all required staffings.
Conduct initial and/or ongoing child safety assessments as required. Prepare initial and on-going safety plans as necessary.
Arrange for emergency placement, emergency medical treatment, and emergency services for children at risk.
Conduct diligent searches for parents and family members when deemed necessary and thoroughly document that the effort has been made to find the parents and family members.
Conduct home studies as required for prospective placements.
Provide relevant medical, psychological, behavioral and educational background information about the child or children to prospective care-givers as needed.
Plan and facilitate parental and sibling visits as needed and appropriate.
Transport and supervise children as needed.
Ensure that all Independent Living functions are completed as required
Attend all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor.
Function as agency on-call Case Manager as scheduled.
Organize, prioritize and complete all work assignments by the established deadlines.

The Registered Nurse Case Manager will practice within the scope of his/her licensure.
Review all medical data which can be provided to establish, update and maintain accountability for a case management plan which will incorporate contact with providers, payers, with the patient and with the patient's primary caregiver.
Assess problems and determine goals and actions designed to meet the needs of the patient and document into the case notes. Determine if these goals are long term or short term and how the patient can be expected to meet those goals. Include the action/intervention the case manager will take to work towards achieving those goals.
Make contact with the payer office to find out and understand any benefit constraints that will have an impact on the plan of action.
Proceed with contacting medical care providers and with equipment vendors to verify medical necessity of care or equipment that has been ordered.
Make care arrangements for quality patient care according to the needs of the patient, the physician's orders and the benefits available.
The Case Manager will work in conjunction with the Case Management Assistant to manage case management files, exclusive of Assessment and/or Care Plan activities, and will provide input in the Annual Performance Evaluation of the CM Assistant assigned. The Case Manager will maintain responsibility for the Case Management file.
Be aware of any alternative treatment possibilities that may allow the patient to reach wellness goal(s). If there are no benefits available for your recommended alternative treatments, provide to the payer a cost-benefit analysis to demonstrate that extra-contractual services will enhance the patient's medical condition and will be cost-effective to the benefit plan.
Become familiar with community resources and funding sources so that the patient can receive quality health care and conserve health benefit dollars. Many agencies exist which provide assistance to persons in financial need or to provide information to persons with specific medical conditions.
Maintain case in computer system documenting case actions for each patient under your case management. Complete all aspects of case in the computer. Prepare timely reports to the payer to detail all case actions, the results of those actions, and the continuing case management plan.
Maintain billing as appropriate in computer system.
Continue to maintain contact with the providers and with the patient across the continuum of care to be sure that patient needs are being met. On any cases which include a chronic condition keep the file open for periodic contacts to verify the clinical status of the patient and additional medical needs.
Negotiate with providers to maximize the medical benefits available to the patient. Make network referral as appropriate.
Act upon any awareness of non-medical issues which involve the patient's safety or welfare. Attempt to direct the patient or family to appropriate providers or community resources, or to personally notify appropriate authorities. Consult with the CM supervisor on a regular basis, and keep the supervisor informed regarding any complaints which may occur about case management services or any issues which arise which the case manager is not competent to handle or does not have the expertise to handle.
Adhere to all company policies as stated in the employee handbook.
All case managers will possess a URAC-recognized certification in Case Management within 3 years of hire. Participate in the Quality Management Program by adhering to all company policies and procedures and identifying opportunities for improvement to ensure quality services are rendered to clients and customers.
This position is eligible for a bonus program.
The salary range for this position is from $72,000 to $81,000 annually.
Work Environment / Physical Demands: This position is in a typical home office environment which requires prolonged sitting in front of a computer. The Clinical Manager (CM) is responsible for managing the delivery of quality clinical services within an assigned group of patients. The CM ensures that all assigned caregivers receive standardized, comprehensive and effective. The CM partners with the Director of Nursing (DON) and Director of Operations (DOO) in identifying, interviewing, and onboarding caregivers; Ensures the completion of a comprehensive assessment upon admission, recertification, post hospitalizations and PRN for all assigned patients
The CM will ensure training for all assigned caregivers. The training will be standardized, comprehensive and effective.
Family First Homecare team members represent the organization and its services in a competent, professional and responsive manner while maintaining standards of high-quality patient care and customer service in accordance with all state, local and federal requirements, rules and regulations.
KEY RESPONSIBILITIES
Partners with the Director of Nursing (DON) and Director of Operations (DOO) in identifying, interviewing, and onboarding caregivers
Provides supervision, leadership, training, guidance, and mentoring for all assigned caregivers
Collaborates with the Family First leadership teams and the compliance committee in identifying, developing, and the delivery of consistent clinical information
Evaluates assigned patient's appropriateness for homecare
Ensures the completion of a comprehensive assessment upon admission, recertification, post hospitalizations and PRN for all assigned patients
Ensures a current physician order, plan of care/ care plan is place for all assigned patients
Manages the patient care by supervision and care coordination for all assigned patients and including the disciplinary team
Ensures that the competency program is followed and provides educational support for the caregivers as needed for skills development
Promotes employee engagement and retention
Participates in the review of policies and procedures to ensure compliance with all State, Federal, and Accreditation standards
Participates in the quality assurance performance improvement (QAPI) program
Assists in the coordination and oversight of all plans of correction and quality initiatives as directed by the DON
Partners with the DON and DOO to lead cohesive teams toward common goals
Assists management with the development and implementation of new products and services
Maintains professional, supportive, and responsive interpersonal communication skills
Performs other duties as assigned
ROLE QUALIFICATIONS
EDUCATION & EXPERIENCE REQUIRED
RN Nursing Degree. Unencumbered RN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognized
Registered Nurse with 1 year of clinical experience
Valid driver license with proof of insurance
Working knowledge of home care regulatory and federal requirements
Proven customer service skills, sound independent judgment, initiative and solid organization and time management skills required
Strong computer skills to include Microsoft Office Suite
Ability to travel to Family First office locations and patient homes throughout designated area
PREFERRED
1 year of supervisor experience
Excellent organization, problem-solving, decision-making, presentation, and communication skills
Possess outstanding leadership, teaching, and training skills
Forward thinker with the ability to be creative and “think outside the box”
STATE SPECIFIC REQUIRMENTS:
Florida: N/A
Illinois: Required: 1 year of pediatric experience
Pennsylvania:
Additional Background Screening: Child Abuse, and FBI check.
Must complete TB screening with 2 step PPD, or 1 step PPD with proof of 2 step PPD administration within the last year.

WORKING CONDITIONS/EQUIPMENT USE
Work is performed indoors in a typical office environment - not substantially exposed to adverse environmental conditions.

Travel may be required by car or airplane to local, out-of-town, or state seminars, conferences, meetings or other branch office locations.
PAY PHILOSOPHY:
The typical starting salary range for this role is determined by a number of factors including skills, experience, education, certifications and location. The full salary range for this role reflects the competitive labor market value for all employees in these positions across the national market and provides an opportunity to progress as employees grow and develop within the role. Some roles at Family First Homecare have a corresponding compensation plan which may include a bonus at rates that vary based on multiple factors set forth in the compensation plan for the role.
KEY DETAILS:
**This position requires the employee to work in the office Monday through Friday during core business hours.** Core business typically fall between 8:00 am to 5:00 pm with some flexibility with start and end times.
