Provide quality care through implementation and evaluation of individualized patient care plans, discharge planning and complex outpatient needs.

Administer medications according to policy and procedures, to include evaluating for response and effectiveness, and document accordingly.
Administer medications, which include controlled substances, in accordance to physician orders and policy and procedures.
Destroy medications that are considered narcotic in nature and all other medication that is no longer being used in accordance with policy.
Communicate to the IDT all pertinent information pertaining to the patient and family unit, actively to participate in review and revision of patients' plan of care.
Documentation should include but not limited to variance in patient condition, interventions and outcomes and address progress towards goals set by the patient's plan of care.
Documentation should include education provided to the family and the patient, including topic, which was taught and their understanding of the education.
Supervise C.N.A.s and LPNs
Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each.
Participate as an active supporter of the Treasure Coast Hospice compliance program, including the emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization's compliance program.
