Following a client referral, coordinate the application process for all clients applying to treatment services.
Ensure the appropriate recommendations are being made for applicant referrals.
Assist and coordinate the application review process for clients with legal, mental health or medical concerns that need to be resolved before the intake process begins. Follow up on any required paperwork through county, courts, etc. as needed.
Stay in regular communication with clients throughout the entire application review process.
Clearly document every interaction with clients.
Provide a positive client experience to incoming referrals.
Manage crisis situations, as they arise.
Handle a client caseload of approximately 100 referrals.
Required Skills & Experience (Minimum Qualifications):
High School Diploma or GED required.
2+ years of relative experience in a customer focused role in a professional setting. Previous experience with mental health and/or addiction preferred.
At least 2 years of freedom from problematic substances.
Must be comfortable working with people with life-controlling challenges, including mental health and addiction.
Strong organizational skills and attention to detail, with the ability to effectively manage time.
Excellent communication and interpersonal skills to interact with clients, staff, and leadership.

Participate with other health care providers in the development and modification of a plan of care.
Determine

We are seeking a Field Nursing Coordinator/ LPN to join our team! This position is a part time position and will ensure that the health care needs of all clients are met. The Field Nursing Coordinator will ensure that staff is trained in Olydia Care Private Duty Nursing procedures and assisted living procedures as well as policies which are current and up to date. The Field Nursing Coordinator will also ensure that the Assisted Living is in compliance with current Assisted Living License regulations. Working cooperatively with the Director of Nursing, answering Directly to the D.O.N, supervises the delivery of services provided to the clients and supervises staff. This is an Administrative Position, which would require the you to be on call for 4 hours a day.
ESSENTIAL JOB FUNCTIONS AND TASKS:
Manage the Assisted Living services that are provided including completing essential
paperwork and complying with state regulations.
Works to assure the facility is in compliance with federal state and local standards and
regulations.
Works with Assisted Living Director to keep forms updated including policies,
procedures, Bill of Rights, and related assisted living forms and manuals.
Meets with Case Workers regularly, as needed.
Maintains an up-to-date knowledge of current trends regarding Assisted Living facilities
Is responsible for investigating and reporting incidents of suspected resident neglect
and/or abuse and reporting these findings to the director of Nursing, and/or appropriate
county officials according to Olydia Care procedures and state regulations.
Works to maintain privacy and confidentiality of client, clients records and client living
environment.
Is willing to carry a cell phone and be "on call" for emergencies, resident change of
condition, and when ULP perform delegated nursing tasks.
Prepare, record, submit and maintain accurate and timely correspondence and
paperwork regarding resident data and billing information with the Office Coordinator as
needed.
Communicates and documents resident data to staff as needed.
Completes initial assessment of potential clients to assure that their needs will be met.
Follows the Assisted Living License regulations for providing timely and thorough nursing
assessments for all clients.
Assures clients maintain residency requirements, reassess and recommend alternative
placement if necessary.
Implements service agreements with all new clients. Reviews and revises service plans
as needed.
Assures clients have up to date care plans. Reviews and revises care plans as needed.
Monitors and maintains resident records and nursing notes.
Communicates to physicians and other personnel (county caseworkers) any changes in
clients needs or conditions.
Monitors and follows thru with ordering medications and all medication renewals as well
as any necessary follow-up with physicians regarding medications and/or medical
conditions.
Assures all physicians orders are signed and implemented with-in 24 hours.
Fills weekly pill boxes for medication administration by unlicensed staff and checks new
medications from the pharmacy.
Assured new medication sheets are in place by the first of each month.
Reviews each clients medication sheet at least monthly and assures accuracy and
appropriate signatures.
Communicates in staff communication books any information that staff needs to be
aware of.
Documents disposal of medication per procedure.
Assures compliance with regulations and is available for federal and state licensure
surveys.
Assists in the development of policies and procedures and implementation of them.
Responsible for management of Tuberculosis / Infection Control Program.
Supervision of other nursing and Unlicensed Personnel.
Supervises, orientations and competency tests unlicensed staff to assisted living and to
each clients individual needs.
Gives input and/or helps perform annual evaluations for nursing and unlicensed
personnel staff.
Supervises, disciplines, and terminates unlicensed personnel.
Coordinates staff education to include at least 8 hours of in-services per year.
Meets with staff as needed.
Provides health and medication training to all new employees as well as continued
education for all staff and documents such training to assure compliance with state,
federal and local regulations.
Assures that staff clearly understands and follows all assisted living procedures.
Monitors staff for compliance of monthly resident treatments as scheduled.
Supervises medication practices and medical equipment being used by assisted living
clients.
Assures that medication administration procedures are followed properly and discusses
any needed corrections, errors, etc., with staff.
Provide support and address needs for programs and services.
Maintain resident records and appropriate documents as required.
Assess clients on an ongoing basis to assure their needs can be met.
Assures clients continue to live safely in their unit, reassess and recommend alternative
placement if necessary.
Communicates the services to be provided for each resident to staff responsible for
delivering the services, and assures follow through.
Inform personnel of changes in clients needs and conditions.
Maintains communication with clients, their families and staff.
Facilitates coordination of resident services with other service providers as needed.
Is available for on-call medical questions and/or emergencies via phone.
Participate and function effectively as a team member.
Develop and maintain a positive working relationship with staff, and other Morgan House
staff including providing backup to other staff as needed.
Articulate information regarding clients to the team.
Provide proactive, constructive participation in staff meetings, committees, etc.
Assists other departments with special events and family functions.
Be compassionate and work with tact and ethical awareness.
Be flexible and adaptable to changing situations.
Responds appropriately to safety hazards, fire drills and other emergency situations.
Reports injuries for self, staff, clients or visitors immediately to the Director of Nursing.
Keep the Director of Nursing informed and involved and advised of needs and problems.
Other duties as assigned.

RESPONSIBILITIES
Complete all preoperative surgical calls (Ambulatory Surgery Center (ASC)).
Ensure surgical chart is complete (H&P, blood work, EKG, IOL pick, surgery order).
Triage patient calls regarding ocular concerns. Determines clinic visit necessity.
Document all calls using the Patient Communication log in the Electronic Health Record (EHR).
Review all pathology reports; contacts patients and/or provider with results as directed and records communication in EHR.
Complete medication refill requests obtained via fax, phone call or patient portal.
Answer patient calls and/or portal requests to provide education, answer questions, etc. to ensure superior patient care.
Send tasks via EHR when necessary to enlist the assistance of other team members.
Complete same day post-operative surgical calls.
Participate in trainings and meets requirements for Merit Based Incentive Payment System (MIPS) measures, HIPAA compliance and Protected Health Information.
Perform all patient related duties as assigned to ensure quality patient care.

MN Eye Consultants is currently hiring for a full-time Nurse Liaison for our Ambulatory Surgery Center in Minnetonka.
SUMMARY
The Nurse Liaison ensures that surgical patients are medically optimized for their surgical procedures. This includes providing support to the providers by answering surgical patient calls, e-prescribing medications, triaging patient concerns, answering general patient phone calls and other medical administrative responsibilities such as patient paperwork to ensure superior patient care. Starting pay range is $28.00-31.00/hour.
RESPONSIBILITIES
Complete all preoperative surgical calls (Ambulatory Surgery Center (ASC)).
Ensure surgical chart is complete (H&P, blood work, EKG, IOL pick, surgery order).
Triage patient calls regarding ocular concerns. Determines clinic visit necessity.
Document all calls using the Patient Communication log in the Electronic Health Record (EHR).
Review all pathology reports; contacts patients and/or provider with results as directed and records communication in EHR.
Complete medication refill requests obtained via fax, phone call or patient portal.
Answer patient calls and/or portal requests to provide education, answer questions, etc. to ensure superior patient care.
Send tasks via EHR when necessary to enlist the assistance of other team members.
Complete same day post-operative surgical calls.
Participate in trainings and meets requirements for Merit Based Incentive Payment System (MIPS) measures, HIPAA compliance and Protected Health Information.
Perform all patient related duties as assigned to ensure quality patient care.
Other duties as assigned.
EDUCATION AND EXPERIENCE
LPN or RN, experience preferred
Open and willing to try new workflows and ideas.
Ability to remain calm at all times and escalate issues to appropriate levels of management.
Comprehensive knowledge of EHR system and ability to use it efficiently.
Knowledge of medical terminology.

Provide clinical supervision, mentorship, and education to nursing staff. Coordinate the delivery of all care and services appropriate to the needs of the patient using an interdisciplinary team approach. Oversee day-to-day operations to ensure customer needs are met while maintaining compliance with federal, state and Joint Commission requirements.
Responsibilities
* Manage the day-to-day operations of the Nurse Clinicians including care coordination, staffing, documentation, and communication.
* Provide direct supervision of the Nurse Clinicians and Nursing Coordinators.
* Complete timely performance reviews of staff according to agency policy including joint home visits
* Maintain the productivity level of the nursing staff to maximize efficiency, assure adequate staffing levels, and control costs; approve timecards.
* Assist Nurse Manager as directed in process improvement projects
* Function as a clinical resource providing infusion nurse clinician expertise to the FHI team members and patients.
* Identify conditions or circumstances that impair the ability of the patients/caregivers to successfully manage the prescribed therapy.
* Provide education to patients/caregivers that enable them to self-administer medications and solutions safely and effectively.
* Understand drug therapies for therapeutic outcomes, adverse reaction, and compliance.
* Maintain and submit complete, accurate and timely clinical documentation on all patients
* Triage telephone calls from patients and provide appropriate follow-up using established policy/procedure and standards of practice.
* Participate in process improvement and committees as assigned
* Coordinate and manage external and internal relationships with regional nursing agencies and/or FHCH as assigned
* Identify and contract with nursing agencies that will provide quality nursing care to patients who receive infusion therapy services from Fairview Home Infusion
* Ensure that each contracted nursing agency complies with accepted standards of care and state licensure requirements
* Oversee and coordinate appropriate clinical follow-up for patients
* Responsible for authorization of visits made by nursing agencies and Fairview affiliate home care agencies
* Establish a point of care for regional patients based on patient input and nursing assessment
* Organize information and communicate effectively with other disciplines, departments, and patients
* Triage calls from patients, caregivers, physicians, and referral sources related to all aspects of infusion therapy
* Take telephone orders from MD. Communicate order(s), write order(S) and function as a liaison between staff, physicians, and pharmacy
* Demonstrate teamwork and effective communication
* Serve as a resource to the affiliations, staff, and physicians regarding pharmacy services. Assist with coordination of services by communicating effectively and in a timely manner with other teams, disciplines/departments.
Required Qualifications
* B.S./B.A. in nursing
* 2 years of recent nursing experience in an acute care setting, preferably in critical dare, oncology or med/surg
* MN Registered Nurse (RN) License
* Basic Life Support (American Heart Assoc or Red Cross)
Preferred Qualifications
* One to two years of previous home infusion therapy
* Supervisory experience
* Certified Registered Nurse Infusion (CRNI) Certification or
* Oncology Certified Nurse (OCN) Certification or
* Critical Care Nurse (CCRN) Certification
